2 1.0 INTENT: 1.1 The intent of this Solicitation is to establish contract(s) with a qualified Contractor(s) to provide boiler preventative maintenance AND maintenance and repair service on an as needed/if needed basis. The work to be done under this contract includes, but is not limited to; providing all labor, materials, supervision, equipment, incidentals, and related items necessary to service/repair, maintain, replace or install boilers; systems and equipment necessary for the successful operation of the heating systems operated by MIHS, in accordance with the specifications contained herein, for repair requirements and projects under $5,000 in total cost. MIHS may elect to use this contract for requirements exceeding $5,000, but reserves the right to obtain competitive bids for requirements exceeding this amount; any deviations from these specifications shall be clearly noted. 1.2 Currently, MIHS has the following three boilers located at 2601 E. Roosevelt St. Phoenix, AZ Hurst Mfg. # 10231, 23081total Capacity, Type FT, Use PB, Natural Gas, method of firing Auto, Heating Surf/BTU/KW 250, MAWP allowed 120 PSI, No. of Temp/Press Controls-2, Fuel Train CSD-1, MSVRC Req/PPH, BTU/HR 17250/PPH, ASME Code S Hurst Mfg. # 8840, total Capacity, Type FT, Use PB, Natural Gas, method of firing Auto, Heating Surf/BTU/KW 2500, MAWP allowed 150 PSI, No. of Temp/Press Controls-2, Fuel Train CSD-1, MSVRC Req/PPH, BTU/HR 17205/PPH, ASME Code S Hurst Mfg. # 10248, Capacity, Type FT, Use PB, Natural Gas, method of firing Auto, Heating Surf/BTU/KW 1500, MAWP allowed 120 PSI, No. of Temp/Press Controls-2, Fuel Train CSD-1, MSVRC Req/PPH, BTU/HR 10350/PPH. 2.0 WORK TO BE PERFORMED (Annual Boiler Inspections and Service calls (Service calls will be scheduled per owners request) Service calls: Conduct an efficiency test on burner-if needed Clean stack if needed Check for any obstructions in piping or ventilation system Check boiler piping for leaks Inspect the burner and adjust as needed Check ignition on boiler Troubleshoot auto flame panel and program as needed Check burner Check motors for boiler Check pilot assemblies, electrodes that pertain to pilot ignition Removal of blower, blower housing and blower wheels as needed repairs No obstructions for blower wheel Valve replacement as needed, identify and communicate valve malfunction Site glass removal and replacement Check safeties during service call as needed Troubleshoot and verification that all controls are working at normal capacity Replace gaskets that stop operation of boiler function

3 Limit switch checks Electrical component checks and make sure all connections are tight Make any recommendations as needed for repairs Assure blower wheel turns freely Utilize proper PPE when working on boilers or around boilers Log any service work completed in the boiler log book located within the power plant engineers room 2.2 Annual Inspections: Clean tubes as needed Remove all soot from base of boiler Test Safety Relief Valves yearly Wipe down exterior of boiler Check all piping for leaks and recommend repairs Check site glass for cracked glass or gasket leaks Check auto flame prior to ignition Check burner probes Check motors and housings Remove k-wool gasket material, add new after completion of work Evaluate tubes for cleaning Evaluate internal walls of boiler for hot spots or corrosion Check blow down valves for proper functionality Verify operation of all controls Verify boiler doors are fully secured Make any recommendations of repairs as needed Utilize proper PPE when working on boilers or around boilers Log all work performed within the power plant engineer s office area log book The existing boiler systems to be serviced under this Scope of Work vary in vintage, design, size, configurations etc. The quarterly inspection and annual servicing includes all hot water and heating boilers. The contractor when complete assures and documents a safe and reliable boiler plant for the associated site. The contractor shall suggest service points with each appropriate model and design as needed to deliver a safe and reliable level of work. Upon completion of the work this contractor shall furnish a list of the items checked on each boiler and the work performed documented on their letterhead. The service company shall provide MIHS Facilities Management with copies of the service and repair records. The Service Company shall keep the maintenance records (checklists) and provide to the owner in a binder and electronically on a quarterly and annual basis. Reports and summaries shall be kept electronically and transmitted to the appropriate MIHS Maintenance person. Annual services on the boiler shall be provided by the Service Company on the boiler and all associated equipment that is related to the boiler operation. Hours for routine maintenance and service shall be between 8:00 am and 5:00 pm Monday through Friday (No MIHS Holidays). Maintenance shall be performed in accordance with manufacturers recommended procedures and applicable industry standards 3

4 Service Company shall prepare a master binder of the various service manuals for the boiler operation. One binder shall be located in the equipment room. Before the servicing of any boilers all monthly and annual checklist must submitted for approval to ensure the requested service will be provided. All work to be performed per UL, FM and IRI insurance companies and boiler inspecting authorities. 2.3 Responsibility prior to annual inspection: MIHS Responsibility: Cool down procedure Draining the actual boiler due for inspection Vendor Responsibility: Remove all doors and access panel Set up all proper PPE for entering into the boiler compartment Fill out proper permit through MIHS for entering a confined permitted space, will attach to this document for review, if you have your own company protocols for entering into a confined space Facility Operations will be willing to review with vendor for approval. Monitor all oxygen levels, flammability, toxic gasses, vapors, dust, Document forms of communication from inside of boiler to outside representation Explain evacuation process on permit, and what materials used for proper PPE during work being performed. Make sure you utilize the buddy system one individual inside and one outside. Document what respiratory protection needed or used Document all protective equipment used or required Identify and document your rescue plan used List the person entering List the standby person List atmosphere tester List the project manager or foreman s name List the Operations Manger s name List an emergency phone number Check all boxes applicable to the permit Inspect thoroughly the inside of boiler, and the outside of the boiler Vacuum and clean all soot form inner parts of the boiler Inspect tubes for determination if tubes need to be cleaned Understand and acknowledge the current policy for entering confined spaces along with confined permitted spaces on MIHS sites. 4

5 2.3.3 Upon completion of inspection from MIHS inspector: Apply new k-wool on each hatch or door for boiler that was opened, install new product no used product can be used for reuse. Close-up and secure boiler Start process to fill boiler Verify all operations of controls on the boiler system Start process to start boiler Make sure all valves are secure and not leaking during startup Make sure operation of boiler are fully functional before leaving site Verify with Power Plant staff prior to leaving Turn in all documentation required by MIHS to the power plant staff 2.4 Contractor must have an extensive working knowledge of Autoflame. 2.5 MIHS reserves the right to increase/reduce the locations during the contract period. Any location shall be serviced at the same rate/cost bid herein. 2.6 MIHS reserves the right to add additional contractors, at MIHS s sole discretion, in cases where the currently listed contractors are of an insufficient number or skill-set to satisfy MIHS s needs or to ensure adequate competition on any project or order work. 3.0 SPECIFICATIONS: 3.1 HOURS OF SERVICE: REGULAR HOURS are between 6:00 AM and 6:00 PM, Monday through Friday, excluding MIHS holidays AFTER HOURS is after 6:00 PM and prior to 6:00 AM, Monday through Friday WEEKENDS are anytime Saturday or, Sunday HOLIDAYS are MIHS Holidays Services shall be available 24/7, 365 days per year Contractor shall provide 24/7; 365 days per year telephone access, and respond to a call for services within fifteen (15) minutes of receipt. 3.2 RESPONSE TIMES: During REGULAR HOURS, AFTER HOURS, WEEKEND or HOLIDAYs, Contractor shall respond on-site within three (3) hours of receipt of a service request If the request is designated by MIHS as an EMERGENCY, the contractor shall respond on-site within one hour of receipt of a service request regardless of the time of day, night, WEEKEND or HOLIDAY. 3.3 TRIP CHARGE: Trip Charges/Fuel Charges: One (1) Flat Trip Charge may be billed for a service call outside of the fifty (50) mile radius from the Contractor s closest business location to the repair site. A Flat Trip Charge shall 5

6 not apply to any service performed where the repair site is located inside of the designated fifty (50) mile radius within a the Contractor closest business location If work is issued to a Contractor and the work sites are in the same geographic area (50 miles out), only one trip charge will be paid. Absolutely no trip charges will be paid for work sites within the 50 mile radious Absolutely no Fuel surcharges are authorized under this contract Quotes/Estimates/Troubleshooting/Lunch/Breaks/Travel time to and From the Jobsite: MIHS will not pay for quotes, estimates, troubleshooting, lunch breaks, breaks or any type of travel time. MIHS will only pay for the time the contractor is actively working on the project. 3.4 CONTRACTOR REQUIREMENTS: Contractor(s) shall supply all labor, supervision, materials, tools, equipment (including large equipment), incidentals and effort necessary to perform the Scope of Work presented The Contractor s service truck fleet and/or warehouse shall carry sufficient supply of repair parts and equipment to perform services per Scope of Work presented The Contractor agrees to utilize only experienced, responsible and capable people in the performance of the work All employees of the Contractor shall wear a company uniform identified with the company name consisting of a minimum of one of the following: Shirt/blouse Vest Cap No one except authorized employees of the Contractor is allowed on the premises of MIHS. Contractor s employees are NOT to be accompanied in their work area by acquaintances, family members, assistants, or any other person unless said person is an authorized employee of the Contractor The Contractor shall perform the work in a way to minimize disruption to the normal operation of building occupants. Upon completion of work the Contractor shall clean and remove from the job site all debris, materials and equipment associated with the work performed Contractor shall adhere to all regulations, rules, ordinances, and standards set by Federal, State, County, and Municipal governments pertaining to safety on the job site. 3.5 BUILDING SECURITY (KEYS): If the Contractor is provided keys to required MIHS Facilities at the discretion of FMD. Keys may be acquired by: 6

7 3.5.2 The Contractor being provided permanent key(s), wherein the Contractor verifies receipt of and accepts responsibility for keys. The keys must be returned at the completion of the work or at the direction of FMD. ). Keys not returned may cause the MIHS to rekey the ENTIRE facility or locations that the set of keys opened with the cost being borne by the Contractor In lieu of or in addition to keys, the Contractor may be provided card access badges at the discretion of FMD The Contractor shall notify FMD within twenty-four (24) hours if any keys are lost, misplaced, stolen or otherwise not within the Contractor s control Once the Purchase Order is complete, expired, or terminated the Contractor shall immediately return all badges and keys to FMD. 3.6 SALVAGE (if applicable): Salvage rights shall be evaluated on a project by project basis by MIHS FMD and shall be determined prior to incorporation in the contractor s bid price. Salvage rights automatically apply for all work if in the best interest to MIHS. Salvageable materials without pre-approved contractor salvage rights shall be securely stored and are not to be transported off the site without written permission from MIHS. If contractor is given salvage rights, salvageable materials shall be removed daily. No on site storage of contractor s salvaged materials will be permitted. 3.7 CONTRACTOR EMPLOYEE BACKGROUND CHECK: A background check may be required for all Contractor employees providing services to MIHS. The cost of this service shall be incurred by the Contractor No Contractor employee may access MIHS property without approval of FMD. This applies to (but is not limited to) the company, sub-contractors and employees and the failure to pass these checks shall deem the respondent non-responsible. 3.8 PROJECT WORK AND TIME AND MATERIALS (If multiple award): Project work shall mean work performed on normal and/or major projects. Contractor(s) assigned to this contract may be provided a request for project quote with a detailed Scope of Work. As such, each contractor MUST submit a response. All terms and conditions are those established under this agreement. Should this contract results in a sole award, work may be quoted as time and materials. All additional labor charges outside the Scope of Work are those bid prices established on the pricing page of the Contractor The threshold from time and materials to project work shall be $5, Exceptions to this shall be emergencies that arise and must be dealt with immediately without the time for project quotes. 7

8 This figure is not firm fixed. MIHS reserves the right to adjust this figure to a higher-level if deemed in the best interest of MIHS (Note: The Facilities department must inform the procurement officer assigned to this contract when the work performed under T&M escalates to $5, and may go higher) If multiple awards are made, MIHS may request multiple quotes on projects, MIHS s project quote sheet will contain the following information: The contract number and name; Name and address of site; FMD site number; Detailed scope of work, Other information relative to the S.O.W., Project start/finish time line (optional), Check box for will quote or will not quote the project, Signature line for both MIHS and the Contractor After site review of the project, all contractors listed under this contract must submit the project quote sheet back to the requestor, either with acceptance and a firm price; or decline with a written reason as to why the project was declined. Contractors who have declined project work a minimum of three times during a six-month period may have their contract terminated with no penalty to MIHS The submitted project price quotes shall be all-inclusive. That is, any cost overruns to be absorbed by the Contractor. Exceptions to this are changes requested by MIHS that incur higher project cost and longer delays. All change orders to a project must be in writing, referencing the contract serial number, and approved by FMD prior to any authorization to proceed. The Contractor who fails to acquire change orders in writing runs the risk of incurring the additional costs without payment Project pricing shall include everything the contractor anticipates is necessary to complete the job (i.e., rental equipment, materials, chemicals, labor, supervision, transportation, subcontractor costs, mobilization costs, demobilization costs, permits, etc.). These costs to be part of the overall project price and as such not itemized Contractors shall be compensated for additional work requested that is not detailed in the scope via the labor rates bid on the pricing page of the Contractor. Note: all extra work outside the S.O.W. must be in writing. 8

9 3.9 PROJECT WORK PRICE CEILING LIMITS: Projects shall not exceed $5, each. If an emergency occurs, this price ceiling may be lifted if approved by the Contracts Officer assigned to this contract, otherwise, the project may be separately bid outside of this contract document. This to ensure MIHS receives adequate competition for such work TIME AND MATERIALS WORK: This contract may also be used for time and materials work (not to exceed $5, without pre-approval from FMD and the Contracts Officer assigned to this contract) and priced per hour as bid in the pricing section All service work performed by Contractor shall be to a professional standard, and susceptible to FMD staff inspection. Documentation, through an audit and feedback system of contract administration may be used in this contract, by MIHS FMD The Contractor shall perform the work in a way to minimize disruption to the normal operation of Patients, Staff, Visitors and all other building tenants. Upon completion of work the Contractor is responsible for cleaning and removing from the job site all debris, materials, and equipment associated with the work performed Contractor must have the capability to use photo documentation when requested by MIHS. This will allow verification of conditions to be photographed both before and after 3.13 In the event the work performance of the Contractor is unsatisfactory, the Contractor will be notified by MIHS and be given one calendar day to correct the work. Labor for all re-work will be at no cost to MIHS CONTRACTOR QUALIFICATIONS: Contractor must have the proper license required through the State of Arizona, Registrar of Contractors to perform the Scope of Work prior to submitting their bid. Licenses must be in good standings Contractor must meet all Federal EPA and OSHA guidelines (if any) in the proper handling and disposal of special waste or contaminated materials generated by services rendered REQUIRED SUBMITTALS: Copy of the appropriate State of Arizona, Registrar of Contractors license as it pertains to the scope of work; licenses must be in good standings to be considered valid Provide a roster of staff technicians listing name, years of experience, and field of expertise BEHAVIORAL FACILITIES: Contractors should be aware to use caution when servicing a behavioral facility. It shall be a requirement that the Contractor s service vehicle be secured and locked. Tools taken into a behavioral medical facility must be monitored and accounted for at all times. 9

10 Areas in the behavioral facilities may require the Contractor to make repairs in stages rather than in whole due to the nature of the facility. This should be a consideration when preparing a quote on a specific project Contractor must meet all Federal EPA and OSHA guidelines (If any) in the proper handling and disposal of special waste or contaminated materials generated by services rendered. 4. LOCATION OF WORK AND DEPARTMENT DESIGNATIONS Work site may be at any MIHS office or MIHS facility. Any department may request any classification of temporary personnel listed in the Contract. The department/location designations in the work classification only indicate the most prevalent user department and/or locations. 10
