Request for access to GMnet My data Benefits Payments Contracts Services LeClub Help Groupe Mutuel Mag To receive your personal identification data to log on, go to click on «My Space» (at the top right of the screen) and fill in the registration form available under «Request for access». GMapp mobile application Manage your health insurance data on your smartphone Important information Correspondence and payments: once your access request has been approved, you will receive correspondence from your health insurer and make your payments exclusively via the Internet. GMapp is the mobile application that allows you to use the GMnet features on your smartphone in a totally secure way. Download the GMapp application to your mobile phone (available on Apple Store and Google Play). Open the GMapp application and enter your address and GMnet password. Enter the code received by SMS on your mobile phone to finalise the procedure and connect to GMapp. When you next log on, simply enter your password. Touch ID: if your mobile phone is equipped with Touch ID, you may also connect to GMapp easily using only your fingerprint. To do this, go to «Settings» on your GMapp mobile application. 3

4 2. Connection to GMnet Please enter your GMnet address (sent to you by or by post when you requested access) and your password (sent to you by post). Then enter the code received on your mobile phone. If you have lost your password or blocked your GMnet account, a link on the screen will allow you to obtain new identification data. With your GMnet username and password, you may also access the GMapp mobile application. 4

5 3. Welcome to GMnet On the homepage you will find a summarised view of your deductible and co-insurance status. From the homepage, you can access the different pages of the website to get an overall view of your existing insurance plans. Be informed at a glance, find out more in a few clicks. 6 4. My data This section provides you with a detailed overview of your personal data and addresses. You can easily change your details at any time. 6

7 5. Reimbursements Check all details relating to the reimbursement of your benefits. Your statements of benefits are displayed from the most recent to the oldest and for the current and previous year. 7

8 6. Send documents This section allows you to send us rapidly your documents (invoices, referral vouchers, etc.) and those of your family members electronically. Simply scan the documents with your personal printer and download them from your GMnet account, complying with the following requirements: one document per pdf file, selection of the type of document and selection of the insured person concerned by the document. 8

9 7. Account status This screen provides information on the status of your premium and co-insurance invoices, as well as on the outstanding balance per health insurer. It also contains any invoices for which you have received a reminder or summons to pay. Reminders and summons to pay are also sent to the insured person by post, even if the person has a GMnet account. You may ask for a payment extension at certain conditions. The terms and conditions for payment extensions are available by clicking on the question mark located next to «Extend the deadline». This option is unavailable in case of a summons to pay or debt collection procedure. 9

10 8. Premium invoices View your premium invoices, displayed from the most recent to the oldest, for the current and previous year. It is easy to see which ones have been paid or not. 10

11 9. Statements of co-insurance amounts This page allows you to view your co-insurance statements (deductible and co-insurance). You may easily see which statements have already been paid. Statements are displayed from the most recent to the oldest and for the current and previous year. 11

12 10. Overview of my contracts This section provides you with an overview of your current insurance contracts (Health and Patrimony). You also have the possibility to modify your Health contracts directly online. 12

13 11. Online documents This screen contains all the documents that were sent to you. They can be viewed directly online. 13

14 12. Activity log This screen allows you to view the documents sent to us via GMnet/GMapp. You may also check the processing status of the documents. 14

15 13. Statement of account This screen allows you to request a statement of account for outstanding premiums and co-insurance amounts. 15

16 14. Forms This section contains online forms to report an accident or order a new insurance card. 16

17 15. Contact Ask any questions, whether general or specific, and have someone call you back according to your availability (date and time of preference). 17

18 16. LeClub Our programme «LeClub» gives you access to a variety of offers designed especially for persons insured with a member health insurer of Groupe Mutuel. Discover LeClub and take advantage of a number of exclusive offers for GMnet users and available under «My offers». 18

19 17. Help The «Help» section contains the User Guide, a glossary to help you better understand specific terms related to health insurance, as well as an FAQ section for your most frequently asked questions. 19
