Anthem Way Florence, AZ Welcome to the Festival This family-friendly event celebrates the season with regional artists and vendors, children s activities, beer & wine, food trucks and live entertainment in the outdoor amphitheater. The 2017 Anthem Celebrates the Arts event provides opportunities for not only vendors and artists to display or sell their work but businesses to sponsor and advertise at a worthwhile community experience. Sponsorship is being sought to support activities and special events during the Festival so do not miss this opportunity to promote your company to the region at large. 4 Anthem Celebrates the Arts Festival Vendor Information BUSINESS/FOOD PERMIT/INSURANCE TOWN OF FLORENCE SPECIAL EVENT VENDOR PERMIT All exhibitors MUST complete a Vendor Form and submit it to the Town of Florence, regardless of whether you are selling items or just sharing information. Forms can be found at Search special event vendor packet BUSINESS OWNER INSURANCE REQUIREMENTS Please direct all insurance questions to Melody Yahrmarkt at or myahrmarkt@aamaz.com. Business vendors must complete and sign waiver of liability on page 4. Business owner participants shall provide and maintain prior to and during participation at least the following types and minimum amounts of insurance: 1. Statutory Arizona Workers Compensation and Employers Liability Insurance with employer s liability limits not less than $100,000/$100,000/$500,000 2.Commercial General Liability Insurance with the following minimum limits: a) Bodily Injury & Property Damage - $1,000,000 per occurrence b) Personal Injury & Advertising Injury - $1,000,000 per occurrence c) General Aggregate Limit - $2,000,000 d) Products & Completed Operations Liability Aggregate Limit - $2,000,000 The Commercial General Liability Insurance must include an endorsement naming the following as additional insureds on a primary and non-contributory basis: Anthem at Merrill Ranch Community Council, Inc., Poston Butte Golf Club, LLC, Anthem Parkside at Merrill Ranch Community Association, Inc., Sun City Anthem at Merrill Ranch Community Association, Inc., AAM, LLC and all their respective directors, officers, members, managers, community managers, employees, agents, volunteers, and staff 3. Automobile Liability Insurance - $1,000,000 CSL each accident, including owned, hired and non-owned liability (if there are no owned autos, this coverage may be included in the above Commercial General Liability Insurance) 4. Professional Liability Insurance - $1,000,000 per claim and aggregate (if applicable) Event vendors must submit a certificate or certificates of insurance including copies of additional insured endorsements evidencing compliance with the above requirements with their Application to Exhibit to: Anthem at Merrill Ranch Community Council, Inc. Anthem Way Florence, AZ Program: Anthem 2017 Celebrates the Arts Festival Waiver of Liability and Hold Harmless Agreement I agree to exercise due care and diligence while participating in this event. I also agree to follow all directions and rules, if any, given to me. I agree to release and expressly waive any claims that I may have against Anthem at Merrill Ranch Community Council, Inc., Poston Butte Golf Club LLC., Anthem Parkside at Merrill Ranch Community Association, Inc., Sun City Anthem at Merrill Ranch Community Association, Inc., AAM, LLC and all of their directors, officers, members, managers, community managers, employees, agents, volunteers, attorneys, producers and staff that may arise out of or relate to this event. I agree not to sue any of the released parties above regarding this event. This express waiver of claims pertains to all claims that I or anyone related or affiliated with me may sustain, including but not limited to claims for death, bodily injury, economic damages and property damages. This express waiver and release shall bind the members of my family and spouse and my heirs assigns and personal representative. In the event any claim is made against any of the parties released above arising out of or related to my involvement in this event, whether or not suit is filed, I agree to defend and indemnify the Association. Specifically, I will pay for the attorney of the Association s choice to defend the claim and will pay for any resulting payment necessary to settle the case, including but not limited to satisfying any judgment. In signing this release, I acknowledge and represent that I have read and understood the foregoing language, that I either had an attorney review it or declined to do so, and am signing it voluntarily as my own free act without any representations from the release parties above.
