2 DIXON DANCE STUDIO Teaching the art of dance, one step at a time Our mission at Dixon Dance Studio is to provide students with quality dance instruction while providing an energetic, fun and creative experience. The Studio prides itself on being a noncompetitive environment. Dancing is something that students want to do, not have to do. The magic of dance can be enjoyed by all age groups. REGISTRATION INFORMATION A registration form along with the non-refundable registration fee will be collected upon enrollment. The first monthly payment is due upon registration. Registration Fees: New Students***********$30.00 per person or $20.00 each additional family member. Returning Students*********$15.00 per person or $30.00 per family. (Returning student within three months of last tuition date) Checks should be made payable to Dixon Dance Studio. We do not accept cash payments. Monthly Tuition: Tuition is paid on a monthly basis. Tuition is due by the 1 st of each month. A 7 day grace period is allowed before a $10.00 late fee is applied. Tuition is based on a four week month. Five week months cover any holidays taken by the staff. "RECURRING PAYMENT OPTION" You will receive a 5% discount off of your tuition when you enroll in are Automatic/Recurring Payment Plan that authorizes us to initiate payments for balances due on your account on the 3 rd of each month. "NON RECURRING PAYMENT OPTION" (With Card On File) If you choose not to enroll in the Recurring Payment Plan option you will receive a 5% discount off of your tuition just for having your card on file. Please note, if your tuition is not received by the 7th, your card will automatically be ran for the current balance, plus a $10 late fee and a $5 administrative fee. All past due payments will be processed on the 8th of the month. Remember tuition is due the 1st. "NON RECURRING PAYMENT OPTION" (With NO Card On File) If you choose NOT to have a card on file, you will not be eligible for any discounts and will pay full tuition rates. If your account has a balance as of the 7th of the month you will be charged a $15 late fee. If your account is not paid in full by the 15th of the month, your student will not be allowed to participate in classes until the account is cleared. If the account is still not cleared by the 30th of the month, your student will be dropped from our system and a registration fee will be charged to reactivate the account. Refunds/Pro-Rating: Refunds are not given for tuition. In the event the student needs to withdrawal early, a cancellation class form must be completed to close the account thereby preventing further billing; 30 day notice is required. A student s absenteeism does not affect tuition payments. Accounts will only be closed with a cancellation class form and will go into effect from the date received.

3 Return Check Fee: If your check is returned unpaid, your account will be debited electronically for the original amount and electronically or via paper for the state s maximum allowable service fee. Payment by check constitutes authorization of these transactions. You may revoke this authorization by calling (800) , ext. 2, to arrange payment for any outstanding checks and service fees due. Drop Class: To drop from the studio, a full 30-day written notice must be turned into the office. Class changes of any kind must be recorded in our office on a Drop Class Form with the parent's signature or a written notice must be sent to the studio. Accounts will still be charged until a drop form is completed. DANCE/ATTENDANCE POLICIES Dance Classes: Students are expected to arrive 5-10 minutes prior to class. Please make sure you are on time to class. If you are more than 10 minutes late, you will not be allowed to dance. This is for our safety as well as the students, to ensure no injuries are sustained. All children under the age of 6 must be accompanied by an adult for the duration of their class, no exceptions. Missed Class: Students who will be absent from class must provide staff with notice to receive a make-up class. Students have 6 months from the date of the absence to make-up their class. Refunds or credits will not be given. Cancelled Class: In the unusual event that a class is cancelled by the staff, make-up classes or make-up times will be offered to the students. No credits or refunds will be given. Dance Wear: All students must wear something comfortable and easy to move in. We require all students have a leotard under their clothing. Absolutely no street clothes or street shoes allowed. Please review individual class requirements for each class. Video & Photo Policy There is NO videotaping or photography during regular weekly classes. Photos will be allowed with the prior permission of the teacher ONLY. Dancers Etiquette Respect for your teacher and fellow classmates is the number one lesson in dance class. Teachers may ask students to wait in the lobby if they are being disruptive or disrespectful. Students should not correct each other. Students should come to class with a positive attitude, ready to dance! Safety Students of all ages should be walked into the dance studio by their parent or chaperone. If they are to go home with someone other than the person who dropped them off, please let the teacher or office manager on duty know when they are dropped off. Parents should arrive to pick up their child BEFORE the class ends. Students should be picked up on time and should not be left in the lobby. Please make sure your child knows not to leave the studio without you. Please come into the studio to pick up your child. Students (of any age) will not be allowed to wait outside for their ride.

4 BIRTHDAY PARTIES Birthday parties are $30.00 per person. This price includes: Dance party, pizza, drinks, cupcakes, crafts, glitter fun, and gift bags. Parents don t do a thing simply come and enjoy or drop the kids off for a two hour break! Please see our staff for more information. HOLIDAYS All Holidays will follow the Dixon Unified School District Calendar Martin Luther King Day -- Presidents Day --Spring Break --Mayfair Friday May Fair Saturday-- Memorial Day --4th of July --Labor Day --Columbus Day --Veterans Day--Thanksgiving Week--Winter Break ANNUAL RECITAL Participation in our annual dance recital is not mandatory. If you do decide to participate, we require that you be at all the rehearsals, as well as, shows and stay until the end of the performance. Additional information is available on our website. 5 Dixon Dance Studio Studio Wide Dress Code Jazz Dress Code Your student is required to be in dance attire for Jazz class. This consists of leotard, dance shorts, dance leggings, etc. Jazz shoes must be worn. Hair must be pulled back in a ponytail. Tap Dress Code Your student is required to be in dance attire for Tap class. This consists of leotard, dance shorts, dance leggings, etc. Tap shoes must be worn. Hair must be pulled back in a ponytail. Ballet Dress Code Your student is required to be in dance attire for ballet class. This consists of a black leotard and pink tights (a ballet skirt may be worn). Ballet shoes must be worn and hair must be in a bun. Hip Hop Dress Code Your student is required to be in dance attire that is appropriate for Hip Hop class. This consists of grey or black sweats, or basketball shorts. Leggings may be worn only if there is a flannel/sweatshirt tied at the waist. A shirt and a leotard underneath must also be worn. Tennis shoes or combat boots may be worn, but please make sure that the soles have been cleaned with Clorox wipes or are only worn in the dance studio.

The bulletin boards should be checked regularly. It is also your responsibility to inform the school of any address or telephone number changes. Signing the registration form authorizes Dixon Dance Studio to use the registered student's image for Dixon Dance Studio advertising purposes. Please see instructor with any discrepancies. Dixon Dance Studio reserves the right to refuse service to any student, parent, or legal guardian for any reason, including, but not limited to, not adhering to the rules set forth in these policies and procedures. RELEASE FROM LIABILITY You understand that there are risks of physical injury associated with, arising out of, and inherent to the activity of dance. In recognition of this, you agree to release Dixon Dance Studio and hold them harmless of all liability, and hereby acknowledge that you are knowingly and voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in a dance class or other dance related activities.
