The applicant has not been convicted of arson or insurance fraud. 2. The dwelling has permanently installed water, electricity and sewage utility services. (DP-2 only) 3. The dwelling does not have any existing structural damage. 4. The dwelling does not have knob and tube wiring or electrical services with less than 100 amp service. 5. The dwelling is not under construction/major renovation. 6. The dwelling is not in foreclosure. 7. The dwellings primary source of heat is not a wood, coal or pellet burning device. (DP-2 only) 8. There are no liquid fuel-powered space heaters or any heat reclaiming devices in the dwelling. 9. The dwelling does not have External Insulation Finish System (EIFS) siding. 10. The dwelling does not have other structures or garages with a wood/coal/pellet burning device. 11. The dwelling does not have polybutelene pipes. (DP-2 only) 12. Explosive or flammable materials are not stored on the premises. 13. Brush clearance is more than 350 feet around the home. 14. The dwelling is not located next to any burned out or abandoned buildings. 15. There is no childcare, homecare, lodging, auto repair and/or chemical processing conducted on the premises. 16. The dwelling is not a manufactured home, dome home, log home, straw built home or condominium. (manufactured homes are eligible for vacant dwelling program) 2

4 BINDING RESTRICTIONS 1. No coverage may be increased within 72 hours of the announcement of an impending disaster, i.e. hurricane, tropical storm, volcanic eruption, earthquake, flood, mudslide, brushfire, etc. 2. Earthquake: If an earthquake occurs generating a reading of 5.0 or greater on the Richter Scale, new business policies and increased coverage requests on inforce business policies will be restricted on the day of the earthquake and for a 3 day period following the earthquake. Any aftershock readings of 5.0 or greater will be considered a new earthquake. The restrictions will apply for risks located within 150 miles of the earthquake s epicenter. 3. Wildfire: No new policies, binders or increases in coverage will be accepted when a wildfire is within 25 miles of the home. QUOTING PROCEDURE All quoting and issuance of policies for these programs is available through i-reli. FOLLOW-UP ITEMS The following items will require the agent/broker to submit items to the General Agent upon binding of an application. Photos 1. Photos of the front and back of the home are required on homes. 2. Photos are required of Other Structures. 3. Photos are required on all Wood Burning Stoves that are not professionally installed. 3

5 GENERAL INFORMATION 1. ELIGIBLE RISKS: A. One to four family dwellings which are owner occupied (primary/permanent or seasonal/secondary) or rental in nature; B. One to four family vacant dwellings; C. One to four family dwellings under renovation. 2. SINGLE DWELLING DEFINITION: A. All dwellings or sections of dwellings which are accessible through unprotected openings shall be considered a single dwelling. B. Dwellings which are separated by space shall be considered separate dwellings. C. Dwellings or sections of dwellings which are separated by an 8 inch masonry party wall which pierces or rises to the underside of the roof shall be considered separate dwellings. Communication between dw ellings with independent walls or through party walls, must be protected by at least a Class A Fire Door installed in a masonry section. 3. CONSTRUCTION DEFINITIONS: A. Frame Class - Exterior walls of wood or other combustible construction, including wood ironclad, stucco on wood or plaster on combustible supports. B. Masonry Veneer - All exterior walls of combustible construction, veneered with brick or stone. For rating Purposes, Masonry Veneer should be rated as Masonry. C. Masonry - Exterior walls constructed of masonry materials such as adobe, brick, concrete, gypsum block, hollow concrete block, stone, tile or similar materials, and floors and roof of combustible construction. 4. PROTECTION CLASSIFICATION We utilize the ISO Protection Class listings in the Public Protection Classification manual and apply these to risks insured under the Dwelling Program policies. The Protection Class indicated applies in a municipality or classified area where a single class of fire protection is available throughout (8, 7, etc.) In a classified area where 2 or more classifications are shown (e.g. 6/9), the classification is determined as follows: Distance To Fire Station Class 1 5 road miles or less with a hydrant or ISO approved water source within 1,000 feet *First protection class (e.g. 6/9. Use 6) 2 5 road miles or less with a hydrant or ISO approved water source beyond 1,000 feet 3 Over 5 road miles All other properties are Protection Class POLICY TERM: A policy will be written for a twelve (12) month term. 6. INSTALLMENT BILLING OPTION: Any policyholder may be given the opportunity to pay by installments. All installments will include a $6 fully earned service fee (not applicable to the down payment). Any policy may be paid in full. 7. WHOLE DOLLAR PREMIUM ROUNDING: All policy and endorsement premiums will be a full dollar amount. For this purpose, an amount of fifty (50) cents or more will be rounded to the next whole dollar. 8. MINIMUM PREMIUMS: A minimum written premium will apply to all new and renewal policies on a per unit basis. The minimum premium is $100. A minimum earned premium of $100 will apply to all new and renewal policies. 9. CHANGES: All changes requiring adjustment of premium will be computed pro rata. 10. CANCELLATIONS: If the insurance is canceled at the request of the Company or the insured, the unearned premium will be computed on a pro-rata basis. 4

6 GENERAL INFORMATION (Continued) 11. OCCUPANCY DEFINITIONS: Primary/Permanent: Dwelling owned by the named insured and occupied by the named insured on a primary/permanent basis. The dwelling must be occupied by the name insured for at least (9) months. Rental: Dwelling owned by the named insured but occupied by others. The dwelling is rented to others for residential purposes. Seasonal/Secondary: A dwelling that is not used as a primary residence of the name insured, but one that is used on an intermittent basis by the name insured. Vacant/Unoccupied: Homes that are unoccupied whether or not the contents have been removed. 5 Renovation: An existing home that is undergoing remodeling or construction. 12. WAIVER OF PREMIUM: When a policy is endorsed after the inception date, additional or returned premium up to $5 may be waived unless the named insured requests the unearned premium be refunded. 13. AUTOMATIC INCREASE IN INSURANCE: An inflation guard factor is included on all policies with a replacement cost loss settlement. OPTIONAL COVERAGES RESIDENTIAL BURGLARY COVERAGE: (Available for DP1 Only - Available for primary/permanent risks and rental risks) Rate per $1000 of coverage. 7 BILLING Payment Plan Options If the policyholder desires to pay their premium on an installment basis, we will allow a 2-payment, 4-payment, or 8- payment option to be selected. Each installment (not applicable to the down payment) will include a $6 service charge. 2-Payment Plan: 50% down payment is required, plus the policy fee and tax with the remaining balance due on Day Payment Plan: 25% down payment is required, plus the policy fee and tax with the other payments due on Day 70, Day 140 and Day Payment Plan: 20% down payment is required, plus the policy fee and tax with the other payments due on Day 44, Day 88, Day 132, Day 176, Day 220, Day 264, and Day 308. Payment Options Credit Cards and Personal Checks are the 2 forms of acceptable premium payment. Credit Card Policyholders have the option to make credit card payments 3 different ways. PayAssurant.com: This website allows either the policyholder or agent/broker to apply payment to an existing policy. Three pieces of information are needed to apply payment: policy number, agent number (8 digits) and name insured mailing address zip code. The website address is: i-reli Payment Tab: Agents/Brokers may apply payment at time of policy issuance or access the i-reli payment tab for existing policies upon logging into i-reli. Customer Service Support Line: Policyholders may call and follow prompts to reach our Customer Service team. A customer service associate will take policyholder credit card information and apply a payment. We accept the following Credit Cards: Visa and Master Card. Personal Check Send all personal checks to the lock box address closest to the billing address. A $15.00 return check fee will be accessed for each check returned by the bank. This will ensure that payments are applied properly if the check becomes separated from the envelope. Payment Status There are 3 ways to obtain policy payment/billing information. i-reli Billing Tab: Agents/Brokers can access real-time billing information through the i-reli Billing Tab. Information concerning payment status, future payment schedules, cancellations and non-renewals are housed in this tab. Assurant s Automated Payment Line: Call to check payment status through an automated phone service. When asked for policy number, enter only the last 6 numeric digits followed by the term number (Example: I , enter ). The policy number will always be 6 digits and a 2 digit term number (8 total digits).
