3 UFLAC Benefits Here for You and Your Family Dear Brothers and Sisters: Look what s new for the 2017 Open Enrollment! UFLAC is excited to announce changes to our current HMO plan and introduce a new HMO option for In order to keep our member s costs down, UFLAC has transitioned our current HMO plan to the Anthem Blue Cross Select HMO network! The Select HMO network includes over 27,000 California Select HMO doctors and specialists and more than 390 hospitals. For our members whose Primary Care Physician (PCP) is outside of the Select HMO network, UFLAC is excited to introduce our new Vivity HMO plan as well. The Vivity HMO plan is smaller than a traditional HMO network, but includes the top 7 hospital systems in Southern California. These premier hospital systems include: Cedars-Sinai, Good Samaritan Hospital, and UCLA Health and provide access to some of the best doctors in all of Los Angeles. The Vivity HMO plan is offered through Anthem Blue Cross and serves as a cutting-edge solution for UFLAC members to access high-quality medical care at lower costs. To check if your PCP is a member of the Select or Vivity networks please visit UFLAC is pleased to continue to offer our members the HSA PPO plan in which members can contribute TAX-FREE money into their individual Health Savings Accounts (HSAs). The UFLAC HSA PPO plan is only available to our members and is the only HSA plan available to Los Angeles firefighters and Los Angeles City employees. For 2017, the City of Los Angeles will continue to contribute $100 per month towards each enrolled members HSA. Effective July 1st, 2016 UFLAC has enhanced its Long Term Disability (LTD) plan as well. The Monthly Benefit is now based off your prior year s W-2 earnings and includes overtime! So even if you are injured on-duty and receiving state-rate, you may still be eligible for an additional benefit from Reliance Standard. These enhancements far exceed the old CAPF LTD policy. You can sign-up for the UFLAC LTD plan anytime without a medical exam! UFLAC will hold a fall open enrollment each year and during this time you can make changes to your Medical and Dental plans. UFLAC will also hold a mid-year open enrollment in April and its during this time you can make changes to your Medical and Life insurance plans. 42 LTD Long Term Disability Benefits Are Fully Insured Long Term Disability (LTD) insurance protects your income if you are disabled and unable to work. LTD coverage offered through UFLAC is provided through Reliance Standard with enhancements effective July 1, LONG TERM DISABILITY NEW in 2016! Monthly benefit NOW includes overtime! Rate is $12.50 per pay period Maximum benefit of $10,000 per month Includes survivor benefit 24 month own occupation coverage 90 day waiting period Coverage is fully insured 41
43 We designed this plan to protect our members, which is why we decided to partner with an insurance company with an A+ financial rating. Being insured by a company in good financial standing means that there are reserves available to pay member claims, even in the case of a large-scale disaster. UFLAC has designed a plan with LA City firefighters in mind, taking into account income offsets such as sick and vacation time, and other disability plans. Our plan coordinates with common sources of disability income, such as: Other group plans, Workers Compensation, Sick pay, Social Security and Disability pensions. This means that your benefit payment from Reliance Standard may be reduced if you are receiving any of these types of income. Back to Index 42
44 CRITICAL ILLNESS Critical Illness Plan This benefit protects your family against unexpected costs due to illness, when you need it the most. This pays above and beyond your medical coverage and can be used at your own discretion. 46 ACCIDENT Accident Insurance Plan This benefit provides lump sum payments for accidents. This pays above and beyond your medical coverage and can be used at your own discretion. This plan may help offset your deductible on the HSA PPO medical plan! 48 NOTICES Notices Important information for you and your families HIPAA - Special Enrollment Notice If you are declining enrollment for yourself or your dependents (including your spouse) because of other health insurance or group health plan coverage, you may be able to enroll yourself and your dependents in this plan if you or your dependents lose eligibility for that other coverage (or if the employer stops contributing toward your or your dependents other coverage). However, you must request enrollment within 30 days after your or your dependents other coverage ends (or after the employer stops contributing toward the other coverage). In addition, if you have a new dependent as a result of marriage, birth, adoption, or placement for adoption, you may be able to enroll yourself and your dependents. However, you must request enrollment within 30 days after the marriage, birth, adoption, or placement for adoption. Finally, you and/or your dependents may have special enrollment rights if coverage is lost under Medicaid or a State health insurance ( SCHIP ) program, or when you and/or your dependents gain eligibility for state premium assistance. You have 60 days from the occurrence of one of these events to notify the company and enroll in the plan. Newborn and Mothers Health Protection Act Group health plans and health insurance issuers generally may not, under Federal law, restrict benefits for any hospital length of stay in connection with childbirth for the mother or newborn child to less than 48 hours following a vaginal delivery, or less than 96 hours following a cesarean section. However, Federal law generally does not prohibit the mother s or newborn s attending provider, after consulting with the mother, from discharging the mother or her newborn earlier than 48 hours (or 96 hours as applicable). In any case, plans and issuers may not, under Federal law, require that a provider obtain authorization from the plan or the insurance issuer for prescribing a length of stay not in excess of 48 hours (or 96 hours). 47
49 Women s Health and Cancer Rights Act If you have had or are going to have a mastectomy, you may be entitled to certain benefits under the Women s Health and Cancer Rights Act of 1998 (WHCRA). For individuals receiving mastectomy-related benefits, coverage will be provided in a manner determined in consultation with the attending physician and the patient, for: All stages of reconstruction of the breast on which the mastectomy was performed; Surgery and reconstruction of the other breast to produce a symmetrical appearance; Prostheses; and Treatment of physical complications of the mastectomy, including lymphedema. These benefits will be provided subject to the same deductibles and coinsurance applicable to other medical and surgical benefits provided under each plan. If you would like more information on WHCRA benefits, call your plan administrator at , extension 2. Patient Protection Disclosure Anthem Blue Cross generally requires the designation of a primary care provider. You have the right to designate any primary care provider who participates in our network and who is available to accept you or your family members. Until you make this designation, Anthem designates one for you. For information on how to select a primary care provider, and for a list of the participating primary care providers, contact the UFLAC Benefits Office at extension 2. For children, you may designate a pediatrician as the primary care provider. You do not need prior authorization from Anthem Blue Cross or from any other person (including a primary care provider) in order to obtain access to obstetrical or gynecological care from a health care professional in our network who specializes in obstetrics or gynecology. The health care professional, however, may be required to comply with certain procedures, including obtaining prior authorization for certain services, following a pre-approved treatment plan, or procedures for making referrals.
