1 October 11, 2016 REQUEST FOR QUOTATION The Sheboygan Water Utility is accepting quotations for: 23% Hydrofluosilicic Acid H 2SiF 6, % Heavy Metals (as Pb), % <0.02 HF, % 0.5 Color, APHA <20 P 2O 5, % <0.2 Estimated Annual Usage: 12 Dry Tons Specifications: 1. Chemical: Hydrofluosilicic Acid shall meet the above requirements and conform to the latest AWWA Standard B as amended by Addendum B703a-13. Product must meet applicable NSF Standards and be approved for use in treatment of potable drinking water. Appropriate data, technical information, and Material Safety Data sheets shall be submitted with your proposal. 2. Delivery: Approximately 6 Each 2,000-gallon bulk truckload deliveries of 20,000 lb. maximum liquid weight each. Total dry weight based on 23% = 20+ tons. Price quote is to include delivery to the Filtration Plant at 72A Park Avenue in Sheboygan, Wisconsin. Unless a special request is made by the Utility, deliveries will be accepted only between the hours of 8 a.m. and 3 p.m., Monday through Friday. Supplier responsible for providing air/pump for off loading. No gravity off load available. 3. Duration of Contract: January 1, 2017 through December 31, Quoted price shall be firm for the duration of the contract period. 4. All prospective suppliers shall carry a minimum of $5,000, in liability insurance and submit proof of insurance certification with quotation. The supplier shall issue a 30-day written notice of cancellation, non-renewal or material change of liability insurance coverage. 5. The supplier to whom the contract is awarded will provide current, on site safety training to Utility employees within 30 days of the first delivery.

Chemical: Aluminum Sulfate shall meet above requirements and conform to the latest AWWA Standard B Product must meet applicable NSF Standards and approval for use in treatment of potable drinking water.
