2 Non-Discovery Health members can influence their future premiums by actively maintaining and improving their health and lifestyle through Vitality. How the Vitality Integrator works 1. Starting premiums are lower The reduction is as follows: Personal Integrator Business Integrator Standard / AcceleRater FlexRater Standard / AcceleRater FlexRater 17.5% 15% 8.75% 7.5% 2. Future premium increases are determined by Vitality status Vitality status Blue Bronze Silver Gold Diamond 2.25% 1.50% 0.50% -0.50% -0.75% Applicable to both Personal and Business Integrators 3. Certainty is enhanced by additional guarantees The premium will never be more than the premium a client would have paid on a similar non-vitality Integrated plan for at least the first five years. After that, the premiums will never be more than the Maximum Protected Premium set out below: Personal Integrator LIFE PLANS Standard / AcceleRater FlexRater 6-10 years NI + 10% NI + 10% years NI + 15% NI + 15% 16+ years NI % NI + 15% Business Integrator LIFE PLANS Standard / AcceleRater FlexRater 6+ years NI % NI + 7.5% (NI is the premium for an equivalent non-integrated plan) The additional annual premium increase will never be more than 2.25%. If a client reaches Gold or Diamond status on Vitality, they can reverse previous premium increases if they continue to manage their health effectively and maintain their Vitality status Certainty is further enhanced by the fact that Integrator adjustments will be capped at 0% on a policy which has a qualifying disability or severe illness claim. The PayBack Benefit The PayBack Benefit refunds a percentage of your client's qualifying LIFE PLAN premiums every five years. The PayBack percentage will depend entirely on Vitality status as follows: Vitality status Blue Bronze Silver Gold Diamond PayBack percentage 5% 7.5% 15% 20% 25% Only applies to the Personal Integrator The Discovery PayBack is not available on the Essential LIFE PLAN. Clients must keep their Vitality Integrated policy in force for the entire five-year period to receive a PayBack at the end of the five-year period. Any reduction in premium may cause a reduction in PayBack Fund. Annual Guaranteed Payback Clients can receive an Annual Guaranteed PayBack of up to 18.75% for the first 10 years, with any additional PayBack in excess of the guarantee being accrued to the Vitality Surplus PayBack, which will be paid out at the end of year five and year 10. This allows clients to still receive up to 25% of their premiums back every five years. 58

3 Determining the level of the guarantee Clients on the Vitality Integrator will receive a Annual Guaranteed PayBack of 7.5%. Additional guarantees for DiscoveryCard holders Clients who have a DiscoveryCard will receive a further 7.5% PayBack guarantee, provided they have an average DiscoveryCard spend of more than R per month. Additional guarantees for Vitalitydrive clients: In addition to the Guaranteed PayBack for Health and Vitality Integrated clients, we can provide further guarantees for Vitalitydrive clients who are on an Engaged or Advanced Vitalitydrive status: Vitalitydrive status Vitality Integrator Engaged 2.5% Advanced 3.75% Cumulating Payback The Cumulating PayBack option benefits clients who choose to receive their PayBacks at year five and 10 by increasing the guarantee applicable to their PayBack benefit. The premium guarantee for Vitality Integrated clients is increased to 12.5% under the Cumulating PayBack option. The additional guarantees for DiscoveryCard and Vitalitydrive clients are the same as for the Annual Guaranteed PayBack, meaning that the Cumulating PayBack provides a guaranteed PayBack accrual of up to 23.75% each year, payable at year 5 and year 10. Technical details The Annual Guaranteed and Cumulating PayBack Options are available to Personal Integrated Classic LIFE PLANs on the latest version of the Financial Integrator Vitalitydrive PayBack guarantee is limited to R pa Vitalitydrive status and DiscoveryCard spend are calculated 90 days prior to policy anniversary 59

4 VITALITY Memory Vitality Memory provides clients with future PayBack guarantees based on their past engagement in Vitality. This benefit is available to Personal Health or Vitality Integrated policies which have the latest version of the Financial Integrator. This benefit rewards clients who have consistently engaged with Vitality over time and it represents a significant enhancement to the long-term sustainability of the Integrated operating model. Vitality Memory uses a point system to reward clients for long-term Vitality engagement. For every year a client is on Vitality, they receive the following Vitality Memory points: Engagement level Points per year Low (Blue) 0 Medium (Bronze, Silver) 5 High (Gold, Diamond) 10 For every 10 years of Vitality engagement a client will receive a guaranteed PayBack at year 15 based on the following table: Guaranteed PayBack percent Vitality Memory score Health Integrator Vitality Integrator Core Saver Comprehensive % 0% 0% 0% % 7.5% 10% 10% % 10% 12.5% 15% % 15% 17.5% 20% % 20% 22.5% 25% A new Vitality Memory is calculated for each PayBack cycle based on the points earned in the 10 years prior to that PayBack cycle.
