2 Medical Residents Benefit Summary All Residents must initiate enrollment at the Benefits website ( within 30 days of employment and/or benefits eligibility. Full-time (regularly scheduled 72 hours or more per pay period) or Part-time (regularly scheduled 32 to 71 hours per pay period) are considered eligible for benefits noted below. Residents who fail to elect or decline benefits on-line within 30 days of the employment or benefits election period, will automatically be defaulted into the Trinity Health PPO associate only coverage. Please make sure you elect your coverage of choice or choose no coverage in order to avoid this situation. BENEFIT WHO PAYS WHEN ELIGIBLE COVERAGE DESCRIPTION Medical Benefits Cost shared with & 1 st day of Hire Residents may select a medical plan from the options offered: Healthy Blue Solutions Plan B PPO (BCBS)** Priority Health-HealthbyChoice HMO Dental Benefits Cost shared with & 30 days of date of hire Residents may choose between two dental plans administered by Delta Dental of Michigan: Delta Premier (a Traditional Dental Plan) Delta Preferred (a PPO Plan)** Vision Benefits Pre- Tax salary deduction 30 days of date of hire Residents may elect to enroll in the United Health Care Vision Plan. Coverage can be elected for spouse and children. At termination, continuation of medical, dental and vision benefits per Federal guidelines. Paid Time Off (PTO) PTO is provided immediately upon hire. Paid Time Off (PTO) is an annual bank of time provided Residents annually based on their contract terms. Residents will be provided with 28 days of PTO. PTO will be forfeited if not used within the terms of the contract or at termination of Residents are not eligible for PTO cash out. Individual sick days will be compensated at 100% until eligible for Salary Continuation Income Program (if eligible). If associate is ineligible 08/10/2011 *SUBJECT to CHANGE 2 of 5 SLS
3 for Salary Continuation, the associate shall use all available PTO hours. After fourteen (14) calendar days of total disability, this benefit provides 75% of base weekly earnings for up to twenty-four (24) weeks while off for a non-work related accident/illness. Available PTO hours may be used to supplement the difference in compensation to equal 100% of base pay. Salary Continuation Income Protection Upon completion of six months of After twenty-six (26) weeks of total disability, this benefit will provide compensation of 60% of base pay up to a maximum monthly benefit $10,000. Residents may elect an optional 50% or 66 2/3% pay benefit. Long Term Disability (LTD) Residents are covered by Group Term Life/AD&D insurance in an amount equal to one times base annual salary. Guarantee issue limit $1million. Group Term Life/AD&D Residents may elect additional Term Life Insurance in an amount Supplemental and Dependent Term Life Insurance Post-Tax Up to 5 times their base annual salary. 4 Health Care Reimbursement Account Dependent Care Reimbursement Account Pre- Tax Salary Pre- Tax Salary The Trinity Health Pension Plan is a defined benefit pension plan. Residents are fully vested after 5 years of participation in the pension (1 year of vesting service equals 1,000 working hours in a calendar year) After vesting, the is eligible to receive a monthly benefit at retirement based on a formula that includes age, years of service and average compensation. The 403(b) Retirement Savings Plan, administered by Diversified Investment Advisors, is a defined contribution plan that grows with contributions from both you and Trinity Health. Retirement website: Children Learning Center, if space is available Child Care Services and Learning Center located on the campus of. Legal Plan The Hyatt Legal Plan provides services such as wills, real estate closing and debt collection defense. The plan can cover you, your spouse and your dependents. This program is voluntary and there is a bi-weekly premium contribution required of full and part-time Residents based on the selected coverage. Educational Allowance 1st day of hire Refer to contract Exhibit 1 for details Bereavement Pay Provides 1.0 FTE and.9 Residents 24 hours pay and.4-.8 FTE 12 hours pay when absent from normally scheduled shift to attend the funeral and/or make arrangements for immediate family member s death. These include current: spouse, child, stepchild, brother, stepbrother, sister, stepsister, parent, stepparent, grandparent, grandchild, mother/father-in-law, brother/sister-in-law, legal dependent or legal ward. In order to receive bereavement pay, the associate must submit a Bereavement Pay Request form to their manager/director. *Jury Duty Residents are compensated for scheduled hours including shift premiums. 08/10/2011 *SUBJECT to CHANGE 4 of 5 SLS
