1 DRIVER RULES AND DEFINITIONS D-1 Use Classifications Defined 1. "Pleasure Use" means there is no business use of the automobile and it is not used in the course of driving to or from work or school a distance of more than 3 road miles one way. If driven 3 or more road miles, but less than 15 road miles one way, not more than 2 days per week or not more than 2 weeks in any 5 week period then pleasure use would apply. 2. "Work or school less than 15 miles" means there is no business use of the automobile, but it is customarily used in the course of driving to or from work or school a distance of more than 3 road miles, but less than 15 road miles one way. If driven over 15 miles one way not more than 2 days per week or not more than 2 weeks in any 5 week period then work or school less than 15 miles would apply. This includes an automobile driven part way to or from work or school, such as to a railroad or bus depot, whether or not the automobile is parked at the depot during the day. 3. "Work or school 15 or more miles" means there is no business use of the automobile, but it is customarily used in the course of driving to or from work or school a distance of 15 or more road miles one way. "Business use" means the use of the automobile is required by or customarily involved in the duties of the applicant in his occupation, profession or business, other than in going to or from his principal place of occupation, profession or business. 5. "Farm use" means the automobile is principally garaged on a farm or ranch and it is not customarily used in any occupation other than farming or ranching or driven to or from school. D-2 Driver Averaging 1. Driver class factors do not apply to Antique, Restricted Use Classic, Motorhomes, Travel Trailers, Other Trailers, or Non-owned Auto. 2. Calculate class factor for every rated driver for every coverage. Unlicensed, non-rated driver MVRs, and excluded drivers are not included in the calculation. 3. Sum the factors for all drivers by coverage and divide by number of drivers. 4. Result is the factor used - by coverage - in the rating methodology for all PPV's on the policy. Example: Driver 1 BI factor =.9600 Driver 2 BI factor = BI factor used for all regular use PPV's = For extra vehicles (vehicles with no assigned driver) the average class factor will still be used. D-3 Classification Definitions 1. "Driving safety record" means: a. The public record of traffic violation convictions available from the Department of Motor Vehicles for the preceding 36 months, together with similar public records of traffic violation convictions in the preceding 36 months that are available from other jurisdictions; and b. The at-fault accidents experienced by the rated driver of the insured vehicle, resulting in bodily injury in excess of $1,000 or death (regardless of the dollar amount of such loss); or resulting in damage to any property, including his own, in excess of $1, "Experience Period" is the three years immediately preceding the policy's original inception date, or its renewal date. For the following activity, the experience period is five years: a. Driving under the influence b. Driver without a valid license c. "Driving Safety Record Points" are the point(s) assigned based on convictions and accidents (See D-4 Merit Rating Plan) 4. "Good Student" means: a. Operators under age 25 (rel goes to1.0 at 25) who are full-time students in high school or enrolled as full-time students in academic courses in a college or university. A full time college or university student is defined as a student taking a minimum of 12 credit hours of classes per quarter or comparable period, and b. The company is furnished a statement (refer to company for form) certified by a school official indicating that the student has met one of the following requirements for the immediate preceding school year. This documentation is required once when requesting the high school GSD and after the first year at a college or university (after age 19): (1) Ranked among the upper 20% of his or her class scholastically; or (2) In schools using letter grades, achieves and maintains a grade point average of B or better or, if the system of letter grading cannot be averaged, no grade is be below B; or (3) In schools using numerical grade points, achieves and maintains a grade point average equivalent of B or greater, such as at least 3 in a 4, 3, 2, 1 point system; or (4) Included in a Dean's List, Honor Roll, or similar list indicating scholastic achievement, or (5) For home schooled children a copy of the certification showing the child is registered for home schooling is required, along with a copy of the most recent testing results showing the child is satisfactorily passing the required testing. c. A classification change resulting from a change in the scholastic standing of the student cannot be effected between anniversary dates of the policy. d. A copy of the grade report(s) for the immediate prior annual period can also be provided by the insured in order to qualify for the credit. e. The Good Student Discount terminates on the first renewal after the driver reaches age "Distant student" means an operator under age 25 (rel goes to 1.0 at 25) who is a resident student without a car at a school, college, or educational institution over 75 road miles from the place of principal garaging of the automobile. 6. "Resident" means a person living in the named insured's household other than the named insured or anyone who qualified as a "family member". An individual in active military service with the armed forces of the United States is not considered a resident in the household unless they principally operate one of the listed vehicles. 7. "Principally at Fault" means the driver's actions or omissions were responsible for more than 50% of the proximate cause of the accident. A single vehicle accident is a principally at-fault accident, unless it involves collision with an animal. 8. "Performance Type Vehicle" is any vehicle classified by ISO as high performance, premium sports or sports cars, and vehicles not classified that have similar characteristics. 9. "Number of Miles Driven Annually" means the estimated annual mileage for the insured vehicle during the 12 month period following the policy inception. For the low mileage discount to apply proof of actual mileage must be provided at each annual renewal. RU-2 Oregon March 2016

"Defensive Driver" means a driver age 55 or older who has no accidents or moving violations and has successfully completed a Defensive Driver accident prevention course meeting Department of Licensing requirements. A credit for successful completion will be given, as shown in the rate factor pages, unless: a. There is a driver in the household under 25 years of age who regularly operates the household vehicles; b. The vehicle is classed or utilized for business; or c. The approved course is taken as a punishment, specified by a court or other government entity, for a moving violation. In order to maintain the credit, the insured must have no at fault accidents or convictions for moving violations within the previous three years. The premium reduction period is limited to three years for drivers less than 70 years of age and two years for drivers 70 or older at the time the course was completed. Recertification must be completed for the credit to continue. 11. "Driver Training Discount" applies if a licensed driver under 21 years of age who qualifies as a "family member" has successfully completed a driver's education course approved by the State Department of Education or other responsible educational agency. When requesting the driver training discount evidence of satisfactory completion must be provided. 12. "Domestic Partners" mean individuals meeting the requirements of a Domestic Partnership as defined under law in the state where the policyholder resides. 13. "Marital Status" Married classification factors apply to: a married named insured and spouse (code M), "domestic partners" (C), an individual whose spouse or domestic partner has died (W), or individuals who are separated (P). Divorced (D) and single (S) receive the single classification. D-4 Merit Rating Plan The Oregon Mutual Merit Rating Plan is designed to complement our rating structure for a broad range of responsible policyholders, not the truly non-standard risk. We will continue to accept only those risks which are characteristically capable of providing us with a loss ratio commensurate with our rating methodology. Driving Record Surcharge Definitions Driving record points shall be assigned for the following motor vehicle convictions and accidents which the applicant or any operator of the vehicle currently a resident in the same household has incurred during the "experience period". For an accident to be considered non-chargeable (non-fault or NF), the police report or other objective documentation must accompany the application so that the Company can determine fault. Otherwise, two points will be charged as follows: 1. Single car accidents or accidents for which the insured was more than 50% at fault, resulting in damage to any property including their own or any bodily injury or death in excess of $1, If the previous insurer determines that the accident was chargeable, the Company will also consider it chargeable. Points will not be assigned for accidents occurring under the following circumstances (proof must be submitted with the application): 1. Applicant, owner or other resident operator involved in an accident is (1) determined to be 50% or less negligent, or (2) reimbursed for 50% or more of his or her damages by, or on behalf of, persons involved in the accident. Exception: A vehicle rolling from a parked position will not be considered to be lawfully parked, but charged to the person who parked the auto. 3. The vehicle was struck in the rear by another vehicle and the applicant or other resident operator has not been convicted of a moving traffic violation in connection with the accident. 4. Operator of the other automobile involved in such accident was convicted of a moving traffic violation and the applicant or other operator resident in the same household was not convicted of a moving traffic violation in connection with the accident. 5. The vehicle was struck in a hit and run accident where the other driver is at fault, if the applicant or other operator reports the accident to proper authority within 24 hours. 6. The accident resulted from contact with animals or birds. 7. Accidents involving Physical Damage limited to and caused by flying gravel, missiles or falling objects. One point shall also be assigned for: 1. Each minor moving violation. 2. Any operator age 19 or older who has been licensed less than three years. This point shall apply for three years from the date licensed. Also see accident forgiveness in Credits and Options. The following point descriptions and charges are provided in the event underwriting exceptions are considered and risks are written with the Oregon Mutual Insurance Company. Refer all to Company prior to binding. Four points are assigned for: 1. An accumulation of points under a state point system or series of citations requiring evidence of financial responsibility as of the effective date of the policy period and any renewal period during the experience period. 2. Any alcohol or drug related conviction. 3. Serious violations such as hit and run, reckless driving, negligent homicide or assault, eluding a peace officer, felony involving vehicle, lending license or using another s license, or excessive speed. D-5 Driver Eligibility Requirements 1. Drivers License Requirements a. All listed drivers must have a valid, permanent driver's license number or learner's permit. Driver's license numbers of excluded persons may also be required. An international driver's license is not acceptable. b. Drivers with a suspended license are not eligible for coverage. 2. Driver exclusions require prior approval and are only available if Oregon Mutual Insurance Company provides coverage for the other members of the household. 3. Financial responsibility filings are not available in this program. 4. A Physician's Statement may be required after a loss or other activity that could indicate a change in driving habits. 5. Any prior cancellation, declination, or non-renewal of automobile insurance coverage must be satisfactorily explained. RU-4 Oregon August 2016

Target risks, examples of which might be: sports promoters, professional athletes, professional musicians, entertainers, actors, or actresses. 2. Risks requiring a driver exclusion. Driver exclusions are only available if Oregon Mutual provides coverage for the other members of the household. Driver's license numbers of excluded persons may also be required. 3. Operator who drives more than 40 miles to work or school or over 30,000 miles annually. D-7 Ineligible Exposures 1. Any of the following or similar serious violations for any driver in the household: a. Hit and run. b. Driving while intoxicated. c. Negligent homicide or assault. d. Reckless driving. e. Speeding contest. f. Eluding a peace officer. g. Felony involving vehicle. h. Lending license or using another's license. i. Speeding over limit 35 mph or more for any driver within the past three years or 100 mph or greater. 2. More than one of the following violations in the household within the past three years: a. Inattention, careless operation. b. Open container. c. Passing stopped school bus. d. Speeding mph over speed limit. 3. Citation and at fault claims activity for the following: a. For new business, any driver age 21 or younger with more than two (2) points. For renewal, any driver age 21 or younger with more than three (3) points, whether waived or not. b. For new business, any driver age 75 or older with more than two (2) points. For renewal, any driver age 75 or older with more than three (3) points, whether waived or not. c. For new business, all drivers age 22 through 74 with more than three (3) points. For renewal, all drivers age 22 through 74 with more than four (4) points, whether waived or not. d. For new business, any household with more than four (4) points. For renewal, any household with more than five (5) points, whether waived or not. 4. No driver may be physically or mentally impaired where such impairment may affect the safe operation of a vehicle. 5. Driver without a valid license, or whose license has been suspended as a result of moving violation(s) or accident(s) within the past five years. 6. Driver age 18 or younger w/o supporting policy for parents. 7. Driver required to provide a financial responsibility filing. 8. 6 VEHICLE RULES AND DEFINITIONS V-1 Vehicle Codes Private Passenger Vehicles (PPV's): A = Auto P = Pickup PC = Pickup with camper shell O = Classic car regular use E = Electric (or hybrid) car Specialty Vehicles (not considered PPV's): Q = Antique auto RO = Classic car restricted use MH = Motorhome AH = Travel trailer T = Trailers other than travel trailer (utility, horse, recreational) NA = Non-owned auto V-2 Private Passenger Vehicles 1. Personal automobile coverages shall apply to individually-owned private passenger autos, pickups, vans, and trailers. 2. Eligible vehicles are described as follows: a. A private passenger auto means a private passenger or station wagon type automobile, not used as a public or livery conveyance nor rented to others. b. A private passenger auto also includes any other automobile of the utility, pickup body, sedan delivery or panel truck type: (1) With a rated load capacity not to exceed 2,000 pounds, (2) A gross vehicle weight not to exceed 10,000 pounds, and (3) Not used in the primary occupation, profession, or business of the named insured or relative other than farming or ranching. 3. Leased private passenger automobiles, of a type eligible for coverage on an Oregon Mutual policy, may be written under the policy provided they are leased for a period of six months or more. V-3 Business Use Vehicles 1. Individually owned private passenger autos and pickups that are used in the occupation, profession, or business of the insured may also be written if: a. They are operated by the named insured and residents of the insured's household, b. There is no employee use, the vehicle is not used for wholesale or retail delivery, and c. There is no towing of trailers for business purposes. d. There is no hauling or transporting of tools, equipment, or supplies. e. There are no signs on the vehicle. 2. Motorhomes and trailers used for business are not eligible. 3. Corporate owned vehicles are not eligible. V-4 Vehicle Age Vehicle Age is calculated by subtracting the effective year of the policy period from the model year of the vehicle. If the policy is effective between October 1 st and December 31 st add one to the after subtraction for the vehicle age. If calculation is <0, use 0. RU-6 Oregon March 2016

7 VEHICLE RULES AND DEFINITIONS (Cont.) V-5 Specialty Vehicles Antique Autos (vehicle type = Q) 1. An antique automobile is a private passenger automobile over twenty-five years old used primarily for exhibition, parades, club purposes, other functions of public interest, and used infrequently for other purposes. 2. An automobile is twenty-five years old when the year in which the policy is being written minus the model year of the car equals twenty-five or more. 3. Comprehensive and collision coverages are written on a stated value basis. This amount should be the actual cash value of the vehicle at the time insurance is purchased. 4. Antique automobiles are not classification rated, and are not eligible for and do not make other vehicles eligible for the multi-car discount. 5. Limit of Liability a. Our limit of liability for a partial loss will be the lesser of the: (1) Actual cash value of the stolen or damaged property; or (2) Amount necessary to repair or replace the property; or (3) The stated amount limit of liability shown in the Declarations. b. Our limit of liability for a total loss will be the amount shown for each scheduled antique auto which is agreed to be the value of the covered auto. 6. Any depreciation of the value of the auto due to unavailability of any part of the auto does not constitute loss. 7. Refer to the separate rate pages for antique automobiles to determine the premiums for the coverages and limits desired. 8. IT IS THE INSURED'S RESPONSIBILITY TO ADJUST (UP OR DOWN) THE STATED VALUE TO ACCURATELY REFLECT CHANGES IN AN ANTIQUE AUTO'S VALUE. Classic Cars 1. A classic car is a stock (or restored), well maintained, usually older auto which, considering its age, has an unusually high value. A replica or reproduction auto may be eligible to be insured as a classic car. The word "auto" does NOT include: hot rods, street rods, "funny cars", customized show cars, or similar autos; or any altered or modified auto, except that minor changes necessary to restore an auto are permitted. 2. For rating purposes, classic cars are of two kinds: Restricted Use (vehicle type = RO) and Regular Use (vehicle type = O). a. Restricted Use means: (1) The auto is over ten (10) years old; (2) Maintained solely for restricted use in classic or replica car club or similar club activities, exhibits, parades or similar functions of public interest; (3) Used only infrequently for other purposes; and (4) Driven not more than 2,500 miles per year, subject to the following conditions and restrictions: (a) "Maintained for restricted use" does NOT include autos, regardless of how infrequently they are used, that are not maintained for use as, and are not generally recognized as, classics or replicas. (b) "Club activities, exhibits, parades or similar functions of public interest" do NOT include: racing, rallies, gymkhanas, or similar activities, or any kind of speed or timed contest. (c) "Used only infrequently for other purposes" does NOT include any regular, usual or customary use driving to or from work or school; or any business, commercial, or advertising purpose. b. Regular Use means the auto is used on a regular basis. 3. An auto over 25 years old with restricted use, as defined above, may be eligible to be insured as an antique auto-refer to the Antique Auto section. 4. With respect to all classic cars, comprehensive and collision coverages are written using a stated value ONLY. The stated value amount will appear in the Declarations. 5. All vehicles over 25 years old (not classed as antique or classic restricted use) requesting comprehensive and collision coverage must be written at a stated value basis under our classic regular use program. 6. Regular Use Classic Cars are rated just like a regular Private Passenger Automobile, except for the Comprehensive and Collision coverages. 7. Restricted Use Classic Cars are not classification rated, and are not eligible for, and do not make other vehicles eligible for the multi-car discount. 8. Limit of Liability a. Our limit of liability for a partial loss will be the lesser of: (1) Actual cash value of the stolen or damaged property; or (2) Amount necessary to repair or replace the property; or (3) The stated amount limit of liability shown in the Declarations. b. Our limit of liability for a total loss will be the stated amount shown for each scheduled classic auto which is agreed to be the value of the covered auto. c. Any depreciation of the value of the auto due to unavailability of any part of the auto does not constitute loss. d. IT IS THE INSURED'S RESPONSIBILITY TO ADJUST (UP OR DOWN) THE STATED VALUE TO ACCURATELY REFLECT CHANGES IN A CLASSIC CAR'S VALUE. 9. On those policy renewal dates first falling after September 30th of each year, a classic car's comprehensive and collision rates will change automatically to those of the current model year, regardless of the auto's actual model year. 10. Symbols based on stated amount use the PPV symbols and rels from the 1980 and older stated value table. V-6 Specialty Vehicles Other Performance Vehicles Performance vehicles are those vehicles with an ISO rated symbol class of s, h, p, or other vehicles with similar characteristics. Recreational Vehicles, as follows: 1. Camper (vehicle type = PC) A pickup used to transport a slide-in (portable) camper unit. Canopies used with pickups are not considered to be campers unless they are equipped with sleeping and cooking facilities. Indicate "pickup with camper" as the model on the Application or Request for Change. 2. Motorhome (vehicle type = MH) Individually owned, self-propelled land motor vehicle with a living area that is an integral part of the vehicle chassis (including pickups or vans with permanently attached living quarters.) The motorhome cannot be used in the occupation, profession or business of the insured or rented to others. RU-8 Oregon March 2016

Trailers (vehicle types = AH & T) A travel trailer (code AH) is a vehicle equipped as living quarters and designed to be towed by a private passenger auto or utility vehicle. This does not include a trailer used as an office, store, display, passenger trailer, or one that is used for any business purpose. Also included are horse, utility, and other recreational trailers such as ATV or snowmobile (code T). V-7 Vehicle Symbols Physical Damage (Comp and Collision) 1. Regular use PPV's not including regular use classic cars. a. ISO RAPA 1 and RAPA 2 symbols will automatically be assigned based on vehicle ID number for model years 1981 and newer. b. If the first digit of the RAPA symbol is P and the model year is greater than or equal to 1990, require cost new to be entered and rate per 1,000 used for above 80,000. c. Model years 1981 or newer where no symbol is found require cost new to be entered. A RAPA symbol is assigned based on the cost new and the 2nd digit = K. d. For 1980 and prior, the symbol will be based on cost new from the table for 1980 and prior. 2. Classic and Antique auto symbols are determined by the stated (current market) value of the vehicle. Use the symbol table 1980 and prior. 3. Motorhomes, travel trailers, and other trailers (utility, horse, etc.) are rated based on ACV. Liability (BI, PD, Med/PIP) 1. Symbols for BI, PD, Medical or PIP are automatically assigned based on vehicle ID number for vehicles 2000 or newer. 2. If no symbol is available, the liability symbol will be coded as 999. V-8 Custom Shield Score 1. A score that is calculated based on various attributes of the policy. 2. The Custom Shield Score (CSS) is calculated at the policy level and is used on all vehicle types. V-9 Prior Approval Required Do Not Bind 1. Vehicles garaged away from the principal location unless the alternate garage is a secondary residence of the named insured. 2. Autos with an original cost new in excess of $100,000 and motorhomes in excess of $150, Vehicles with a load capacity in excess of 2000 pounds or Gross Vehicle Weight in excess of 10,000. V-10 Ineligible Vehicles 1. Customized or modified vehicles including but not limited to: kit cars (full electric or hybrid cars from major manufacturers are acceptable), hi-rise trucks, vehicles with oversized tires, altered engines, altered suspensions, or customized paint. 2. Motorcycles or vehicles specifically designed for off-road use. 3. Vehicles used for delivery. 4. Dune Buggies. August 2016 Oregon RU-9

Vehicles that are in an unsafe condition, have pre-existing damage, or are rebuilt or salvaged vehicles. 7. Vehicles that require a State of Federal Motor Carrier permit to operate. 8. Corporate owned vehicles. 9. Motorhomes used in business or motorhome only policies. 10. Vehicles of the following makes: Bugatti, Ferrari, Lamborghini, McLaren, Panoz, Delorean, Alfa Romero, Aston Martin, Bentley, Lotus, Rolls-Royce, or model Viper and similar high end/high performance or uncommon vehicles. Contact underwriter for questions or specific vehicle eligibility. 11. Vehicles over 25 years old that require physical damage must be written as classic or antique auto, whichever applies. POLICY LEVEL RULES AND DEFINITIONS P-1 Named Insured 1. You and Your refer to the person named in the Declarations and the spouse. This includes a person who is a partner in a domestic partnership, as defined under Oregon law, and who is a resident of the same household. 2. The named insured must be a person; a business name is not acceptable. 3. Non-related individuals may be named insureds if: a. They are residents of the same household, and b. They have joint ownership of the vehicle or vehicles insured on the policy. 4. Life estates, trusts, and other similar interests may be added as additional insureds. 5. If there are more than two named insureds, the additional persons will be added as additional insureds. 6. Both the insured and their resident spouse, or individuals qualifying as domestic partners under Oregon law, may be named even if there is no joint ownership. P-2 Other 1. A policy can be written only when there is at least one regular use Private Passenger Vehicle insured for liability. 2. If there is a regular use PPV with liability coverage; motorhomes, specialty vehicles, and comp only vehicles may also be written. 3. Proof of continuous prior insurance is required. Coverage may be verified electronically or, if not available electronically, a copy of the current declarations page will be required. a. Prior Standard Company: Six months proof of continuous prior insurance is required. b. Prior Non-standard Company: 12 months proof of continuous prior insurance is required. 4. A lapse in prior coverage may be acceptable as follows and, if eligible, a no prior insurance surcharge will be applied (unless a valid no need statement where applicable is provided for the period of no coverage): a. A single lapse of up to six days within a six month period for prior standard carriers or within a 12 month period for prior non-standard companies. Lapse must be coded and a surcharge will apply. A single lapse of 7 to 30 days within a six month period for prior standard carriers, or within a 12 month period for prior non-standard companies. Lapse must be coded a surcharge will apply. c. Multiple lapses of any duration in either a six or 12 month period or a single lapse greater than 30 days are not eligible. 5. BI limit is policy level and applies to all vehicles other than travel trailers and other trailers or vehicle specifically marked as comp only. 6. PD limit is policy level and required if BI split limits are selected. Applies to all vehicles other than Trailer or comp-only vehicles. If CSL limit is selected, a separate PD limit is not required, as we use the CSL limit to rate PD. 7. For Collision coverage to be selected, both liability and comp coverage are required for the vehicle. 8. UMBI must match BI limits unless signed reduction form. Minimum limit is Financial Responsibility requirement of 25/50, UMPD is optional and cannot be greater than PD limit. 10.
