2 Why this course? The Cranfield Safety and Accident Investigation Centre (CSAIC) is pleased to offer this exciting six-week intensive Marine Accident Investigation course. The course content has been carefully developed in close collaboration with the accident investigation community, including the UK s Marine Accident Investigation Branch (MAIB), and with the endorsement of the Centre s Industry Advisory Board. In collaboration with the UK s Marine Accident Investigation Branch Course overview This six-week course provides a practical approach to investigating marine accidents. The course brings together the considerable expertise and experience of investigators, operators and researchers to deliver a level of training that is world-class. The core investigation skills are successfully explored and developed using a combination of lectures, relevant case studies and group exercises. In order to ensure a balanced training experience, the first three weeks of the course focus on the fundamental skills required by an accident investigator by drawing upon the experiences of air, marine and rail transport investigators. This first module includes a three-day exercise to investigate a simulated transport accident and practise the new skills. The second three weeks concentrate on applying techniques to the specialised aspects of marine operations and types of accidents. These include the technical, operational and human aspects of conducting an effective investigation. One of these weeks is based in Southampton, a renowned maritime centre, including visits to the MAIB s office. You will undertake a five-day accident investigation simulation exercise, culminating in the preparation of a full investigation report. Course objectives On successful completion of this course, you will have gained the skills needed to confidently conduct a marine accident investigation. The course is formally assessed and completes the core element of the MSc/PgDip/PCert in Safety and Accident Investigation (Marine Transport). Who should attend? The course is aimed at those who may have to conduct, or participate in, a marine accident investigation. This may be in the role of official investigator for a State or as a representative of a manufacturer, operator, service provider, insurer or regulator. There are no formal qualifications for entry. All lectures and workshops will be in English. Due to the intensity of the course a thorough knowledge of the English language is important.

3 Course content The course consists of two key modules: Module one Fundamentals of Accident Investigation This module features lectures, tutorials and practical training, including a three-day simulation of a transport accident investigation. The theory is brought to life through case studies and hands-on exercises. Topics include: Legislation and regulation. Appraisal of the accident site. Disaster response. Wreckage recovery. Evidence collection. Accident photography. Hazard management on site. Interviewing techniques. Structures and crashworthiness. Human factors for investigators. Media management. Accident pathology. Data recorders and their analysis. Analytical techniques. Systemic approach to investigation. Managing investigations. Liaising with victims and their families. Relations with the regulator/interested parties. Developing and managing. recommendations Report writing. Follow-up actions. Court procedures for investigators. Module two Applied Marine Accident Investigation Topics include: International legal basis for marine accident investigations. International Safety Management (ISM) Code and accident investigation. Managing a marine accident investigation. Marine accident site risk assessment and hazard identification. Evidence preservation and collection issues in common marine accident scenarios. Enhanced interviewing techniques Human factors and performance in marine accidents. Human factors analysis and classification tools. How to prepare an effective accident report and develop justifiable safety recommendations. Collision, contact and grounding accidents. Structural failures, loading and stability issues. Ship fires. Mechanical failures and maintenance errors. Occupational health and safety accidents. A major element is a five-day accident simulation exercise where you can apply the new skills learned. The simulation covers aspects such as team and site management; media management; site photography; interviewing; analysis techniques; report writing and recommendation development.

Mr Graham Wilson Course Director Senior Lecturer in Accident Investigation Graham has held various specialist roles in the maritime industry, including six years as a Consultant Naval Architect, before spending 11 years as an Inspector at the UK s Marine Accident Investigation Branch (MAIB), where he led various types of investigations on a wide range of vessels. In May 2013, Graham took up the new role of Maritime Risk and Investigations Director for Costa Crociere and was responsible for incident investigation for the Costa Group fleet of vessels. Graham joined the Cranfield Safety and Accident Investigation Centre in November 2015 and is able to provide a broad insight into the practical aspects of accident investigation, given his experience as a Flag State and commercial investigator.
