Payable at Nagpur) 1. Bids are invited for providing manpower from reputed Firms/ Agencies/Companies having capacity to provide manpower for housekeeping, Security and a pool car driver. Tenderers/Bidders are advised to study the tender documents carefully before submitting the Tender Form. It will be presumed that the tenderer/bidder have considered and accepted all the terms and conditions of the tender. No enquiry whatsoever verbal or written shall be entertained in respect of acceptance/rejection of the tender. BIDS MUST BE UNCONDITIONAL. 2. NO EXEMPTION OF EMD SHALL BE ALLOWED TO ANY ONE INCLUDING PSUs/COOPERATIVE SOCIETIES/ORGANIZATIONS.

2 3. The tender cost and Earnest Money fees, is to be deposted at United India Insurance Regional Office as per above time schedule. This is essential otherwise the bid will not be opened of that bidder. 4. Cutting/overwriting if any in the figures of the tendered documents is equired to be clarified/indicated in words, duly signed, failing which the tender may be rejected. 5. Deviation of any kind is not to be quoted in the bid. Such deviations shall not prevail. 6. All tender documents should essentially be signed and furnished. PROCEDURE OF SUBMISSION OF BIDS: The tenderers should submit sealed offer in a specific manners as mentioned below with superscription Tender for Providing manpower on contract basis in one main Envelope containing one Technical Bid in Envelope No. 1 & Financial Bid in Envelope No. 2. The copy of the following documents should be placed/enclosed with the Technical bids 1. Self Attested copy of PAN No. Card under Income Tax Act. 2. Self attested copy of Service Tax Registration Number. 3. Self attested copy of Valid Registration of No. the Agency/Firm 4. EMD Security of Rs. 1000/- in form of DD/Bankers Cheque 5. Prescribed Technical Bid Tender Form (Annex-C) 6. Prescribed Financial Bid Form (Annex-D) CANCILLATION OF TENDER OR CONTACT 01 The Bids will be rejected in the event of information being found false or detected incorrect or incomplete at any stage prescribed in the tender or any ineligibility being detected, and no correspondence thereof shall be entertained, whatsoever. FORFEITURE OF BID SECURITY. The Bid Security will be forfeited in the following conditions:- 01 If at any stage, any of the information/declaration given by the bidder is found false. 02 If a bidder withdraws his bid during the period of bid validity specified in the terms and conditions of tender. 03 In case of any lapse/default in honoring of the terms and conditions at any stage after submitting the tender. TERMS & CONDITIONS: Terms and Conditions of the tender are as under:- 01 The successful tenderer shall have to deposit a Security Deposit by way of a Demand Draft of 5% of the yearly contract value to the Company with agreement within 07 days of award of tender. 02 The successful tenderer shall not engage any sub-contractor or transfer the contract to any other person/firm/agency in any manner. The tenderer shall not be permitted to transfer their rights and obligations under the contract to any other person/organization or otherwise.

3 03 Tenders not conforming to the requirements of the United India Insurance Co. Ltd. will be rejected and no correspondence thereof shall be entertained, whatsoever. 04 The Tenderer shall indemnify the Untied India Insurance Co. Ltd against all other damages/charges and expenses for which Untied India Insurance Co. Ltd may be held liable or pay on account of the negligence of the Tenderer or his staff or any person under his control whether in respect of accident, injury to the person or damage to the property of any member of the public or any person or in executing the work or otherwise and against all claims and demand thereof. The Untied India Insurance Co. Ltd shall not be responsible financially or otherwise for any injury to the driver or person deployed by the Tenderer during the course of performing duties. 05 The rate contract for providing manpower shall be valid initially for one year from the date of issue of work order and Untied India Insurance Co. Ltd reserve the right to extend the validity of contract on mutual consent on the same rates and terms & conditions for a maximum of one more year upon the satisfactory functioning of the Tenderer. 06 During the period of contract, the rates will not be revised with the revision of any taxes by the Government of India. 07 The Tenderer shall not employ any person who has not completed 18 years of age. The Tenderer shall comply with all the statutory provisions as laid down under various Labour Laws/Acts/Rules like Minimum Wages, Provident Funds, ESI, Bonus, Gratuity, Contract Labour Act and other Labour Laws/Acts/Rules in force from time to time at his own cost. In case of violation of any such statutory provisions under Labour Laws or any other law applicable by the Tenderer, there will not be any liability on the Untied India Insurance Co. Ltd. 08 The prices quoted should be showing separately the monthly payable wages + applicable service charges + applicable service tax. 09. The CRM United India Insurance Co. Ltd. reserves the right to terminate the contract without assigning any reason by giving a notice of 15 days to the Tenderer at any point of time during any of the years of the contract. 10. All the disputes shall be subject to Nagpur Jurisdiction. PENALTIES: 01The agency shall provide manpower as per the requirement of the Corporation. In case of the failure of the successful tender-er to provide the same, the Company shall be free to take action as under:- (i) To hire manpower from any other agency, and if the rate charged are higher than the approved rates, the same (difference) shall be recovered from the agency, out of pending bills of the agency or from the security money deposited with the company. (ii) For breach of any of the conditions of the contract: Termination of contract and forfeiture of Security Deposit. (iii) For persistent breach or unsatisfactory services- termination of contract along with forfeiture of security deposit and blacklisting. PAYMENT 01 The payment shall be made monthly on submission of the duly verified bills (In Duplicate) after deducting penalties if any. No advance payment will be made.

4 02 United India Insurance Co. Ltd. will deduct Income Tax at source under Section 194-C of Income Tax Act from the Tenderer at the prevailing rates. AGREEMENT AND SECURITY DEPOSIT (i) Successful bidder shall furnish agreement on non-judicial stamp of denomination Rs 100/- borne by bidder, in a prescribed format within 7 days from the date of issuance of work order. Format of Agreement will be provided from United India Insurance Co. Ltd. Regional office, Nagpur. (ii) Successful bidder shall have to deposit security deposit amount of 5 % of yearly contract value (at the rate of approved monthly amount) not less than the EMD amount. Security Deposit shall be deposit in form of DD/Bankers Cheque in favour of United India Insurance Co. Ltd, Nagpur payable at Nagpur. (iii) Successful bidder will convert/merge the EMD in to Security Deposit. (iv) No interest will paid on the Security Deposit amount. The expenditure incurred to complete agreement and stamp paper will be borne by bidder and the copy of counter foil will be furnish free of cost to Corporation. EMD forfeiture :- EMD will be forfeited on following reason (i) If bidder wants to revert the proposal after opening the bids and before accepting the bid or made any modification. (ii) If successful bidder will not furnish agreement within the prescribed period. (iii) If successful bidder will not deposit the Security Deposit within the prescribed period. (iv) If successful bidder fails to commence the work/services within the stipulated period or does not comply with the stipulated terms and conditions. OTHER TERMS AND CONDITIONS: 1. Firm have to furnish compulsorily the copy of receipt of service tax deposit along with the bill after completion of every three months. In absence of receipt of service tax counter foil, Company will not release the payment. 2. The contract of successful bidder may be further extended at the same rates and terms & condition with mutual consent. In case, if required, additional manpower to be provided by the agency on the same rates/terms and conditions during the contract period. 3. Although the tender is for one year but company is free to breach the tender by giving 15 days notice in case the services are not required by the company or unsatisfactory services provided by the firm, but bidder has no right to do so. 4. Bid shall be evaluated by lowest rate but company reserves the right to execute the work at higher rates depending on the basis of quality assessment 5. The decision of Chief Regional Manager or his authorized nominee in any matter arising out of this tender shall be final & binding.

The Tender firm/agency/company should have a valid registrations such as Permanent Account Number(PAN) of the Income Tax department, Service Tax Registration Number, Registration number of the Agency/Firm. The Firm must be registered under Contract Labour (Regulation and Abolition) Act The service provider should have at-least two completed contracts with Public Sector Undertakings/ Government Undertakings. The function of the tenderer should be in conformity with all the laws as relevant/applicable. It should submit an undertaking with the technical bid to the effect that he or his firm has not been black listed by any of the Departments/Organizations of the Government of India and no criminal cases are pending against them on the date of submission of the bid (Prescribed proforma as per Annexure-D) The agency should be complying with the minimum wages act of the state for wage payments. The tenderer should submit duly signed copy of original bid terms and conditions of the tender.

6 Job description of the manpower for office boys (Full Time / Part Time): The man power so placed under the above head shall be engaged for the following purpose: 1. Daily dry and wet cleaning of the complete floors. 2. Daily dry and wet cleaning of all glass doors and vertical blinds and windows. 3. Dry and wet cleaning of all laminated work stations, partitions, doors, cabinets, racks,chairs etc. 4. Emptying and cleaning of all the tray and dustbins of all the floors. 5. Daily Dry cleaning of all the computers of the office. 6. Daily wet and dry cleaning of all wooden cum laminated tables, almirahs etc. 7. Daily wet and dry cleaning of all racks and almirahs in the stationary room. 8. Daily wet and dry cleaning of the pantry. 9. Removing of ceb webs and dust from the ceiling. 10. Dry vaccuming of the carpets in the office on day to day basis. 11. Proper cleaning of water coolers and dispensers. 12. Daily wet and dry cleaning of the floors, walls of WCS wash basis and bathroom fittings. 13. Deodorizing of all toilets cum bathrooms. 14. Web scrubbing and buffing of the entire floor area, once in every three months. 15. Dry from shampooing of all upholsters, chairs, sofas and carpets, once in a year. 16. Disinfecting and sanitizing of toilets once in every month. 17. Wet and dry cleaning of all washable walls, laminated partitions, tables, cabinets to remove stains and sticker marks. 18. Duty hours will be from Morning AM to PM on all working days. Job Description for Security man (Day time): 1. Supervising and monitoring the parking of vehicles at the parking slot of the company. 2. Keeping a vigil and watch on the parked vehicles of the company employee. 3. Keeping a watch at the entrance and prevent intruders from entering the premises. 4. Duty hours will be from Morning AM to PM on all working days.

7 Job Description for Driver of the Pool Car: 1. The Driver should keep the vehicle neat and clean daily / timely before reporting duty. 2. As and when required, drive the vehicle with caution both within the city or beyond the municipal limits. Wherever required, should be prepared for night stay outside the city. 3. The Driver shall not smoke / be drunken while on duty. 4. The Drivers shall abide by the rules laid down by the Transport Authority or any other Authority relevant to the subject and should always strictly follow the Traffic Rules and Regulations, so as to endure safety of the passengers. He shall wear neat uniform on duty. 5. The duty hours of the driver is from 9.00 AM to 7.00 PM on all working days. However there may be extended hours depending upon the exigencies. In the event of engagement on holidays, Saturdays and Sundays, he will be Rs. 800/- for 10 hours and and 500/- for 5 hours. Additional out station duty Rs. 400/- per trip. Additional Early morning and late night (10.00 PM to 6.00 PM) conveyance and refreshment allowance of Rs. 200/- will be paid. 6. Along with relevant documents like RC Book of the vehicle, Pollution Under Control Certificate, Comprehensive Insurance Policy of Vehicle, the Drivers on duty shall keep routes of Nagpur, vidarbha, Marathwada and khandesh. The Drivers provided by the Agency should be holding a valid Driving License. The drivers should also have knowledge of car mechanism so as to attend minor repairs. 7. The driver shall always remain with the vehicle or at the office during the time of duty and in case of any urgency he should seek permission the user administration department before leaving the vehicle. Driver to be necessarily provided with mobile communication facility. 8. The driver shall note the opening meter reading and time of the car on the duty slip. The driver shall inform and show the opening as well as the closing meter reading and get it countersigned by the user. Pre-Conditions for the agency: 1. The engaged employees should be physically and mentally fit with absolute integrity. 2. The engaged employees should be polite, dressed in uniforms and should wear identify card. 3. The agency to provide insurance coverage (WC policy for all the engaged employees). 4. The agency to provide the PF remittance details and salary paid details of the engaged employees every month. 5. The Driver should hold a valid driving license to drive at-least LMV and should not have disqualified. He should have a clean track record in the past.
