1 Run of Our Range Full Membership Plan This Membership Plan Requires a 12 Month Contract Paid Monthly or Prepaid in Full Full Membership Contract Requirements This membership plan requires a minimum twelve (12) month contract with automatic payments debited on a monthly basis, or prepaid in advance by cash, check, debit or credit card. Membership Categories Individual Membership: One youth or adult member Family Membership: Family is defined as up to two head of household adults and up to two minor children all living at the same address. Membership for additional family members may be purchased at a flat monthly per person fee. Plan Benefits Full monthly range access during any available public range hours Youth archers under the age of 13 must have a parent or guardian present at the range. 70% Equipment Rental Discounts Each and every time you come in. 10% Discount on Archery Equipment any stock or standard order item. Shipping charges may apply, special orders may require a non-refundable deposit. 50% Discount on Parties Groups, Birthdays, Family Gatherings 50% Discount on Lessons or Special Classes Excludes USA Archery Certification and Coaching Courses. This member plan level Includes participation in our USA Archery Programs. A USA Archery individual or family membership is required to complete this plan and fully activate your Black Sheep Archery membership. During the plan term you may participate in our JOAD or Adult Achievement program during every scheduled weekly class over the term of the twelve (12) month membership, provided your payments are current. Participation in all club qualifying events is included. Monthly club tournaments may be purchased at a cost of $8.00 per person, per event. Other tournament or special event fees may apply. Full League Night participation is included. Some leagues and special shoots may have additional costs.

2 Conditions of Membership for Black Sheep Archery General: In these Terms and Conditions Black Sheep Archery means Black Sheep Archery, its heirs and assigns; Member means the member(s) named on the attached Membership Agreement Form(s); the Range means the Black Sheep Archery Range located at 37 Cottage Street, Sanford Maine; Contract means the contract between Black Sheep Archery and the Member subject to the terms and conditions (a) set out below, (b) on the Membership Application Form and (c) in the plan benefits description; Membership Application Form means the application form completed by the Member to join the Club; and Plan Benefits means the benefits of membership set out in the plan description. Administration: Memberships are not refundable or transferable. Photo ID cards (e.g. Student card or driver s license) must be carried and shown upon request. Members are responsible for any access cards issued. Shared Range access with a non-member (not including a registered guest) will result in forfeiture of membership effective immediately. Each Member must respect other Range users and behave in an appropriate manner at all times. Inappropriate behavior at the Range includes, any illegal, dangerous or offensive acts or the taking of photographs without Black Sheep Archery Staff permission. This behavior can result in the Member being asked to leave the Range immediately and membership being terminated. Black Sheep Archery Staff reserve the right to rescind the rights of Members not complying with the terms and conditions of the membership. The terms and conditions of Black Sheep Archery may change from time to time without notice. No one under the age of thirteen years (13) may enter the Live Range area without a parent or guardian present. Appropriate covered footwear and a shirt must be worn at all times and the USA Archery dress code will be in effect during all scheduled qualifying events and activities. Direct Debit Payment: All Members shall pay an initial plan fee and access card fee (when required) in the sums specified within this document, before being issued with a membership card. The initial plan fee and card fee are not refundable under any circumstances. Membership is payable in advance annually, monthly or quarterly. In the event that the Member falls into arrears in respect of any fees or dues payable, Black Sheep Archery may levy a late payment fee of $ Membership dues for monthly or quarterly Members may be increased at the discretion of and at any time by black Sheep Archery, subject to at least one calendar months notice. Where the monthly or quarterly payment method is by direct debit, if any payment whatsoever under this Contract remains outstanding beyond the due date, the Member s signature on the direct debit instruction form shall constitute the Member s unconditional and irrevocable authority for Black Sheep Archery to debit the recorded credit/debit card account with the total amount due, without notice to the Member and charge the late payment fee.

3 Where the payment method is not by direct debit a deposit equivalent to one month s membership fee must be paid. All those members who cancel their direct debit with their bank will incur a $25 cancellation of direct debit fee. Late Payment: You acknowledge that this offer of membership is made on the basis of you strictly complying with the terms of this agreement. This includes paying the regular payments on the dates on which they are due, as detailed in this agreement. Should payment not be made within three days following the due date, a late payment fee of $10.00 in addition to the unpaid (or late payment) instalment is payable by you to Black sheep Archery. In addition to the late payment fee, you agree to pay interest at the rate of 1.5% per month [18% per annum] on all outstanding money, together with all costs of collection both prior to and post legal action, incurred by Black Sheep Archery, (including collection agency costs, court costs, and solicitors fees) regardless of judgment, all of which may be paid or incurred by Black Sheep Archery or its nominee. Suspensions: A Member may suspend their membership. A membership can be suspended for a minimum term of one month and maximum term of 12 months. A suspension fee of $5.00 is payable upon suspension. A Member must specify a start and finish date. Early reactivation is permissible. To suspend a membership, the Member must complete a suspension form and lodge it with Black Sheep Archery ten (10) working days prior to the commencement of their suspension. Access Cards: An access card fee of $35.00 must be paid upfront when applying for membership. All Members must bring their access card on each visit to Black Sheep Archery. All Members must register their attendance by swiping their access card at reception prior to entering the Range area. Members can obtain a replacement card if an access card is lost or stolen at a cost of $ If an access card is faulty a replacement card will be issued at no cost to the Member, only upon return of the original faulty card. All Members are responsible for their own cards. If a card is lost or stolen, Members must contact Black Sheep Archery immediately to cancel their card, as Members will be held accountable for any unauthorized activity that is committed by someone else using their access card. Access card fees are non-refundable. Cancellation of Membership: You agree that you will join Black Sheep Archery for the specified period as stated on the Membership Application Form. If you wish to cancel your membership you must give Black Sheep Archery twenty (20) working days written notice of your intention to cancel your membership. Black Sheep Archery reserves the right to terminate a membership immediately, if the Member has breached any terms of this agreement, by giving notice in writing or by verbal notice in person. *Initials:

4 Before signing this document, I acknowledge I have read, understand and hereby agree to the terms and conditions of membership as defined in the preceding section of this membership form, and I understand that this plan is not fully activated until I have provided proof of a purchased USA Archery individual or Family membership. I further acknowledge that I have read and signed the Black Sheep Archery waiver, and have had each additional plan member read and sign the Black Sheep Archery Waiver (Family Plans Only). In addition, I have completed an emergency contact form for each youth member under the age of 18, and a JOAD or Adult Achievement club registration for any new program archers.
