2 FIRE AND EXTENDED COVERAGE INSURANCE It is required that both FIRE and EXTENDED COVERAGE be written. This program is designed to be used for 1, 2, 3 & 4 family dwellings only. Apartment houses, boarding and rooming houses and other commercial risks must be written under one of our Commercial programs - please submit to Home Office for acceptance and rating. Vandalism is an optional peril for Basic Form (DF-1). Vandalism may not be deleted on Broad Form (DF-2) or Special Form (DF-3). This program does not apply to farm property - refer to our Farm programs. POLICY TERM Policies are written on a 1 year basis. The policy will be automatically renewed each year with an updated declarations page. The policy will be subject to the premiums, rules and forms in effect at the time of renewal. Policyholders will initially receive copies of all policy forms. Upon renewal they will receive only an updated declarations and forms that are new or revised. Policyholders may receive all policy forms upon request a statement notifying the insured of this will be on each annual declarations. COVERAGES 1. Basic Form (DF-1) - Covers the Residence, Related Private Structures, Personal Property and Additional Living Costs/Fair Rental Value for direct physical loss to the property covered caused by a peril listed below unless the loss is excluded in the General Exclusions. Fire or Lightning Windstorm or Hail Explosion Riot or Civil Commotion Aircraft Vehicles Sudden and Accidental Damage from Smoke Sinkhole Collapse Volcanic Action Vandalism (Optional) NOTE: DF-1 only, Vandalism does not include loss to glass or safety glazing material. 2. Broad Form (DF-2) - Covers the Residence, Related Private Structures, Personal Property, and Additional Living Costs/Fair Rental Value for direct physical loss to the property covered caused by a peril listed below unless the loss is excluded in the General Exclusions. Includes all the perils listed for Basic Form (DF-1), plus: Vandalism Sudden and Accidental Tearing Apart, Cracking, Burning Burglary Damage or Bulging of a Heat or Air Conditioning System, Falling Objects or Water Heater. Weight of Ice, Snow or Sleet Freezing of a Plumbing, Heating, or Air Conditioning Accidental Discharge or Overflow of Water or System or Household Appliance. Steam Sudden and Accidental Damage from Artificially Generated Electrical Current. 3. Special Form (DF-3) - Covers the Residence and Related Private Structures for risks of direct physical loss unless the loss is excluded. Personal Property is covered for the same perils as provided in Broad Form (DF- 2). Additional Living Costs/Fair Rental Value is covered for the same perils applicable to the covered property damaged. 4. The following coverages are described in DF-1, DF-2 and DF-3 (unless indicated otherwise) and only apply if a limit is shown on the declarations for that coverage. Coverage A - Residence Coverage B - Related Private Structures (Up to 10% of the limit on residence automatically applies) Coverage C - Personal Property Coverage D - Additional Living Costs/Fair Rental Value (Up to 10% of the limit on residence automatically applies) 5. Liability and Medical Payments coverages are also available - see Liability paragraphs. 6. NOTE: There is no theft coverage provided by Basic Form (DF-1), Broad Form (DF-2) or Special Form (DF-3). 3 MINIMUM COVERAGE LIMITS All coverages are written on an ACTUAL CASH VALUE basis, however, to make it easy to determine minimum coverage amounts the minimums are based off of a dwelling's full replacement cost. Existing business will not be subject to the minimum dollar limits shown but will be subject to the percentage minimum shown. Seasonal Dwellings and Mobile Homes are subject to the minimum dollar limits shown. 1. DF-1: 50% of full replacement cost but not less than $30, DF-2 *: 60% of full replacement cost but not less than $40, DF-3 *: 60% of full replacement cost but not less than $50, * Exception: 80% or more of full replacement cost is required on Forms DF-2 and DF-3 if replacement cost loss settlement coverage is purchased. Automatic Inflation Guard will apply to dwellings written on a replacement cost basis. See the paragraph titled Replacement Cost Coverage for more details and the applicable charge. 5. Seasonal Dwelling: $15, Mobile Homes: $ 5,000 ELIGIBILITY A Dwelling Policy may be issued to provide insurance under: 1. Coverage A - on a dwelling building: a. Used solely for residential purposes except that certain incidental occupancies are permitted; b. Containing not more than 4 apartments; and c. Which may be in a townhouse or row house structure; or d. In course of construction. 2. Coverage A - on a mobile or trailer home: a. Using Basic Form (DF-1) only; b. Used solely for residential purposes except that certain incidental occupancies are permitted; c. Containing not more than 1 apartment; and d. At the permanent location described in the policy. 3. Coverage B - Related Private Structures: a. At the same location as the dwelling eligible for insurance under Coverage A; b. Not used for business purposes except a permitted incidental occupancy or when rented for use as a private garage; and c. At a separate location when used in connection with the insured location but not for business purposes. 4. Coverage C - Personal Property in: a. A dwelling, mobile or trailer home eligible under Coverage A; or b. A dwelling with rental apartments including furnishings, equipment and appliances in halls or utility rooms; or c. Any apartment, cooperative or condominium unit used as private living quarters of the insured or rented to others. 5. Coverage D - for the additional living costs incurred to maintain the insured s household or the loss of the fair rental value of: a. A building eligible for insurance under Coverages A or B; or b. 4 UNDERWRITING RULES 1. A completed Dwelling Fire application must be submitted along with at least two recent photos of the dwelling (front and rear). Photos of all structures on the premises should be submitted for review. 2. Total dwelling insurance must equal 50% or more of full replacement cost but not less than $30,000 for Basic Form (DF-1), 60% or more of full replacement cost but not less than $40,000 for Broad Form (DF-2) and 60% or more of full replacement cost but not less than $50,000 for Special Form (DF-3). Exception: 80% or more of full replacement cost is required on Forms DF-2 and DF-3 if replacement cost loss settlement coverage is purchased. Automatic Inflation Guard will apply to dwellings written on a replacement cost basis. See the paragraph titled Replacement Cost Coverage DF-1467 or Replacement Cost Coverage DF-1786 on page NSDWG 18 for more details and the applicable charge. Attach a MSB RCT calculation for all dwellings insured. 3. Maximum limits available for dwellings insured under Coverage A are: Protected - $400,000; Partially Protected - $300,000; Unprotected - $200, Minimum coverage written on seasonal dwellings is $15,000. Minimum coverage written on mobile homes is $5, Any existing hail or windstorm damage must be disclosed on the application. 6. Owner-occupied mobile homes less than 15 years of age are eligible. 7. Dwellings must have a solid and continuous foundation. 8. A central heating system is required for all dwellings rented to others. 9. Following is a list of Dwelling Fire risks not to be bound and generally will not be written: a. Property in generally below average physical condition. b. Risks cancelled or non-renewed by other carriers. c. Risks which have sustained 2 OR MORE losses during the past 3 year period. d. VACANT DWELLINGS. Refer to page NSDWG 4. e. Risks with older wood roofs or risks with asbestos slate roofs or siding, or tile roofs. f. Risks where insureds have a history of financial instability within the last 5 years. g. Hobby farms or any risk formerly occupied as FARM PROPERTY on which any former farm buildings still exist. h. Risks with more than 2 related private structures or with a related private structure larger than 1,600 square feet. Refer any structure over 1,600 square feet to underwriting. i. Unusual risks including basement homes, log homes, earth homes, dome homes and homes with flat roofs. j. Risks with unvented wall furnaces. k. Rental or seasonal mobile homes unless we have other good supporting coverage. The mobile home must be permanently located. l. Tenant occupied dwellings with wood burning/solid fuel equipment. m. Risks with permanent built in swimming pools. DEFINITION OF A SINGLE BUILDING All buildings or sections of buildings which are accessible through unprotected openings shall be considered as a single building. Buildings which are separated by space shall be considered separate buildings. Buildings or sections of buildings which are separated by an 8 inch masonry party wall which pierces or rises to the underside of the roof and which pierces or extends to the inner side of the exterior wall shall be considered separate buildings. Accessibility between buildings with independent walls or through masonry party walls described above shall be protected by at least a Class A Fire Door installed in a masonry wall section. DWELLING UNDER CONSTRUCTION - DF-1375 For dwellings under construction, coverage must be written at 100% of completed value. Two methods are provided for insuring a dwelling under construction. 1. Named Insured Is The Intended Occupant - A builder (contractor) may be designated as an additional insured. Pro rata cancellation permitted upon completion of the dwelling. The policy premium shall be 65% of the premium for an owner occupied dwelling. 2. Named Insured Is Not The Intended Occupant - Policy shall specify building is in course of construction and permission is granted to complete. The policy premium shall be 100% of the premium for a non-owner occupied dwelling. North Star Mutual Ins. Co. NSDWG - 3 Renewals
5 LOSS SETTLEMENT Basic Form (DF-1), Broad Form (DF-2) and Special Form (DF-3) - All forms provide coverage on an ACTUAL CASH VALUE basis. This means that loss to all covered property is subject to applicable depreciation. Loss to property covered under Coverages A and B (form DF-2 or DF-3 only) can be changed to a replacement cost basis if DF-1467 or DF-1786 is added and an additional premium is charged. The building(s) must be insured for 80% or more of full replacement cost. Inflation Guard will automatically apply to the building(s) written on a replacement cost basis. See the paragraph titled Replacement Cost Coverage DF-1467 or Replacement Cost Coverage DF-1786 for more details. EXCLUSION OF LOSS DUE TO BY-PRODUCTS FROM PRODUCTION OR PROCESSING PRODUCTS DF-2119 Coverage will be excluded for loss to an insured premises caused by smoke, vapor, gas, or any substance released in the course of production or processing of any controlled substance for any rental dwelling and can be excluded on any other dwelling, farm barn, building, or structure unless the loss is caused by fire or explosion that results from the release of any such substances. Attach Endorsement DF INFLATION GUARD Buildings written on an Actual Cash Value Basis. (optional) The policy may be endorsed to provide automatic annual increases in the Coverage A - Residence and Coverage B - Related Private Structures limits based on changing dwelling construction cost factors. Indicate which building(s) Inflation Guard is to apply to on the application. Buildings written on a Replacement Cost Basis. (mandatory) The Inflation Guard Provision is required on all buildings written on Replacement Cost Basis. PROPERTY COVERAGES will automatically be updated annually based on changing dwelling construction cost factors. The residence must be insured to at least 80% of Replacement Cost, the application must be completed and a photograph of the residence attached. To be assured that the insured meets policy conditions, it is recommended that all residences be insured to 100% of Replacement Cost. MASONRY VENEER Dwellings of masonry veneer construction and their contents and Apartment Contents in masonry veneer buildings are subject to Masonry premiums. VACANT DWELLINGS Vacant/unoccupied dwellings are not eligible as a new business risk. Coverage may be continued on dwellings currently written with North Star Mutual if they become vacant/unoccupied during the policy term. Coverage will be written on Basic Form DF-1 with no coverage for V & MM. A 40% vacancy charge will be applied. MINIMUM PREMIUM 1. A minimum annual premium of not less than $150 must be charged for each policy. 2. The $150 minimum annual premium may include all chargeable endorsements or coverages for Fire and Allied Lines. 6 CALCULATION OF PREMIUM North Star has an automated rating system for the Dwelling Program that can be accessed on the Internet. Go to the North Star web site ( click on Agents Only, enter your User ID and Password, click on submit, click on Dwelling System. Step 1 - Determine the Territory 1 Base Premium based on the number of families, protection, and construction classifications that apply and determine the occupancy classification. Step 2 To determine rates for other territories, apply applicable Location of Risk Modifier on page NSDWG - 13 to the rate determined in Step 1. Step 3 - If Coverage A applies, select the Coverage A premium for the policy limit, apply the factor for the deductible option selected, and apply any special rating factors. For Form DF-1 only, if vandalism does not apply, reduce the premium by the vandalism discount, apply the factor for the deductible option selected, and apply any special rating factors. The Coverage A premium is the sum of these premiums. Step 4 - If Coverage C applies, use the Coverage C premiums and follow the process described in Step 3. Step 5 - If any premium modifications apply, adjust the Coverage A and/or Coverage C premiums determined in Steps 3 and 4. Step 6 - If any coverage options apply, add the additional charges to the Coverage A and/or Coverage C premiums determined in Step 5. Step 7 - If both Coverage A and Coverage C apply, the total policy premium is the sum of the premiums determined in Step 6. FIRE PROTECTION CLASSIFICATION Protected: Building is located within 1,000 feet of a fire hydrant and within five (5) road miles of a responding fire department. Partially Protected: Building is located over 1,000 feet from a fire hydrant, but is within five (5) road miles of a responding fire department. Unprotected: All Others. CONSTRUCTION DEFINITIONS 1. Frame (FR) - Exterior walls of wood or other combustible construction. 2. Brick, Stone or Masonry Veneer (MV) - Exterior walls of combustible construction veneered with brick or stone. 3. Brick, Stone or Masonry (JM) - Exterior walls constructed of masonry materials such as adobe, brick, concrete, gypsum block, hollow concrete block, stone, tile or similar materials and floors and roof of combustible construction. 4. Mobile Home (MH). 5. Aluminum, Plastic, or Steel Siding over Frame (SF) - Exterior walls of wood or other combustible construction and covered with aluminum, plastic or steel siding. 6. Stucco (ST) - Exterior walls of wood or other combustible construction and combined with stucco. 7. Fire Resistive (FS) - Exterior walls and floors and roof constructed of masonry or other fire resistive materials. NOTE: Mixed (Masonry/Frame) - a combination of both frame and masonry construction shall be classed and coded as frame when the exterior walls of frame construction (including gables) exceed 33 1/3% of the total exterior wall area; otherwise class and code as masonry. 7 PREMIUM CHARTS AND COMPUTATION The premiums shown in this manual section are annual premiums and are for a $1,000 deductible. All premiums are to be rounded off to the nearest dollar. Fifty cents and above shall be considered the next higher dollar. Premiums shown in this manual section are to be applied separately to each building and to the contents of each building. The gross annual premium for each building item and each contents item shall be the premium shown in the tables on pages that follow according to the number of families, the construction and protection class. 15 OPTIONAL COVERAGES BACK UP OF SEWER, DRAIN OR SUMP SYSTEM - DF-208 (Policy Forms DF-2 and DF-3) (This is not Flood Insurance) For an additional premium coverage for direct loss to covered property caused by water which enters the dwelling through sewer, drains or sump systems may be added. This coverage is available when the Broad Form (DF-2) or Special Form (DF-3) is written. This coverage does not apply to Coverage B or D. This coverage does not apply to Debris Removal under Incidental Coverages or to a loss caused by the named insured's negligence. The policy deductible amount and provisions apply to this coverage. Limit of Liability: $1,000 $2,500 $5,000 Additional Premium: $15 $30 $45 NOTE: An agent cannot bind coverage. Coverage does not become effective until 15 days after the effective date shown on the Declarations issued for this coverage. CONDOMINIUM UNIT-OWNERS ADDITIONS Broad Form - Unit-Owners - DF-32 When policy form DF-2 is issued to a condominium unit-owner to cover the unit contents, 10% of the Coverage C limit is automatically extended to cover the unit-owner s interest in fixtures, additions, alterations, structures, installations, and other building related items. Coverage applies on a broad perils basis at no extra charge. The limit that applies will show on the Declarations as Coverage A - Condominium Fixtures, Additions, and Alterations. The 10% limit may be increased for an additional charge shown in the chart below. The additional charge applies only to the amount of increase. Protected Part Protected & Unprotected Increased Limit (per $1,000) Special Form - Unit-Owners - DF-33 Coverage for the unit-owner s interest in fixtures, additions, alterations, structures, installations, and other building related items may be changed from named perils coverage to risks of direct physical loss unless the loss is excluded from coverage. Use the Base Limit rate in the chart below to calculate the charge for converting the base limit (10% of Coverage C limit) to special perils coverage. If a higher limit applies then also charge the Increased Limit rate in the chart below to the amount of increase. Protected Part Protected & Unprotected Base Limit (per $1,000) Increased Limit (per $1,000) Coverage for condominium fixtures, additions, alterations, structures, installations, and other building related items may be provided on a replacement cost basis for an additional charge (DF-32 or DF-33). The additional charge is calculated by adding the base limit (10% of Coverage C limit) and increased limit, if any, together and multiplying by 0.50 per $1,000. 16 LOSS ASSESSMENT AND ASSOCIATION DEDUCTIBLE - SCHEDULED LOCATIONS - DF-50 Property coverage for loss assessment and association deductible arising out of a scheduled location can be added. The location this coverage applies to must be listed with a limit shown for that location. Loss Assessment coverage (under Incidental Coverages) applies when the insured s financial obligation arises out of an assessment levied by the association and that results from a loss to property that is owned collectively by all association members. Association Deductible coverage applies when a master policy deductible is charged against the insured during the policy period and results from a direct loss to property that: would be eligible for coverage under the Dwelling Fire policy if it was owned by an insured or was an insured s insurance responsibility; is covered under the association s master policy; and is caused by a peril insured against, other than earthquake, that is covered by the Dwelling Fire policy. 17 MOBILE OR TRAILER HOMES (Fire Policy) Underwriting Requirements: 1. Must be permanently located on permanent foundation. 2. Must be written with other good supporting coverage. 3. Can be written only under Basic Form (DF-1). Not acceptable for Broad Form (DF-2) or Special Form (DF-3) coverage. Annual Premium: The annual Basic Form (DF-1) premium shall be that shown in the Base Premium Chart for a 1 family frame dwelling under the same protection class plus the following loading per $1,000: Mobile/Trailer Home - Occupied by owner... $10.00 Mobile/Trailer Home - Not occupied by owner Contents - All occupancies Seasonal Mobile Homes shall be rated the same as a non-seasonal Mobile Home. For minimum coverage amount, refer to the Minimum Coverage Limits paragraph on page NSDWG-2. RELATED PRIVATE STRUCTURES The perils and deductible option must be the same for all Coverage A and Coverage B structures covered by the same policy. Apply the appropriate deductible factors to the rating of Related Private Structures. 1. Coverage is provided in the forms on a blanket basis as follows: Basic Form (DF-1), Broad Form (DF-2) or Special Form (DF-3) - Up to 10% of the residence limit is available as additional insurance. The blanket limit may not be increased. To be eligible for coverage under this rule, a private structure cannot be: -- used for business purposes; or -- rented or held for rental to anyone other than a tenant of the residence, except for use solely as a private garage. 2. Increased Limit - An additional amount of insurance can be provided for specific private structures that are covered under Coverage B. Coverage B is intended to provide coverage for structures that are pertinent to the household and not used or designed for business purposes. Typically these are detached garages or small backyard storage sheds for lawn mowers, snow blowers, garden equipment, etc. Use the rating information shown in item 6. below. Endorsement ML-48 will be used and entries to describe the covered structures and show the additional amount of coverage that applies to each structure will be made. 3. On Premises - Specifically Scheduled Structures - When an acceptable private structure is over 1,600 square feet or there is more than 1 private structure on the premises then the private structure must be specifically scheduled for its actual cash value. The limit that applies to Coverage B does not apply to specifically scheduled structures. With underwriting approval, a building is acceptable when the following are met: a. Building is not used or designed for business purposes. b. Building must be in good condition. c. Building must be written at 100% of actual cash value. Use the rating information shown in item 6. below. Endorsement NS-248 will be used and entries to describe the covered structures and the amount of coverage that applies to each structure will be made. 4. Residential Rental to Others - A private structure rented, in whole or in part, to others for dwelling purposes shall be rated as a non-owner occupied, Coverage A - Residence. It will then be designated as a Coverage A - Residence on the declarations. 5. Other Related Private Structures - Other related private structures, not described above, can be covered if they are not: -- used for business purposes; or -- rented or held for rental to anyone other than a tenant of the residence, except for use solely as a private garage. Rate each structure separately as a one-family, owner-occupied Coverage A - Residence. The structure will be described on the declarations along with its location and the limit that applies. For Replacement Cost Coverage on the Dwelling, increase base premium by 10% and schedule the dwelling on DF For Replacement Cost on Related Private Structures scheduled on NS-248, increase the premium charge for the scheduled structure by 10% and schedule the related private structure on DF NOTE: Inflation Guard will automatically apply to those buildings written on a replacement cost basis. The limits of insurance for the building(s) will automatically increase annually based on changing construction costs factors. To qualify for Replacement Cost Coverage, the following Eligibility Rules/Underwriting Requirements must be met: 1. Recent photos showing all roof surfaces must accompany each application or endorsement request. 2. The dwelling must be insured to at least 80% or more of its full replacement cost. A dwelling that has replacement value substantially greater than the market value is not eligible. 3. The Related Private Structure must be specifically insured for a minimum of 80% of its full replacement cost. 4. The policy must be written on a Broad Form (DF-2) or Special Form (DF-3) policy. 5. Any existing hail or windstorm damage must be disclosed on the application or endorsement request. 6. There cannot be over one layer of roofing (shingles). 7. Dwellings or buildings with wood shingles are not eligible for replacement cost. 8. Dwelling updates must meet the following requirements: a. Electrical 100 amps. b. Plumbing updated in the last 40 years. c. Heating replaced in the last 30 years. d. Shingles - 10 years of age or less.
