2 Delaware Valley Vascular Society 40 th ANNUAL SCIENTIFIC MEETING Dear Industry Colleagues: We would like to invite you to participate as an exhibitor at the 40th Annual Scientific Meeting of the Delaware Valley Vascular Society to be held on Thursday, April 26, 2018 at the Union League of Philadelphia. This year s Annual Meeting will focus on Reinterventions in Vascular Surgery and will be featured through presentations by an expert Panel. Our featured Dinner Speaker will be the Professor and Chief of Vascular Surgery from the University of Massachusetts Memorial Medical Center, Dr. Andres Schanzer who will speak on, Aortic Aneurysm Repair: The Future is Now. Again this year, the Society is planning to offer Self-Assessment Credit that meets the ABS requirements for MOC Part 2 for the Annual Meeting. This provides an added benefit to our membership, which also will help increase attendance at the meeting. The Annual Meeting typically attracts vascular surgeons and caregivers from the tri-state area. During the meeting, both the Lunch and Cocktail Reception will be held in the exhibit area to increase interaction between you and our guests. Additionally, your representatives are invited to attend the dinner immediately following the meeting. Exposure to your company's products and ideas are increasingly important to us. I urge your company to become a partner with us in these important educational efforts and support the meeting with an Educational Exhibit in the amount of $3,500. Your support would go directly toward the Research Awards and educational sessions. 4 DELAWARE VALLEY VASCULAR SOCIETY EXHIBIT INFORMATION EXHIBITOR FEES $3,500 (6 Table-top display) The deadline for receipt of the application is Friday, March 23, However, companies are encouraged to return applications as soon as possible as space is very limited. After March 23, 2018, space will be assigned in order in which application is received with full payment. Exhibitors wishing to avoid assignment of space adjacent to a particular competitor should indicate this on their application. PAYMENT A full payment of the contracted space should be forwarded with the application form. Checks should be made payable to: Delaware Valley Vascular Society and mailed with the Application for Exhibit Space. The Society regrets that it cannot allow Companies to display if the exhibit fees are outstanding following this deadline. REFUNDS/CANCELLATION POLICY Cancellations must be received in writing by March 30, All refunds in exhibit fees are subject to a 25% administrative fee. There will be no refunds for cancellations received after March 30, 2018, unless space can be resold to another company. EXHIBIT AREA/SIZE Exhibits will be set up in the Meade Room, 1st Floor of the Union League. Exhibit display size is one 6 x 30 draped table with two chairs. No free standing floor exhibit or free standing equipment will be permitted, unless previous arrangements have been made with the Society. Careful consideration will be taken when assigning space and the Society will make every effort avoid having competitors next to each other. The Society will work with on-site exhibitor representatives to ensure the table assigned to your company is placed in an acceptable location EXHIBIT HOURS Thursday, April 26, 2018, 12:30 p.m. 6:30 p.m. (Subject to change based on program) FUNCTIONS IN EXHIBIT HALL: Functions that will be held in the Exhibit Hall include Lunch, afternoon Coffee break and the Reception preceding the Annual Dinner. INSTALLATION AND TEARDOWN OF EXHIBITS The Meade Room will be available for set up from 9:00 a.m. to 12:00 Noon. Please note: A Reception is held in the Exhibit Hall immediately following the conclusion of the Scientific Sessions. We ask all exhibitor representatives to keep exhibits intact until the official closing time of 6:30 p.m. or remove their exhibit prior to the reception to keep the room clean and presentable during the reception. After the close of exhibits, all materials must be prepared for shipment by, or removed no later than 7:00 p.m. Assembly of or teardown of exhibits during regularly scheduled exhibit hours will not be permitted. EXHIBITOR SERVICES Exhibitors will receive complete information including Confirmation of Space Assignment, Shipping Information, Services Information, Preliminary Scientific Program and Registration Forms for Company Representatives starting in March. HOTEL INFORMATION: A block of rooms is not offered for this meeting. For representatives interested in overnight accommodations, they may contact the Inn at the Union League. 5 OFFICIAL RULES AND REGULATIONS DELAWARE VALLEY VASCULAR SOCIETY BADGE POLICY All participants affiliated with exhibits must be registered. Each representative will be issued an exhibitor badge and must be employed by the Exhibiting Company or have a direct business affiliation. Three (3) badges are allowed for each $3, exhibit, additional badges will need to be purchased at the rate of $75.00 each. CONDUCT OF EXHIBITS The Delaware Valley Vascular Society reserves the right to restrict the operation of, or evict completely, any exhibit which, in its own opinion, detracts from the general character of the exhibit program and exhibition as a whole. This includes, but is not limited to an exhibitor which, because of noise, flashing lights, method of operation, display of unsuitable material, are determined by the Society to be objectionable to the successful conduct of the exposition as a whole. No drawings, raffles or quiz-type contests of any type will be permitted. No bags or containers for collection of samples are to be distributed by an exhibitor. This applies to any envelope, folder, portfolio, box, or other item that provides carrying space for more than a single sample. Electrical or other mechanical apparatus must be muffled so noise does not interfere with other exhibitors. Character of the exhibits is subject to approval of the Society. The right is reserved to refuse applications not meeting standards required or expected, as well as the right to curtail exhibits or parts of exhibits which reflect against the character of the meeting. This applies to displays, literature, advertising, novelties, souvenirs, conduct of persons, etc. DIRECT SALES No retail sales are permitted within the exhibit area any time, but orders may be taken. ENTERTAINMENT OR SOCIAL ACTIVITIES/UNOFFICIAL MEETINGS The Delaware Valley Vascular Society s Executive Council requests that no functions be held in conflict with the Delaware Valley Vascular Society Annual Meeting. Companies wishing to invite meeting attendees to Restaurants or other functions must do so following the official conclusion of DVVS events and Annual Dinner Meeting. Exhibitors intending to host sales, pre-convention or post-convention meetings, involving only their employees, must inform the Society. FIRE PROTECTION All materials used in the exhibit area must be flame-proofed and fire-resistant in order to conform to the local fire ordinances and in accordance with regulations established by the local Fire Department. Crepe paper or corrugated paper, flame-proofed or otherwise, will not be permitted. Excelsior or other paper is not to be used in crating merchandise. Display racks, signs, spotlights and special equipment must be approved before use, and all displays are subject to inspection by the Fire Prevention Bureau. Any exhibit or parts thereof found not to be fire-proof may be dismantled. All aisles and exits must be kept clear at all times, and fire stations and fire extinguisher equipment are not to be covered or obstructed. HAZARDOUS WASTE Exhibitor assumes responsibility for and any liability for removal or disposal of any material considered to be hazardous waste material. Exhibitor also agrees to conform to any local ordinances and regulations concerning the disposal of any and all hazardous waste. Any and all costs incurred in the removal of hazardous waste from the exhibit facility will be the sole responsibility of the exhibitor. INFRINGEMENTS Interviews, demonstrations and the distribution of literature or samples must be made within the area assigned to the exhibitor. Canvassing or distribution of advertising matter outside the exhibit area will not be permitted. DISTRIBUTION OF MATERIAL WITHIN THE UNION LEAGUE IS NOT PERMITTED.

6 LIABILITY AND INSURANCE The Union League of Philadelphia and the Delaware Valley Vascular Society shall not be liable for any death or injury arising from or out of any occurrence in, upon, at or relating to the Facilities, or damage to property of The Union League or of others located in the Facilities, nor shall they be responsible for any loss or damage resulting from the negligence of the Union League, the Delaware Valley Vascular Society, the Society for Vascular Surgery, its agents, servants or employees or other persons for whom it may, in law, be responsible. Without limiting the generality of the foregoing, The Union League of Philadelphia and the Delaware Valley Vascular Society shall not be without limitation, any subrogation claims by the Union League of Philadelphia, the Society for Vascular Surgery and the Delaware Valley Vascular Society s insurers. The Exhibitor assumes sole and entire responsibility or any and all bodily and personal injury (including death) and damage to property that may be sustained in connection with or resulting from the Delaware Valley Vascular Society s use of and conduct of the Facility and shall indemnify and hold harmless the Union League, the Society for Vascular Surgery, the Delaware Valley Vascular Society, from any claim or damage (including attorney s fees and expenses of legal proceeding) they may sustain, or be required to pay, in connection with or arising out of the Annual Meeting by reason of the act, omission or negligence of any person associated with the Annual Meeting or where presence in the Facility is attributable to the Annual Meeting, including, without limitation, any violation by the exhibitor of the rules and regulations in the prospectus. Such assumption or responsibility and indemnification by the Delaware Valley Vascular Society shall apply unless such damage or injury results from the sole negligence, gross negligence or willful misconduct of the Delaware Valley Vascular Society, Union League of Philadelphia and their employees and agents. In addition, Exhibitor acknowledges that the Delaware Valley Vascular Society, the Society for Vascular Surgery, and the Union League do not maintain insurance covering Exhibitor s property and that it is the sole responsibility of Exhibitors to obtain business interruption and property damage insurance covering such loses by the Exhibitor. MAILING LIST As a benefit of participation, attendee mailing lists will be offered to Exhibitors. Use of this list will be restricted to a one time only use and information directly related to the DVVS Annual Meeting prior to and following the meeting. PROTECTION OF FACILITY Exhibitor shall not permit anything that will injure, mar or in any manner deface the Union League and will not drive, or install, or permit to be driven or installed any nails, hooks, tacks, or screws into any part of the Union League and will not make or allow to be made any alterations of any kind to the building or equipment of the Union League. Exhibitor shall not post or exhibit, or allow to be posted or exhibited, any signs, advertisements, show bills, lithographs, posters or cards of any description on any part of the Union League. Anything in connection therewith necessary or proper for the protection of the building, equipment, or furniture will be at the expense of the exhibitor. The cost to repair any damage caused to the building as result of the violation of this provision shall be paid by the exhibitor. Exhibitor agrees to indemnify and hold harmless the Delaware Valley Vascular Society and the Society for Vascular Surgery for any and all claims arising out of damage to the Union League. SHIPPING INSTRUCTIONS Deliveries will not be accepted more than 48 hours prior to the meeting date of Thursday, April 26, 2018 but should be shipped to arrive at the League no later than Wednesday, April 25, Detailed information will be sent with Exhibitor Confirmations.
