Please read the customer the short disclosure statement that appears on the screen. Be sure to include the applicant s full name, address and social security number to avoid inaccurate or unavailable IBS information. 5 Utah Automobile 4 Verification Policy File Maintenance and Verification Please refer to the file retention guideline available on QBE s website. Application The signed application must be retained in the agent s file. Claims History/MVRs QBE uses LexisNexis Comprehensive Loss Underwriting Exchange (CLUE) reports and/or Motor Vehicle Reports (MVRs) and/or Explore Information Services to verify each driver s accident and violation history. For inforce policies we also use information from QBE s records, including insured questionnaires, to verify driver accident and violation information. Note: Full disclosure is essential in order to avoid premium conflicts or policy cancellation. Any unrevealed driving activity will be added to the driving record and the policy will be re-rated accordingly. Prior Insurance and Prior Coverage Limits Information about prior insurance and prior coverage limits is required. This information includes: Name of prior insurance carrier. Length of insurance coverage with prior carrier. Bodily Injury Limits on prior policy. Any days lapse in prior coverage.
