1 TRAVEL POLICY PROMOTION TERMS AND CONDITIONS 1. This travel policy offer (the Travel Policy Offer ) is being run by Irish Life Health dac ( Irish Life Health ) in conjunction with Mondial Assistance Ireland Limited trading as Allianz Global Assistance and AWP P&C SA. 2. The Travel Policy Offer applies to persons who are (i) aged 18 years or over (ii) are residents in the Republic of Ireland (iii) purchase or renew one of the types of health insurance listed below (the Health Policy ) with Irish Life Health between 1 January and 28 February 2017 (inclusive), and (iv) register with Irish Life Health dac in the prescribed form, details of which are set out below, within 30 days of the start date of your Health Policy or by 31 March 2017, whichever is the earlier (the Closing Date ) (the Eligible Participants ). 3. The eligible health insurance plans are outlined in point 15 below. 4. Customers who purchase or renew health insurance policies with Irish Life Health other than those policies listed above are not eligible to participate in the Travel Policy Offer. 5. The Travel Policy Offer allows the Eligible Participants to avail of a free travel insurance policy. The Travel Policy Offer is for a Worldwide Annual multi-trip travel policy for an individual, a couple or a family (the Travel Policy ). Family means 2 adults and up to 4 children under the age of 23 on the day the policy is taken out and all family members must be living at the same address. All children must in full time education and children must be accompanied by an adult that also has travel insurance when travelling. Eligible Participants can only avail of a family policy if all family members are covered under, or have, a Health Policy included in the Travel Policy Offer. 6. The Travel Policy is underwritten by AWP P&C SA and is administered by Mondial Assistance Ireland Limited trading as Allianz Global Assistance. AWP P&C SA is authorised by L Autorité de Contrôle Prudentiel et de Résolution in France and is regulated by the Central Bank of Ireland for conduct of business rules. 7. The terms and conditions of the Travel Policy will be issued to you with your Travel Policy directly from Allianz Global Assistance. 8. This Travel Policy Offer is not transferable and cannot be substituted for cash or an alternative policy. 9. Eligible Participants may purchase winter sports upgrades on the Travel Policy by contacting Allianz Global Assistance directly on / insurance@allianz-assistance.ie. Irish Life Health shall have no liability for any additional payment required for these upgrades and any cost associated with the upgrade is for the Eligible Participant s account. 10. In order to get avail of the Travel Policy Offer you must register your details on within 30 days of the start date of your Health Policy or by 31 March 2017, whichever is the earlier. After you do this Irish Life Health will validate your information and forward your details to Allianz Global Assistance. Applications made after the Closing Date will not be considered.

2 11. Your Travel Policy will be sent to you via from Allianz Global Assistance no later than 14 days no longer than 14 days after you complete the online form to redeem the Travel Policy offer. Your cover under the Travel Policy will start on the same date as your Health Policy start date and will run for a period of 12 months. 12. Irish Life Health is not responsible for any matters arising in connection with your Travel Policy once issued and does not accept any responsibility for the acts or omissions of AWP P&C SA, Mondial Assistance Ireland Limited or any other third party. Irish Life Health has no liability to you in respect of the Travel Policy save for any emergency medical and associated expenses incurred by you which may be covered by your Irish Life Health Policy (subject to the terms and conditions of your Health Policy) as detailed in your Travel Policy. 13. The terms and conditions of the Travel Policy Offer are governed by Irish law and any dispute arising out of or in connection with the Travel Policy Offer shall be dealt with exclusively by the Irish Courts. 14. By registering for the Travel Policy Offer you agree that Irish life Health may process and share the information provided by you with Mondial Assistance Ireland Limited trading as Allianz Global Assistance and AWP P&C SA for the purposes above and as required to provide the Travel Policy. Please see the Privacy Statement at for further information about how we process your information or how you may access and correct the personal data that we hold about you. Irish Life Health reserves the right to withdraw or amend the Travel Policy Offer at any time without prior notice. 15.
