Key points We review activity trends for NHS elective procedures undertaken by private hospital operators, including trends for RHC. The share of NHS elective volumes for private hospitals has fallen in line with funding constraints, with waiting lists continuing to rise. RHC recorded improved volume growth in recent months as well as increased share of the private market following weakness over FY18. Event We provide an overview of activity trends for NHS elective procedures undertaken in the private sector. Elective surgical activity accounts for the majority of RHC s UK volumes, with ~80% of admissions funded via the NHS. Key points Private share of elective NHS volumes declining: The proportion of NHS elective volumes serviced by the private sector fell to 5.7% in (down from a peak of 6.5% in Mar-17), with 12-month rolling volume growth of -2.8%. We see this as reflective of demand management strategies for elective care in more recent periods, in line with NHS funding constraints (as per Spire 1H18 commentary). Pressure on the NHS system continues to build: The number of patients on waiting lists and the average time to treatment has increased in line with these changes. For, the number of referred patients waiting for treatment increased to 4.2mn, up from 3.9mn in. At the same time, median waiting times have continued to rise, up 7% YoY to 7.6 weeks in. RHC showing signs of improvement: RHC recorded volume growth below the sector average in the 12 months from, impacted by a hospital closure. However, improved trends have become evident more recently, with growth ahead of the sector average and recovery in market share. Earnings and target price revision No change. However, a continuation of recent improved activity trends and confirmation of additional NHS funding would be incrementally positive for the stock. More broadly, our investment view for RHC is underpinned by an expectation of contributions from brownfield developments and incremental operational efficiencies, which we see as supportive of growth in FY19 and into FY20. In addition, the completion of the acquisition of Capio would provide additional earnings growth over and above our current FY19/20 forecasts. RHC currently trades on 18.6x 12-month forward consensus EPS, below the two-year average of 22.7x. We retain an Outperform rating. 2 Share of elective volumes (%) Growth (%, 12-month rolling) RHC UK on the mend? RHC s UK business accounted for ~7% of group EBITDA in FY18 (on a proportionate basis). Compositionally, the majority of RHC s UK activity is elective, day surgical procedures (low acuity), with ~80% of admissions funded via the UK National Health Service/NHS (with the balance from private medical insurance/pmi and self-pay). In this note, we provide an overview of activity trends for NHS elective procedures in the private hospital segment as well as recent performance for RHC. While private sector volumes accounted for a rising proportion of elective NHS volumes from 2014 into 2017, funding constraints and subsequent demand management strategies for elective care in more recent periods (including restriction of eligibility for certain treatments by Clinical Commissioning Groups/CCGs and the relaxation of waiting list performance targets) resulted in declining elective volumes for private hospital providers. In August 2018, the proportion of NHS elective volumes serviced by the private sector had fallen to 5.7%, down from a peak of 6.5% in March On a 12-month rolling basis, volume growth had fallen to -2.8%. In addition, the proportion of patients referred for non-emergency procedures treated within 18 weeks has fallen to 87%, below the 92% target as specified in the NHS constitution standard. In addition, the number of patients waiting more than 18 weeks has been risen, with ~530,000 recorded in (up 29 YoY), while the number of patients waiting more than 52 weeks for treatment was 3,407 (up 96% YoY). The number of patients on the waiting list in Mar-18 was 3.8mn (compared to 4.2mn in, as provided above). The number of patients waiting more than 52 weeks should be halved by 2019 (2,755 in March 2018 vs 3,407 in ). We note this slowdown coincided with the broader trends among private hospitals, but with additional impacts from a hospital closure (Bodmin Treatment Centre) as well volatility in volumes in line with the opening of the Tees Valley Hospital (a new facility replacing the Tees Valley Treatment Centre). More recently, volumes have shown signs of improvement. 5 Appendix 1 The UK Hospital Market In the UK, private hospitals are funded via three systems: Private Medical Insurance (PMI either employer sponsored or purchased directly), the NHS and self-pay. PMI has historically accounted for the largest proportion of private hospital revenues (over 50%), followed by the NHS (~25%) and the balance via self-pay 1. The patient journey typically begins with a GP visit. The GP may refer the patient directly or provide a referral to a consultant. Self-pay and patients with PMI will be treated in a private hospital. An NHS patient can choose a either a public/nhs or private hospital via the NHS e-referral online portal. Private hospitals can also receive additional volumes from NHS outsourcing/local contracting in order to reduce waiting times.
