1 Oak Hall School Booster Club Sports Fantasy Getaway Raffle OFFICIAL RULES AND REGULATIONS The following rules and regulations govern the Sports Fantasy Getaway (SFG) By entering the SFG Raffle, you agree to abide by the following rules. 1. Term of Sports Fantasy Getaway: The Sports Fantasy Getaway raffle will begin on October 12 th, 2016 and end on May 10 th, 2017, 7:00pm 2. Sports Fantasy Getaway Raffle Drawing: The SFG raffle will take place at the Oak Hall School 8009 SW 14 th Ave Gainesville Fl on May 10, At approximately 7:00PM one (1) winner ( Prize Winner ) will be randomly drawn from among all eligible entries received. The Prize Winner will receive the Sports Fantasy Getaway package detailed below. 3. Sports Fantasy Getaway raffle prize: The Prize Winner will choose a regular season sporting event to be held within the continental United States or Canada within six (6) months of prize drawing and notification date. Travel must be completed within 15 months of the draw date and notification of recipient. The prize will include: 2 tickets (best seats available at the time of purchase) to said event, first class airfare for 2 (if available) to the nearest destination city, luxury hotel accommodations (one room) for 2 nights, transportation to and from the airport/hotel as well as to and from event and vouchers to cover a dinner for 2 and souvenirs. Total package value not to exceed $10,000.

2 4. Cash Prize Alternative: Prize Winner may choose to receive a $5,000 cash prize alternative instead of the trip listed in number 3. This choice must be requested in writing by May 17, Federal Income Tax Regulations (6) apply. 5. Need Not Be Present to Win: Prize Winner need not be present at the physical drawing event in order to win the prize. 6. Subject to Federal Income Tax Regulations: Prize awarded under the SFG raffle is subject to all applicable Federal tax regulations, including, but not limited to, applicable IRS reporting and/or withholding requirements. The Prize Winner should consult with their personal tax advisor(s) regarding their winnings. Oak Hall School (OHS) may require Prize Winner to complete and submit certain documentation required under Federal tax regulations, including, but not limited to, an IRS Form W-9, as a condition of receiving their prize. Failure to timely complete, sign and return any such documentation may result in Prize Winner's disqualification and the forfeiture of his or her interest in the prize, in accordance with the terms of Paragraph 20 below. Prize Winner is hereby notified that OHS will require an IRS Form W-9 to be completed so that an IRS Form W-2G can be issued to both Prize Winner and the IRS. 7. Entry Tickets - Format: For each entry received, a unique ticket will be issued which will include the entrant s name, and telephone number, and address ( entrant s contact information ). SFG administrator will issue an entry ticket utilizing entrant s contact information and will include said entry ticket in the drawing pool. For tickets purchased online via the OHS website an immediate automated response receipt will be generated. 8. Eligibility: The SFG raffle is open only to legal U.S. residents who are 18 years of age or older at the time of entry. Entrants need not be current Oak Hall families.

3 9. No Purchase Necessary: No purchase is necessary to enter or win the SFG raffle. 10. To Enter: There are two (2) ways to receive an entry ticket into the SFG raffle: a. Purchase a SFG Fundraiser raffle ticket: SFG raffle tickets can be purchased: i. Online at before 3:30 P.M. on May 9, ii. In person, prior to the May 10th, 2017 event. One ticket is valued at $100. Tickets purchased must be a completed transaction by 6:00 PM on May 10th, b. Enter without Purchase: Enter the SFG raffle without purchasing a ticket by sending a hand-written request to SFG Raffle attention: GFS Raffle Entry Request, 8009 SW 14 th Ave, Gainesville, FL Written request must specify entrant s name, address telephone number and address. All entries submitted by sending a written request must be postmarked on or before October 19 th, Same Chance of Winning: All valid entries will have the same chance of winning regardless of method of entry. OHS will not be responsible for any incomplete, lost, late, stolen, misdirected, illegible, garbled or unintelligible entries, or for failure to receive entries due to technical problems or transmission failures of any kind, including without limitation, the malfunctioning of any network, hardware or software. Any questions regarding the validity of entries submitted shall be determined by OHS in its sole discretion and OHS reserves the right to disqualify any persons who tamper with or abuses any aspect of the SFG raffle. All entries become the property of OHS and none will be returned or individually acknowledged.

4 12. Minimum Number of Entries: If fewer than 100 tickets are sold, then no winner will be drawn, and the money will be refunded. 13. Number of Eligible Entries: There is a limit of 500 eligible entries into the SFG raffle. Odds of winning any prize depend upon the number of eligible entries received. 14. Winner Notification: Prize Winner will be announced on May 10 th, 2017 at 7:00 pm at Oak Hall School. Winner will be notified by and mail by May 17th, Waiver of Prize: Prize Winner may waive their right to receive a prize. In the event Prize Winner elects to waive prize, Prize Winner will notify OHS of his or her intention to waive the prize in writing on or before June 10th, If prize is waived under the terms of this Paragraph, the prize will be forfeited permanently. 16. Prize Delivery: The Prize winner will be notified as addressed in paragraph fourteen (14) and will receive a certificate by mail if not in attendance at the drawing; or in person if in attendance at the drawing with prize information as listed in paragraph three (3) and paragraph four (4). a. SFG Package Delivery: Prize Winner must submit, in writing, the SFG package prize request for game and travel to the SFG administrator within six months of the drawing date, or September 17, If the SFG administrator does not have the SFG prize request by November 10th, 2017 then the Prize Winner will forfeit the prize. b. Cash Prize Alternative Delivery: Prize Winner may choose to receive a $5,000 cash prize alternative instead of the trip and details listed above in paragraph three (3). This choice must be requested in writing by July 10 th, Federal Income Tax Regulations (4) continue to apply.

5 17. Forfeiture of Prizes: Should Prize Winner fail to claim, in writing, the SFG prize or the Cash Prize Alternative on or before June 10 th, 2017 Prize Winner forfeits the prize. If a prize is forfeited, the prize will be forfeited permanently, and will not be awarded to another recipient. 18. Limitation of Damages: Should there any disputes, claims, and/or causes of action arising out of or in connection with SFG raffle, participants agree that any and all liability, judgments and/or awards will be limited to actual out-ofpocket costs incurred by the participants. 19. Choice of Law: all issues and questions concerning the construction, validity, interpretation and enforceability of these official rules, or the rights and obligations of participants and OHS in connection with the SFG raffle shall be governed by and construed in accordance with the laws of the State of Florida. The invalidity or unenforceability of any provision of these official rules shall not affect the invalidity or unenforceability of any other provision so long as the general intent of these official rules remains in effect. In the event that any such provision is determined to be invalid or otherwise unenforceable, these official rules shall be construed in accordance with their terms as if the invalid or unenforceable provision was not contained therein. 20. Other Conditions: all federal, state and local laws and regulations apply. OHS reserves the right, in its sole discretion, to cancel, terminate, modify or suspend this SFG raffle or any portion hereof, or to disqualify any individual, if for any reason there is: (a) infection by computer virus, bugs, tampering, unauthorized intervention, actions by participants, fraud, technical failures, or any other causes which, in OHS s sole opinion, corrupt or affect the administration, security, fairness, integrity or proper conduct of the SFG raffle; (b) the SFG raffle or any website associated therewith (or any portion thereof) becomes corrupted or does not allow the proper

6 processing of entries per these official rules; or (c) the SFG raffle is otherwise not capable of running as planned. By entering, participants agree to release and hold harmless Oak Hall School, and its parent, subsidiaries, affiliates, divisions, advertising and promotional agencies, wholesalers and retailers, vendors, contractors, subcontractors, and each of the foregoing entities' employees, trustees, officers, directors, shareholders, members and agents (collectively the "released Parties"), from and against any and all claims, actions and/or liability for any injuries or death, loss or damage of any kind arising from or in connection with participation in and/or entry into the SFG raffle or acceptance, delivery, or use of the Prize. The released Parties are not responsible or liable for any incorrect or inaccurate entry information, and assume no responsibility for (a) typographical or other errors in the printing of the promotion materials or the offering or announcement of the prizes; (b) any error, printing error, omission, interruption, defect or delay in operation or transmission at any website; (c) failure of any entry to be received by OHS due to technical problems, printing error, human error or traffic congestion on the internet or at any website; (d) communications line, hardware and/or software failures; (e) damage to any computer (software or hardware) resulting from participation in the SFG raffle; (f) theft or destruction of, tampering with, unauthorized access to, or alteration of entries and/or entry information; or (g) entries which are late, lost, stolen, damaged, illegible and/or unintelligible (or any combination thereof). By entering, participants agree to comply with these official rules. Any participant who attempts to tamper with this SFG raffle in any way shall be disqualified. Additional restrictions may apply. Participants agree to indemnify and hold harmless released Parties, from and against any and all claims, losses, costs, damages, liabilities and costs and expenses (including reasonable attorneys' fees costs) which arise out of any breach in connection with an entry, covenant, agreement, obligation, representation or warranty set forth herein.
