3 Na(onal Healthy Worksite Program The Na8onal Healthy Worksite Program is designed to assist employers in implemen8ng science and prac8ce- based preven8on and wellness strategies that will lead to specific, measureable health outcomes to reduce chronic disease rates.
4 The Healthy Arizona Worksite Program To help Arizona employers successfully implement evidence-based healthy worksite initiatives to improve the health of their employees and businesses.
5 Healthy Worksite Program Goals Reduce the risk of chronic disease among employees Promote sustainable and replicable workplace health ac8vi8es Foster the integra8on of health promo8on and health protec8on programs Promote peer- to- peer business mentoring Provide a sustainable community model
6 Worksite Health Best Prac(ces The approach that has proven most effec8ve is to implement an evidence- based comprehensive health promo(on program that includes individual risk reduc8on programs, coupled with environmental supports for healthy behaviors and is coordinated and integrated with other wellness ac8vi8es However, only 6.9% of employers offer a comprehensive worksite health promo8on program, according to a 2004 na8onal survey. 11
7 NHWP Program Reach The Na)onal Healthy Worksite Program will Engage and recruit 100 employers to implement comprehensive worksite health programs Small ( < 100 employees) Medium ( employees) Large* (251 1,000 employees) Represent a variety of industry sectors Establish 8 communi8es with robust worksite health capacity through roundtables and training * Employers with > 1000 FTE may nominate a single worksite with no more than 1000 FTE

11 NHWP / HAWP Assessments Assessment of employer and employee needs, interests, health risks and exis(ng capacity At the individual level, the NHWP will u8lize a health risk assessment (CAPTURE) and health and safety culture audit (INPUTS) At the organiza8onal level, the NHWP will assess elements of culture, prac8ces and policies (CDC Worksite Health ScoreCard
12 CDC Worksite Health ScoreCard The CDC Worksite Health ScoreCard (HSC) is a resource to employers to assess their health promo8on programs, iden8fy gaps and priori8ze high- impact strategies to prevent heart disease, stroke and related condi8ons.

14 Program Benefits Employers Par(cipa(ng in the NHWP will benefit through: Intensive onsite support to build their own unique workplace health program tailored to their organiza8on and employees Assistance in building a high- quality workplace health program Onsite health assessment and program evalua8on Onsite program management support and capacity building
15 NHWP / HAWP Training Five session Worksite Health 101 Provide training and assistance to employers on developing comprehensive healthy worksite programs Develop employer worksite health capacity for the purposes of long- term sustainability of the healthy worksite program Encourage social support among employers in NHWP communi8es and create forums for employers to share successes, barriers and challenges during the program
