11 SFO Medical Clinic RFP General: RFP to select medical organization to operate the Clinic RFP documents: available at the websites of the SFO s Current Business Opportunities City and County of San Francisco s Office of Contract Administration: Selected operator will enter into Professional Services Agreement with SFO (see RFP documents, Appendix D) Term of Agreement: 3 years with two 1-year options 11

12 Scope of Work (RFP Appendix F): Serve as provider/director of clinical programs Contract with or hire Clinic staff Develop programs, administrative infrastructure and personnel to operate the Clinic Operate/manage Medical Information Systems Maintain patients medical records Develop/implement quality assurance programs related to healthcare services Manage billing/collection Refer patients/clients to appropriate service providers for after hour care Assist SFO on wellness education and health promotion activities Maintain hours/days of operation as specified in the contract 12

13 Minimum Qualifications: 7 yrs. experience in operation of medical clinics (industrial, travel, and/or Airport setting). Experience in hiring & supervision of staff for urgent care, occupational medicine, travel medicine, and pre-placement medical examinations. Licensed by the State of California to operate medical clinics. If minimum requirements are not met: Proposer will not be eligible for contract award. 13

15 Cost Proposal: Provide cost proposal using the format presented in Appendix H. Comply with Generally Accepted Accounting Principles. Include: Projected annual revenues, expenses, operating profit/loss Projected payments by SFO (including the proposed annual management fees) Cover the period between July 1, 2018 and June 30, Submit in a sealed envelope labeled as Cost Proposal, Contract with the name of the proposer. Include Appendix I, Labor Detail Form, with information about staffing and projected labor costs. 15

18 Submission Requirements: Deadline: October 20, 2017; 3:00pm (late submissions will not be considered). Postmarks will not be considered in judging timeliness of submissions. 19 Selection Criteria Proposals meeting minimum qualification requirements will be evaluated & scored by a selection committee. 20 Contract Award: SFO intends to award contract to the organization that will provide the best overall services. SFO reserves the right to accept other than the lowest priced offer. SFO will commence negotiations with the highest ranked proposer. Selection of any proposal does not imply acceptance of all terms of the proposal; terms may be subject to further negotiations. If a satisfactory contract cannot be negotiated in a reasonable time, SFO may terminate negotiations with the highest ranked proposer and begin negotiations with the next highest ranked proposer. 20

21 Professional Services Agreement: The successful proposer will enter into a contract substantially in the form of the Agreement for Professional Services (Appendix D). Failure to timely execute the contract, or to furnish insurance certificates or other required documents, will be deemed an abandonment of the contract offer. 21
