1 Commonwealth of Massachusetts RFR ITT46 Network Services Version 1.2 4/18/2012 Attachment C: Cost Tables Contract User Instructions 1 Cost Tables must contain all goods and services for which compensation will be sought on this Statewide Contract. 2 Compensation will be based solely on the goods and services and their associated pricing identified in the Cost Tables supplied by the Bidder and accepted by the SST and published on Comm-PASS. 3 Taxes, surcharges, fees and other "regulatory cost items" may not be invoiced unless they are specified (description and pricing) in the Cost Tables accepted by the SST and published on Comm-PASS. 4 Non-standard site-specific extension and engineering costs, if any, must be quoted in writing upon completion of a site survey prior to the issuance of a purchase order. 5 Categories are indicated in the Cost Table tab numbering, i.e., Category 1 services are numbered 1.x, Category 2 services are numbered 2.x, etc. 6 Each proposed service type should be specified on a separate tab. For example: Cost Table Local Exchange Service, Cost Table Long Distance. 7 Eligible Entities are not required to sign vendor documents unless the document is approved by OSD and published on Comm-PASS.
