2 Notes on how to complete your applcaton form Complete all the questons you are asked. If any nformaton s mssng or unclear we may not be able to process your applcaton and you wll not receve any fundng. Submttng evdence When provdng evdence you can send ether the orgnal evdence tem or a certfed copy of the evdence. A certfed copy s a photocopy of an orgnal document whch must have been stamped, dated and sgned as beng a true copy of the orgnal by a person of good standng n the communty, for example, a mnster of relgon, doctor, lawyer, cvl servant, teacher/lecturer or polce offcer. The person certfyng the copy must not be a relatve and must provde ther name, job ttle, address and contact number. If you are sendng certfed copes of evdence you should ask the person certfyng your evdence to complete the certfer nformaton page at the back of the form. 1 your personal detals 1.1 Customer Reference Number Your Customer Reference Number s your personal reference number and s 11 dgts long. You wll have one of these f you have prevously had a student loan or any other student fnance from the Student Loans Company (SLC). You may also have one f you have gven fnancal nformaton for another student s applcaton. If you do not have one, or do not know what your Customer Reference Number s, leave ths blank. 1.2 Personal detals You must send: e Change of name deed, f approprate. Marrage/dvorce certfcate, f approprate. Cvl partnershp/dssoluton order, f approprate. 1.3 Natonal Insurance number If you have prevously worked n the UK you wll fnd your Natonal Insurance number on: Your Natonal Insurance number card or letter. A payslp. An ncome tax document such as a P45 or P60. A form from your local Jobcentre Plus, Jobcentre or Socal Securty Offce. If you have never been gven a Natonal Insurance number, leave the Natonal Insurance number box blank. Let us know as soon as possble f you get a Natonal Insurance number. 2 of 6 SFW/EUPTL1N/1718

3 2 prevous study and qualfcaton hstory 2.1 Honours degree from UK educatonal nsttuton If you already hold an honours degree from a UK nsttuton you wll not be elgble for part-tme fundng. 3 resdence 3.1 EU natonalty 3.2 Famly member of an EU natonal e e You must send: Passport or natonal dentty card. You must send: Your spouse or famly member s passport or natonal dentty card (to show ther EU natonalty). Your brth or marrage certfcate (to show your relatonshp to them). 3.3 Resdence hstory Academc years begn n the autumn, wnter, sprng and summer, and last for 12 months. Use ths table to work out when your academc year begns. 4 4 about your unversty or college and course 4.8 Tuton Fee amount charged If you are not sure how much you are beng charged for tuton fees, ask your unversty or college to confrm ths. 5 your loan request 5.1 Tuton Fee Loan We wll pay your Tuton Fee Loan drectly to your unversty or college n three nstalments wthn the academc year. The frst payment of the Tuton Fee Loan s made after we receve confrmaton from your unversty or college that you are n attendance on your course. Tuton Fee Loan lablty You wll be lable for your Tuton Fee Loan two weeks after the frst day of term 1, and at the start of terms 2 and 3, not the date when the nstalments are pad to your unversty or college. Interest wll be charged from the day we pay the Tuton Fee Loan to your unversty or college. Lablty % of total Tuton Fee Loan that you wll be lable for Term 1 25% Term 2 50% Term 3 100% Interest Applyng for a loan at a later date or changng the amount requested Interest s added to your loan balance from the day the frst loan payment s made to your unversty or college. You can apply for a Tuton Fee Loan at any tme n the academc year provded you are wthn the tme lmt. The tme lmt for applyng s usually nne months from the start of your academc year. 5 6 declaraton Power of Attorney Data Protecton Act If you cannot sgn the form t must be sgned on your behalf by your Power of Attorney. To grant a person Power of Attorney, you need to fll n a legal form gvng ther name, the specfc perod for whch they can act on your behalf, and the specfc tasks that they can perform. The Power of Attorney letter must be sent wth the applcaton before a sgnature from that Power of Attorney wll be accepted. The Student Loans Company (SLC) and the Welsh Government are jont Data Controllers under the Data Protecton Act We hold nformaton for the purposes specfed n our notfcaton to the Informaton Commssoner ncludng determnng elgblty for student fnance, assessng the maxmum amount of fnancal support avalable, payment of student fnance and the detecton and preventon of fraud, and may use ths nformaton for any of these. If false or naccurate nformaton s provded and fraud s dentfed, detals may be passed to fraud preventon agences to prevent fraud and money launderng. We may share the nformaton you have gven us wth other organsatons ncludng Her Majesty s Revenue & Customs (HMRC), Her Majesty s Passport Offce (HMPO), other Government bodes and your unversty or college for these purposes. We may also get nformaton about you from other organsatons. If so, t wll only be for the purposes descrbed and as the law permts us to do for the purposes of verfyng elgblty for student fnance, processng your applcaton and managng our repayment functons. 6 Next steps n your student fnance journey 1 You complete and return your applcaton form to us, along wth any evdence, as soon as possble. Make sure you read, sgn and date the declaraton. You are here 2 We process your applcaton. If any nformaton s mssng or ncorrect we wll get n touch. When we have all the nformaton we need, we wll send you a Student Fnance Enttlement letter. 3 You show your Student Fnance Enttlement letter to your unversty or college (f they ask to see t). 4 We pay your Tuton Fee Loan drectly to your unversty or college after they tell us that you are attendng your course. 6 of 6
