3 HealthFlex Annual Election Guide Annual Election is your once-a-year* opportunity to: Select your 2017 HealthFlex plan for medical/behavioral health/pharmacy benefits. If you don t select a plan, your coverage will default to the plan you have currently (if still offered) or to your plan sponsor s default plan choice. Set aside pre-tax money into flexible spending accounts (FSAs) to save for dependent care and/or health care expenses in (See page 3 and information online for details.) If you don t make this election during Annual Election, current FSA elections will not continue in Change which dependents you cover. Contact your benefits office for a HealthFlex Enrollment/Change Form if you want to add/remove a spouse or child from your coverage. [Note: Fill out your forms early. Completed forms must be received by Wespath Benefits and Investments (Wespath) before November 17.] * After Annual Election, you can make certain plan changes or change FSA elections only if you experience a qualifying change of status event, such as marriage, divorce, death of a spouse, birth/adoption of a child, or loss of spouse s health coverage. wespath.org log in to HealthFlex/WebMD 1

4 HealthFlex annual election 2017 New for 2017 Starts January 1 Obtain maintenance medications through Walgreens or Home Delivery. For your convenience, you may fill 90-day maintenance prescriptions at a local Walgreens pharmacy or through the OptumRx Home Delivery (mailorder) service. Behavioral health benefits change. Behavioral health benefits will have the same in-network and out-of-network co-payments and/or deductible and co-insurance as your medical plan benefits. However, behavioral health outpatient office visits with an out-of-network provider will be covered at in-network levels up to a reasonable and customary amount. (See benefits booklets for details.) OptumRx formulary changes. Certain medications with no clear clinical advantage will no longer be covered if a comparable alternative is available, as part of the HealthFlex and OptumRx commitment to keeping costs lower. OptumRx will contact impacted participants directly about medications no longer covered. New MDLIVE* Telemedicine Services Talk with a consulting physician 24/7, either through videoconferencing or by phone, when you can t get to your own provider but need a professional medical opinion and treatment or prescription quickly. Great for non-emergent acute conditions like cold/flu, skin concerns, sore throats and more. * MDLive telemedicine and nurseline services do not replace the expertise of your personal physician or other primary care provider (PCP). Consult your PCP for more comprehensive diagnostics and guidance. Nurseline* through your medical carrier. When you aren t sure if you need to contact a physician, you can call the nurseline number on your medical ID card 24/7 with questions about symptoms, an injury or general health questions. Virgin Pulse rewards changes. Watch for upcoming communications on Virgin Pulse s streamlined approach to earning incentives. 2

5 HealthFlex annual election 2017 Save Money with Pre-Tax Reimbursement Accounts Annual Election is your only opportunity* to select optional pre-tax savings for 2017 through flexible spending accounts (FSAs). Flexible spending accounts (FSAs) Medical reimbursement account (MRA, also called health care FSA) Participants can set aside pre-tax savings for eligible health care expenses. Unused balance at yearend: up to $500 carryover. Dependent care account (DCA, also called dependent care FSA) Participants can set aside pre-tax savings for eligible dependent children up to age 12, and for adults incapable of self-care. Unused balance at year-end: no carryover. Health reimbursement account (HRA) Provided with all consumer-driven health plans (CDHPs C2000 and C3000). Unused balance at year-end: no limit on accumulated carryover while employed in The United Methodist Church. Participants cannot add personal funding to an HRA; this is funded by the plan/plan sponsor only for eligible health care expenses. If you want to make FSA contributions in 2017, make that election during this Annual Election period.
