5 Dear New York and Presbyterian Hospital Queens PPS, This notice is intended to inform you of your upcoming payment following the conclusion of DSRIP Year 2 (DY2). This payment will include three components; DY2, Payment 2, High Performance Fund Payment (if applicable), and DY2 Payment 1 Reconciliation. The table below provides the breakdown of this payment across each of the three components. Additional details about each component are provided following the table. Payment Component Payment Amount 1) DY2, Payment 2 $1,111,838 2) High Performance Fund Payment $2,16 3) DY2, Payment 1 Reconciliation $165,748 Total Payment $1,517,62 1) DY2, Payment 2 The DY2, Payment 2 amount was determined based on the results of the DY2, Q3 and DY2, Q4 PPS Quarterly Reports. This payment includes funds earned for Domain 1, Domain 2 P4R, Domain 3 P4R and P4P, and Domain 4 P4R. The amount of funding available for this payment and the distribution across the various components was calculated in accordance with the revised Attachment I of the STCs as agreed upon by DOH and CMS in July 217. Details regarding the specific percentage allocations across domains and between P4R and P4P can be found in the revised Attachment I on the DSRIP website. 2) High Performance Fund Payment PPS were eligible to earn High Performance Fund payments as a result of their performance on a set of measures identified a High Performance Fund eligible measures during Measurement Year 2. PPS could earn High Performance Fund payments by closing the gap to goal on a High Performance Fund measures by 2% or greater in Measurement Year 2 or by reaching the Statewide Performance Goal for a High Performance Fund measure. 3) DY2, Payment 1 Reconciliation When the DY2, Payment 1 amounts were initially calculated, DOH was engaged in discussions with CMS on internal inconsistencies within Attachment I of the STCs that impacted the calculation of DSRIP performance payments. As a result, the DY2, Payment 1 amounts were calculated in accordance with the original language from Attachment I, resulting in a lower payment amount to PPS. In order to ensure that PPS receive the full payment amount earned based on DY2, Q1 and DY2, Q2 performance, a reconciliation has been completed to determine the additional funding earned by PPS based on the final Attachment I. Total Payment Earned DY2, Payment 1 Additional Payment (based on final Attachment I) (made in April 217) (to be made in August 217) $975,866 $81,118 $165,748 Additional details about the revisions to the payment calculation based on the final Attachment I, approved in July 217, will be provided in a webinar on August 1, 217.
