2 New User Registration Instructions (Already registered? Enter your username & password and click login.) To Begin Visit connect.viverae.com or click on the Good For You! Wellness Program login from schoolcare.org Click Register Today This will take you to a Self-Registration process STEP 1 Enter your Last Name Enter your Date of Birth (DOB) use format MM/DD/YYYY Employee Identifier: DOB + Last 4 of SSN (Ex: ) Spouse Identifier SP-DOB+ Last 4 of employee s SSN (Ex: SP ) 65+ Identifier: DOB + Last 4 of SSN (Ex: ) Enter the Registration Code: schoolcare Username* Password* Note: If you receive an error message in yellow, it is indicating some or all of the information entered in Step 2 is incorrect. If further assistance is needed, please contact the Viverae Health Center at Last Name* STEP 2 & 3 Review and ensure that all information is complete and current. Create a User Name & Password Select a Security Question and Answer and then click Save. Registration is complete! Click to edit Master title style Record your User Name, Password, and Answer to the Security Question in a safe location Note: Always click Log Out at the end of your session to protect your personal health information (PHI). 3 Hot Topics Forgot Username or Password? Visit connect.viverae.com Click Forgot? Answer the security question to login Unable to re-set yourself? Call Viverae at (888) choose option 4. Username* Password* Update Address Place your updated address under alternate address will automatically be sent to alternate and will be fully updated by the 1 st of the following month on your account. I recently moved and my address isn t updated If you are an active employee you will need to complete a change of address form with your Employer (spouses cannot update addresses). Your Employer will then provide the update to SCHOOLCARE Your address will be updated on your Viverae account by the 1 st of the following month Retirees may call SCHOOLCARE directly to update contact information. When will I receive my incentives? Incentive checks are tallied by the end of the month following the end of the quarter Ex: Q1 ends 9/30, checks will be tallied by 10/31 Either SCHOOLCARE or employees Employers will issue incentives How many incentives can I earn? 1 point = $1 Who can participate? SCHOOLCARE Health Insurance subscribers, their covered spouses and 65+ retirees. Employees can earn up to $800 per plan year Spouses and 65+ Retirees can earn up to $400 per plan year Cash incentives, payments and rewards paid to wellness program participants are taxable income. Please review with your tax consultant for more information. 3
4 Easily Navigate YOUR Wellness Account Click on Good For You! logo to go back to Home Page from anywhere on the website INCENTIVES PAGE Click Earn More to view your incentives page EARN MORE Click on Descriptions & Guidelines to view an outline of all programs and availability Under Actions, it will show the maximum amount of points you can earn per activity, and what you ve earned thus far 4. Click on the gray arrows to expand the action 5. To find out the details of each activity, click on the activity s title Descriptions & Guidelines Home Page Actions 4. The Home Page displays the amount of points that you ve earned (1 point = $1) You can also view your Health Score and Health Age. 5. Member Health Assessment RESOURCES The Resources tab contains the Document Library consisting of: Physician Lab Form to submit biometrics Certificate of Completion for Healthy Events Form to self-report physical activities and more Click on the blue button(s) on the right to download/print any of the documents. 4
5 Member Health Assessment (MHA) Complete Each Plan Year = 75 Points (7/1 6/30) The Member Health Assessment can be accessed from the Profile tab To begin, select the Complete Your Health Assessment link Here you ll find a series of questions on current lifestyle habits Once completed, if you ve already obtained your biometrics, you will receive a health score and health age based on the results of the MHA and biometrics Complete Your Health Assessment Tell US More About You 5
6 Biometrics Physician Form OR On-site Screening Event Physician Lab Form Option 1 Complete Each Plan Year = 100 Points (7/1 6/30) To locate the form click on Profile, Resources and Forms and Documents ~ Download and print Form ~ Send/Bring to your physician s office Physician s office completes the Form and either mails/faxes to Viverae or returns to you If Form is returned to you, it can be submitted securely by scanning the form and going to the Submit Form button on the Resources Tab. On-Site Screening Event Option 2 If you do not have recent lab work (back to 1/1/2016) or an upcoming physical you may attend an on-site screening Click on Notifications All available locations are listed (the times available are in blue) If there is a location you would like to attend, click on the drop-down box to select your desired time. Once you ve selected your time, click Schedule 4. You will see You re Going next to the event and an confirmation will be sent to you. Schedule 4. 6
7 Healthy Events Filling out the Certificate of Completion for Healthy Events One Form per Event = 25 Points Employees can do up to 3 per quarter Covered spouses and 65+retirees can do 1 per quarter The Certificate can be found under the Resources tab Select Forms and Documents Click on the Form to automatically download the Form and save the form to your computer, then close out of it Open Form from where you saved it on the computer (this enables the Form to become fillable so you may type in your information). Submitting the Certificate of Completion for Healthy Events From the homepage, click on Earn More, then select the current program quarterly activities. Scroll down to a Healthy Event, click on Self-Report Enter date you completed activity (comments are not required) 4. Select Attach File and locate the Form you want to upload 5. Once the Form is attached, click Save Once saved, you will see your 25 points have been awarded. Note: Healthy Event Forms can only be completed and submitted within the same quarter to earn incentives Date Completed* Comments 7
8 Preventive Care Services Add Completed Preventive Care Actions Complete two (2) gender and age-specific Preventive Care actions = 50 points (1/1-6/30) Under the Well-being tab, click on Care Select Add next to the action you want to add Select Appointment Date, Provider Name, and Provider Phone number and click Save 4. Once the Appointment has been entered successfully, the date the appointment was completed will be displayed After 2 are completed you can go to Earn More on your Home Page and see your 50 points Point available beginning in Quarter 3 Preventive actions can be added from 1/1/17 up to current appointments 4. 8
9 Peer Challenges Create A New Peer Challenge Click on the Well-being tab Select Available Challenges from the drop down menu Select Create a Peer Challenge 4. Select your challenge type, click Next 5. Determine the challenge parameters, click Next 4. Invite your Peers to your challenge (up to 19) 21 days of data are required in order to earn your incentive. Water Consumption Steps 4. Next Peer Challenge Research shows it takes 21 days for a new behavior to become habit, that s why SCHOOLCARE requires a minimum duration of 21 days for Peer Challenges Ex: If you are participating in a challenge, it s required that you enter a value every day, even if it s 0. This will serve as a placeholder to show you re actively participating in the challenge for the minimum of 21 days. Duration Based If the challenge is goal-based, the person who reaches the total number specified for the goal first will be the winner Back 5. Next 6. 9
10 Health Coaching Options to complete Health Coaching There are two options to complete a Health Coaching session: Telephonic coaching session: To schedule your session, call Viverae at (888) Secure messaging coaching session: Select the Messages icon in the upper right corner Here you ll find your inbox and the option to Compose a new message Based on your needs, choose the Health Coach that best suits you from the drop down menu 4.Compose your message and hit send, just like sending an Download the Viverae App! New Viverae Mobile App 4. With the new Viverae mobile app, you can participate in the Good For You! Wellness Program wherever your day may take you. Available for both Android and ios devices To download, search for Viverae in the app store or Google Play Once downloaded, enter your username and password to sign in You ll have full access to Good For You! including: Participating in Employer and Peer Challenges Tracking steps for the 250k Step Movement Chat with a Health Coach directly from your phone Sign up for on-site biometric events And so much more! 10

Quarter 1 (7/1/2017 9/30/2017) Biometrics- $100 Complete a Physician Lab Form or attend an on-site screening. Points available Quarters 1-4: July 1 to June 30 Quarter 2 (10/1/ /31/2017) Health Coaching- earn up to $75 Earn $25 each quarter when you engage with a Health Professional to support YOU in achieving your health goals. Points available Quarters 2-4: October 1 to June 30 Quarter 3 (1/1/2018 3/31/2018) Preventive Care Compliance- $50 Report a minimum of 2 of the recommendations listed on your portal.
