6 Anthem Medical Plan Deductible and Out of Pocket Accumulations are Embedded. This means that each member has an individual deductible/oop amount. No individual family member is required to contribute more to the family deductible than their individual deductible amount. The Out Of Pocket accumulates on an embedded basis also. 6 of 33

7 125 Company HRA Benny Card An HRA (Health Reimbursement Account) is an employer funded account to help pay for your medical expenses that are subject to the medical deductible. You will receive a Visa branded Benny card (in a nondescript white envelope) to use at the pharmacy, doctors office, or medical facilities to pay your deductible expenses. This card can only be used for eligible medical deductible expenses. Synaptek will pre-fund (on your Benny card) the full innetwork deductible in the following amounts: Employee Only: $3,000 Employee & Spouse; Employee & Child(ren); Family: $6,000 7 of 33

8 125 Company HRA Benny Card All deductible expenses must occur in your plan year (Oct 1 Sept 30) to be reimbursed. You will have 90 days after your plan year ends to submit any remaining claims for reimbursement. Funds do not roll over from year to year. You can swipe your card to pay for services or you may pay on your own and submit receipts for reimbursement. Please keep all receipts as they may be requested to substantiate claims. This is required by IRS guidelines. 8 of 33

9 125 Company HRA Benny Card HRA How to pay for expenses When you go to an In-Network doctors office or a medical facility: Present your medical ID card, the doctor will file the claim with Anthem, you will receive an Explanation of Benefits (EOB) from Anthem and a bill from the doctor after the claim has been finalized. You can then pay the bill with your Benny card. When you go to a pharmacy: Present your medical ID card, the pharmacy will run the claim through Anthem and tell you the amount due at the time you pick up your prescription. You can use your Benny card to pay for the prescription at that time. 9 of 33

12 Delta Dental Plan Network Differences Dental - Important Information Seeing a Delta Dental PPO dentist will be your least expensive dental option. These dentists are contracted with Delta Dental to accept payment in full based on the allowance and any co-insurance you are responsible for. Your co-insurance responsibility is lower with a PPO dentist than with other types of dentists. A Delta Dental Premier dentist will accept the allowance and co-insurance as payment in full, but you are responsible for higher co-insurance amounts when you see a Premier dentist. 12 of 33

19 Voluntary Life / AD&D Insurance Employee Guarantee Issue: Up to $100,000 Spouse Guarantee Issue: Up to $25,000 Child Guarantee Issue: Up to $10,000 Evidence of Insurability* is required for amounts over the Guarantee Issue amount. Guarantee Issue is only available if you enroll when first eligible. 20 125 Company Medical FSA A medical FSA can be used to pay for eligible medical, prescription, dental and vision expenses with pre-tax dollars. The FSA is funded solely by you as the employee. The Annual election maximum for October 1, 2017 to September 30, 2018 will be $2, of 33

21 125 Company Medical FSA The current FSA plan runs until Sept 30, The new plan will start on October 1, The FSA includes a Grace Period of 75 days to incur claims. This means that you have until December 15, 2017 to incur claims. Any claims incurred after September 30 th (that you wish to use any remaining FSA dollars from 10/1/2016 to 9/30/2017 for) will need to be submitted for reimbursement the debit card cannot be used. You will have an additional 15 days after the Grace Period ends to submit any remaining receipts for reimbursement. 21 of 33

22 125 Company Dependent Care A Dependent Care FSA can be used to pay for eligible dependent care expenses with pre-tax dollars. Eligible expenses include day care, before and after school care, nursery or pre-school, summer day camp Eligible dependents are children under age 13 and a spouse or dependent unable to care for themselves while you work The current plan runs until Sept 30, The new plan will start on October 1, The Annual election maximum is $5,000 per family 22 of 33

29 Frequently Asked Questions Q: What is the difference between an HRA and an FSA? A: The HRA is an account funded by your employer and pays only for your medical deductible. The FSA is an account funded by you that can be used to pay for any eligible medical, dental and vision expenses. Q: What accounts can I use the 125 Company Visa Benny Card for? A: The Benny Card can be used for both the HRA and the FSA account. The money will be in separate accounts on the card. When your card is swiped or a claim is submitted it will pull from the appropriate account based on the merchant code and what the service applies to (deductible, co-pay after deductible, dental expense, vision expense, etc) 29 of 33

30 Frequently Asked Questions Q: What expenses can I pay with my HRA? A: Only expenses related to the Medical Deductible are eligible to be paid with the HRA. Q: Why do I need to submit a receipt for my HRA or FSA claim? A: IRS regulations require that all claims need to be reviewed to confirm that they are eligible expenses. While many claims can be auto-adjudicated, some claims cannot and require a receipt as proof of being eligible in order to be paid. Q: Will I get a new Visa Benny card each year? A: No. It can be used until the expiration date listed on the card. You will get a new card once it expires. 30 of 33

31 Enrollment Instructions All enrollments are due by Friday September 29 th Medical, Dental, and Vision enrollments will automatically continue as is. For changes elect through the PayCom portal. Medical FSA and Dependent Care MUST be re-elected through the PayCom portal every year. Transit and Parking automatically continue on a monthly basis. You may change these elections monthly on the PayCom portal. 31 of 33
