1 HealthyBlue Simple Steps to a Healthy Reward
2 Taking control of your health has its rewards. With HealthyBlue, all you need to do is complete 3 steps within 180 days of your effective date to earn a Healthy Reward and reduce your deductible. Saving money has never been this easy! My HealthyBlue Rewards My Account makes it easy to earn and track your Healthy Reward. > Sign-in to My Account at > Click on the Manage My Health tab > Click on My HealthyBlue Rewards Q & A My child turns 18 during the plan year. Can he earn the adult reward? No. The reward is based on your child s age at the time of the plan s effective date.
3 Step 1: Select a PCP 3 Steps to Earn a Healthy Reward If you haven t already, select a Primary Care Provider (PCP) in My Account. > From the Qualification tab, click Select PCP Schedule an appointment with your PCP for the Health and Wellness Evaluation. Step 2: Complete the Online Health Assessment (Adults 18+ only) > From the Qualification tab, click Begin Assessment Step 3: Complete the Health and Wellness Evaluation Form online > From the Qualification tab, click Begin Evaluation Steps 2 and 3 can be completed in any order. Receiving the Healthy Reward is not dependent on your specific results or scores.
4 The Online Health Assessment includes a variety of health and lifestyle questions and only takes a few minutes to complete. Online Health Assessment Be sure to click Yes on the consent portion so the results can be shared with your PCP. Q & A I recently completed the screenings, immunizations and health measurements required on the Health and Wellness Evaluation Form. Do I still need to make an appointment with my PCP? You may not need an appointment for a Health and Wellness Evaluation, but you will need to have your PCP sign the completed form.
5 Take the Health and Wellness Evaluation Form, which can be found in your enrollment book or online at to your scheduled appointment. Health and Wellness Evaluation Form Then, input your results through My Account. Submitting your form online allows CareFirst BlueChoice, Inc. to process the form immediately, which means you receive your Healthy Reward faster! Q & A When will I receive my Healthy Reward? Once you complete the 3 steps, you will receive your deductible reduction within 2 business days. If you already met your deductible, you will receive a check for the difference within 10 business days.
6 Once CareFirst BlueChoice, Inc. receives your Health and Wellness Evaluation Form, and your online Health Assessment is complete, we ll reduce your annual deductible. It s that simple. If you have already met your deductible, your Healthy Reward will be in the form of a check sent directly to you, the subscriber. Individual Plan Members with an individual plan can earn up to $300 1 for completing the 3 steps. How the Healthy Reward works with your deductible Example: Individual Plan In-Network Deductible Out-of-Network Deductible Deductible $500 $1,000 Healthy Reward earned ( $300) ( $300) Remaining deductible $200 $700 Family Plan Adults can earn up to $300 for completing the 3 steps, while children 2-17 can earn $25, for a maximum reward of up to $700 per family. 1 Children under age 2 are not eligible to participate in the Healthy Reward program. Children under age 18 do not have to complete the online Health Assessment to receive the reward. 1 MSGR Option B HSA/HRA plans and MD, DC and VA Option D HSA/HRA plans have a maximum individual reward of $250 and a maximum family reward of $500.
7 Want to check your progress? Check the status of your Healthy Reward Once you have completed the 3 steps, you can check the status of your deductible in My Account to see if your Healthy Reward has been applied. > Click on the Manage My Money tab > Click on My Deductible Status Q & A How often can I receive a Healthy Reward? The reward qualification process is annual, so you can requalify each year you re enrolled in HealthyBlue. Can I complete the incentive process for my dependents? Yes, you can submit the required information for the Health and Wellness Evaluation Form in My Account for your dependents under age 18. However, members 18+ need to set-up their own account in the My Account tool and complete the process themselves.
