30 Background On August 6, 2014, the Electrical Safety Authority (ESA) launched a due diligence review of smart meter incidents reported in Saskatchewan The objective was to determine whether there were any systemic electrical safety impacts for Ontario relating to the incidents in Saskatchewan This due diligence review is consistent with ESA activity to monitor relevant developments from other jurisdictions 3
31 Current Status On August 5, 2015, ESA publically released its Meter Safety Due Diligence Review (DDR) which outlined conclusions and recommendations Recommendation #3 in the Review outlined that: the systems and processes for reporting and sharing information about electrical safety incidents involving utility assets can be made more efficient and effective through some changes to processes and requirements A similar sentiment was outlined by the Ontario Auditor General* who raised concerns in relation to the monitoring of smart metering-related fire safety risks: There have been cases of fires arising from smart meters in Ontario and in other jurisdictions. However, no accurate and complete information on smart meter-related fires was available in Ontario to determine and monitor the scope and extent of the problem across the province. 32 Current Status (cont.) As outlined in ESA s Review, we have committed to seeking an adjustment to mandatory reporting requirements for Local Distribution Companies (LDC) to include all electrical safety incidents involving meters This commitment was advised to LDC s with a request for LDC Voluntary Reporting in the Summer, 2015 Thus far, 15 LDCs have submitted voluntary reports 5
33 Path Forward Seek an adjustment to mandatory reporting requirements under section 12 of the Distribution Safety Regulation, O.Reg 22/04 6
34 Desired Outcome and Intent Outcome: Improve process efficiencies and detection capabilities that will assist in enhancing an understanding of electrical safety hazards within the electrical safety system As a result: more informed decisions to respond to serious electrical incidents involving utility assets can be achieved. creating opportunities to develop prevention strategies and responses to enhance regulatory effectiveness Intent: Adjust mandatory reporting requirements for electrical safety incidents involving utility assets under 750v in order to better detect serious electrical safety concerns for the purposes of responding, reducing and preventing risks to public safety 7
35 Benefits Responds to the Ontario Auditor General who has raised concerns in relation to the monitoring of metering-related fire safety risks (Ontario Auditor General Report Smart Metering Initiative, 2014) Opportunity for an appropriate level of information sharing in order to facilitate risk evaluation and conduct trend analysis of serious electrical safety incidents and emerging aging infrastructure considerations involving LDC assets Enhance appropriate safety-related processes in order to increase public safety, reduce electrical fire fatalities, serious injuries, and property damage Alignment opportunities - potential to create a cyclical circle of ever-improving mitigation strategies by making continuous, systematic and transparent efforts to address harms Collaboration with a range of safety system partners who help shape a culture of safety in the interest of Public Safety 8

LDC-sector Working Group will be held to obtain feedback and identify any further impact considerations of the proposed change (Feb-March) 2. A public consultation will invite all stakeholders to provide comments on the proposed change (45 days between May/June) Objectives: Solicit feedback from LDCs, industry and the regulated community on proposed changes to O.Reg 22/04 to appropriately address areas of concern and identify potential impacts (cost and safety) Inform ESA s impact assessment and develop a robust proposal to amend O.Reg 22/04 *See ESA ConsultatiZon Plan 11

41 Certificate of Deviation Bulletin Overview: ESA accepts that a certified list of deviations from Section 5 of Regulation 22/04, approved by a P.Eng is acceptable. The LDC may establish or utilize a certified list of deviations from required standards, if the LDC deems it appropriate to do so. A certified list shall state the failure to meet the standards will not materially affect the safety of any person or property. 2
42 Certificate of Deviation Bulletin Origin: Third Party Attacher approached ESA with how/if a standing list of certified deviations could be applied and satisfy the requirements of Regulation 22/04. The example appended to the bulletin is designed around a Third Party Attacher. 3
43 Certificate of Deviation Bulletin Examples: Neutral Wires that sag below the line of sight of Communication Attachments when: 4 a. Span length is less than 75 meters b. Neutral wire is part of a multi-grounded neutral system c. Neutral wire is measured in its maximum sag condition. d. Communications is measured in its Thermal sag condition e. Closest distance between the Neutral and Communications is greater than 0.3m under the above conditions.
