4 What s New for the 2016 Benefits Plan Year Great news! This year is a passive benefits enrollment for the 2016 plan year o If you don t want to make any changes to your SYKES plan elections, dependents, or beneficiaries from the 2015 plan year, then you don t need to take any action for most plans. However, you must re-enroll annually into spending accounts (Flexible Spending Account and Health Savings Account). Your elections, dependents, and beneficiaries from the previous plan year will carry over. Please note: There could be changes to the rates of which you ll need to be aware. To check premiums, please see the rate sheets, go online to the Mercer Marketplace () or contact your HR representative. o In order to enroll, make changes to plan enrollments or to check premiums, you ll need to take the steps as noted in this presentation and in the enrollment materials. There is more good news! o Only small changes to the plans and plan design from last year to this year. Page 4

6 Your Medical Plan Options Medical plan options are provided by Blue Cross Blue Shield of Florida or United Healthcare. The carrier offered to you will depend on your location. PPO - A low annual deductible but higher paycheck premium contributions Consumer-directed health plans (CDHPs) - A higher annual deductible but lower paycheck premium contributions CDHPs can be paired with a Health Savings Account (HSA) SYKES will contribute $250 for each employee who has an open HSA account at time of enrollment. Additionally, SYKES will make matching contributions of up to another $250 for Employee Only coverage and up to $750 for Employee + Spouse, Employee + Child(ren) and Family coverage. Note: Please be aware that all medical plans offered by SYKES satisfy your obligation to have medical insurance as mandated by the ACA. * Note: Carrier is based on state of residence. Page 6

8 The Benefits of Health Savings Accounts (HSAs) With the CDHP and HSA, I m in control. Having three young children means we go to the doctor often throughout the year, so I was apprehensive about moving from the PPO to a CDHP. I had heard about the high deductible and wasn t sure how my out-of-pocket costs would look for the plan year. The lower premiums and tax advantages of the HSA made me take a second look at the CDHPs. My spouse and I did the math, and realized that we could set aside enough funds to cover our deductible (and save on taxes!). I use my HSA all the time now, especially the debit card. Karen, 43, Family Coverage HSAs have many advantages Triple tax advantage Tax-deductible. Contributions to the HSA are 100% deductible (up to the legal limit of $3,350 for an individual or $6,750 for a family) Tax-free. Withdrawals to pay qualified medical expenses, including dental and vision, are never taxed Investment income* is also tax-free. HSA money is yours to keep! 12 Flexible Spending Accounts (FSAs) Health Care FSA Use this account to pay eligible out-of-pocket medical, dental, and vision expenses you incur during the 2016 calendar year You elect to contribute $5 each pay period up to $2,500 annually to your FSA (pre-tax) Use your Benefits debit card to pay your eligible expenses NOTE: You cannot have both a Healthcare FSA AND a Health Savings Account Dependent Care FSA Use this account to pay approved expenses you incur during the 2016 calendar year that relate to caring for a dependent (child or spouse) while you work Not to be used for your dependent s medical expenses You may elect to contribute $5 each pay period up to $5,000 annually to your FSA (pre-tax) Use your Benefits Debit Card to pay your eligible expenses Transportation FSA Reduce your commuting expenses by setting aside pre-tax money for qualified commuter and parking expenses that you incur while getting to work (train, bus, subway, ferry, etc.) Can save you 40% or more! The pre-tax maximum contribution for transit is $130 per month and $250 per month for parking. Helpful FSA Tip: Use it or Lose it! If you don t use all the funds in your account during a plan year, then you ll lose those funds, so you ll need to plan wisely. Page 12

14 Voluntary Short-Term & Long-Term Disability Coverage by The Hartford ( Short-Term Disability (STD) Company-paid voluntary coverage that pays you a fixed dollar benefit while away from work for a short-term disability Coverage of 60% of your weekly earnings up to $1,500 per week Long-Term Disability (LTD) Company-paid voluntary coverage that pays you a fixed dollar benefit while away from work for a long-term disability Coverage of 40% of salary up to $5,000 per month o Additional 20% buy-up of your base earnings for a total of 60% of salary up to a maximum of $7,500 per month Evidence of Insurability (EOI) Evidence of Insurability may be required if you purchase additional long-term disability insurance. If you have forgotten your username, please contact Mercer Marketplace at Password: Your password will be reset to your last name and the last four digits of your Social Security number (Example: Jones1234). First-Time Enrollees to Mercer Marketplace: Username and Password: You ll need your Social Security number, last name, and date of birth to identify yourself and then you will be prompted to select a unique username and password that you will use going forward. 21 Smart Ways to Save Money on Healthcare Expenses Here are two easy ways you can save money on healthcare expenses: 1. In-network vs. out-of-network: Before you visit a doctor, dentist, provider, or facility, make sure that the person or facility is in-network, meaning he or she (or it) is part of your health plan. If you see an out-ofnetwork provider, your costs will be higher. You can check on this by calling the provider s office, or by looking online. Usually, it s safest to call. 2. Generic medications: Make sure that when you visit the pharmacy or call mail-order for your (or your family s prescriptions), you ask for the generic equivalent of the medication.
