Moriarty opened the meeting and thanked the Committee for all their work leading up to the successful Ballot Vote on 11/4/ Record A motion was made by J. Day and seconded by B. Davis to approve the 10/29/14 Permanent School Building Committee meeting minutes. No discussion, motion passed unanimous by those attending Record Voucher for reimbursement to Smith s Restaurant and Catering for beverages and snacks for the Holbrook Court presentation held on 10/8/14 for $ was reviewed. A motion was made by F. White and seconded by B. Davis to approve the Voucher. No discussion, motion passed unanimous J. Seeley J. Seeley reviewed the Final Budget and Expended Update, attached, for the Feasibility and Schematic Design Phase. Out of the $796,272 appropriation, the Committee completed the work with a balance of $114,889.62, inclusive of the above Voucher. J. Seeley will prepare a budget revision to re-allocate the balance to the Owner s Contingency, once the PFA is executed J. Seeley J. Seeley reviewed the line item budgets for the Design Development thru Construction Administration Phases on the Total Project Budget, attached. J. Seeley to input the Total Project Budget line items into the Budget Tracking Form and provide a monthly update to the PSBC Record J. Seeley reviewed FAI Amendment No. 8, dated 11/12/14 for Design Development Phase thru Construction Administration Phase Basic Services. The Basic Services fee is $7,560,000. A motion was made by M. Moore and seconded by B. Davis to approve FAI Amendment No. 8, dated 11/12/14 and recommend signature by T. Gordon, no discussion, motion passed unanimous Record J. Seeley reviewed FAI Amendment No. 9, dated 11/12/14 for Final Wetland Flagging to be performed by Nitsch Engineering. The cost of the services will be $2,700 plus 10% administrative fee mark-up for FAI, for a total of $2,970. A motion was made by M. Moore and seconded by F. White to approve FAI Amendment No. 9, dated 11/12/14 and recommend signature by T. Gordon, no discussion, motion passed unanimous Record J. Seeley reviewed FAI Amendment No. 10, dated 11/12/14 for Final Topographic Survey to be performed by Nitsch Engineering. The cost of the services will be $48,800 plus 10% administrative fee mark-up for FAI, for a total of $53,680. A motion was made by P. Mahoney and seconded by B. Davis to approve FAI Amendment No. 10, dated 11/12/14 and recommend signature by T. Gordon, no discussion, motion passed unanimous Record J. Seeley reviewed FAI Amendment No. 11, dated 11/12/14 for Final Geotechnical Investigation to be performed by Geotechnical Services Inc. The cost of the services will be $7,100 plus 10% administrative fee mark-up for FAI, for a total of $7,810. A motion was made by P. Mahoney and seconded by F. White to approve FAI Amendment No. 11, dated 11/12/14 and recommend signature by T. Gordon, no discussion, motion passed unanimous.

Seeley reviewed SMMA Amendment No. 2, dated 11/12/14 for Design Development Phase thru Construction Administration Phase Basic Services. The Basic Services fee is $2,639,484. A motion was made by P. Mahoney and seconded by F. White to approve SMMA Amendment No. 2, dated 11/12/14 and recommend signature by T. Gordon, no discussion, motion passed unanimous Record J. Seeley reviewed SMMA Amendment No. 3, dated 11/12/14 for Independent Cost Estimating to be performed by AM Fogarty. The cost of the services will be $47,500 plus 10% administrative fee mark-up for SMMA, for a total of $52,250. A motion was made by P. Mahoney and seconded by F. White to approve SMMA Amendment No. 3, dated 11/12/14 and recommend signature by T. Gordon, no discussion, motion passed unanimous Record J. Seeley reviewed the Overall Project Schedule, dated 10/31/14 and attached J. Seeley J. Seeley reviewed the draft Design Development Phase PSBC Meeting and Agenda Schedule, attached. Committee Discussion: 1. J. Seeley to change the 11/19/14 meeting to 11/25/14; the 12/3/14 meeting to 12/10/14; and the 12/17/14 meeting to 12/23/ J. Seeley to schedule a Community Forum to present the design progress to the community after the new year. 3. J. Seeley to update the schedule and issue Record J. Seeley reviewed the actions of the CM Prequalification Committee to date and the CM Selection Timeline, attached J. Seeley C. Mahoney J. Seeley reviewed the next steps with MSBA as follows: 1. The Town is to forward the certified results of the Special Town Meeting and Ballot vote. J Seeley will contact the Town Clerk to obtain. 2. The Total Project Budget is to be entered into the MSBA s Propay system. J. Seeley to coordinate with C. Mahoney to input. 3. The MSBA Transition Meeting needs to be scheduled. The afternoons of either 12/3/14 or 12/4/14 work for the Committee, J. Seeley to follow-up with MSBA T. Gordon J. Seeley reviewed potential third party assistance with the MSBA Propay process and financial management. T. Gordon will provide additional information at the next Committee meeting K. Kovacs K. Kovacs described the Administration and Teacher Meetings process and schedule to be held over the next several months to refine the design to meet the educational program. FAI will record the minutes of all the meetings and documents presented and will bring to the PSBC for findings and decisions. K. Kovacs to provide a schedule of the meetings so that interested PSBC members can attend K. Kovacs Committee Discussion: 1. M. Reith asked that a log of all existing Memorial Items in the school and on the grounds be developed. K. Kovacs will work with the School Administration to develop the log Record Next PSBC Meeting: November 25, 2014 at 7:00 pm at the H-Cam Studios.

Does not include any potentially eligible contingency funds and is subject to review and audit by the MSBA. 2 - Pursuant to Section 3.20 of the Project Funding Agreement and the applicable policies and guidelines of the Authority, any project costs associated with the reallocation or transfer of funds from either the Owner's contingency or the Construction contingency to other budget line items shall be subject to review by the Authority to determine whether any such costs are eligible for reimbursement by the Authority.
