1 Bookings and Payments How do I book for a scheduled course? You may book for a scheduled course through our online booking system, via or contacting our customer service representatives by phone. For online bookings, please visit our website: Please note: Online booking is currently made available for non-residents of Malaysia only. To make bookings through , please send your s to bookings@msts-my.org To make bookings through the phone, please contact our HQ at during office hours from Mondays to Fridays at 0830hrs to 1730hrs (Malaysia Time : GMT +8hrs). How do I find out if a seat is available for a scheduled course? To find out about the seat availability for a course, please contact our customer service representatives via at bookings@msts-my.org or by phone at during office hours from Mondays to Fridays at 0830hrs to 1730hrs (Malaysia Time: GMT +8hrs). I have sent in my course booking form to MSTS. How do I find out if my seat is confirmed for the course? Upon receiving your course booking form via or fax, our customer service team will make all the necessary arrangements and provide you with a Course Confirmation Form (CCF) and joining instructions. Upon issuance of the Course Confirmation Form (CCF), your seat is confirmed. You are advised to read through these documents accordingly. You may also contact our Head Office at during office hours or forward your enquiry to our customer service representatives at bookings@msts-my.org. How do I make a payment? We accept payments in cash, bank drafts, company cheques or wire transfers made payable to MSTS Asia Sdn Bhd or MSTS Asia (S pore) Pte Ltd, whichever is applicable. Credit card (limited to Visa or Mastercard only) and cash payments are accepted at our training centres. Details for making payments to our bank account can be found on our Course Confirmation Form (CCF). Please note: For online booking, only credit card (limited to Visa or Mastercard) payments are accepted. I am interested to make a booking for a course but I do not see it on MSTS Asia s course calendar, how do I enquire for more information? Our customer service representatives can assist you to arrange for courses which are not regularly scheduled and only available upon request. 2 Rescheduling and Cancellations I booked a course and received the Course Confirmation Form. Is it possible to reschedule my course? Rescheduling is only allowed provided the participant must make the request in writing at least 48 hours (2 working days) prior to the commencement date (Malaysia Time: GMT +8hrs). Failure to do so will result in the participant having to bear cancellation fees. How do I request for my course to be rescheduled? To reschedule your training to another date, you will need to send your request in writing at least 48 hours (2 working days) prior to the commencement of the training date. Please contact our bookings team via at bookings@msts-my.org or by fax your letter to How do I make a cancellation? What is the cancellation policy? All cancellations must be made in writing at least 48 hours (2 working days) prior to the commencement of training. Cancellation notice period is calculated from the first day of the training (Malaysia Time:GMT +8hrs). We are unable to attend to cancellations made on Saturdays, Sundays and Public Holidays. Cancellations made less than 48 hours (2 working days) prior to the course commencement are subject to the following charges; a) For cancellations made within 48 hours (2 working days) prior to course commencement: 30% of course fee will be imposed. b) For cancellations made within 24 hours (1 working day) prior to course commencement: 50% of course fee, full one way pick up service cost and one day of hotel stay (if applicable) will be charged. c) If no cancellation is made prior to the commencement of the course or if the cancellation is made after the commencement of the course or if the participant(s) did not show up for the training, the full fees plus any applicable transport/hotel costs will be charged to the client. Certificates and Medical Reports I have completed my training. When can I obtain my certificate? A certificate will be issued to each course participant who passes / completes the course satisfactorily and after payment is cleared. How can I get a certificate to be reissued? MSTS can reissue certificates that are valid. Please send your request to certificate@msts-my.org and our representative will get back to you.

3 When should I renew my BOSIET Certificate? The OPITO approved BOSIET certification is valid for 4 years. You are required to renew your BOSIET certification by undergoing the 1-day OPITO approved FOET (Further Offshore Emergency Training) before or on the expiry date. I have a HUET (EBS) OPITO-approved certificate; must I attend the entire BOSIET course? Not necessary. OPITO allows 3 months for participants to bridge between HUET (EBS) (must be OPITO approved) and BOSIET (OPITO approved). However, the certificate will be backdated to the date of the HUET (EBS) course. Where can I get an offshore medical check-up? Is it possible for MSTS to make arrangements for my medical assessments? Medical assessments are available at our health facility at Klinik Falck Bestari located in Johor Bahru. We do not provide medical assessment services at Miri or Singapore currently. If you are attending training at our Johor training centre, your medical assessment can be arranged one day ahead or after your scheduled course at Klinik Falck Bestari. To make an appointment at Klinik Falck Bestari, please call during business hours from 0800hrs to 2200hrs (Malaysia Time: GMT +8hrs). The clinic is closed on Sundays. Alternatively, please contact our customer service representatives via at bookings@mstsmy.org or by phone at during office hours from Mondays to Fridays at 0830hrs to 1730hrs (Malaysia Time: GMT +8hrs). Will I get a Personal Safety Training Record book? A Personal Safety Training Record book is optional for participants. It can be issued to participants at an additional cost. For participants with existing Personal Safety Training Record book, it can be updated and stamped on completion of the course free of charge. How can I obtain an MSTS Safety Card? Participants have an option to obtain the MSTS Safety Card at RM20/SGD10/USD7 from our training centres. I understand that OPITO certificates are valid for 4 years. Why would my employer require me to go for a refresher course while my certificate is still valid? Although the BOSIET OPITO certificate is valid for 4 years, depending on the operators or the particular country's authority, you may be required to attend a refresher training in less than 4 years

4 Transportation and Accommodation Can MSTS arrange for my accommodation, daily pick-up to / from hotel to the training centre and airport transfer? Yes. For your convenience, you may book a course package which includes transportation and accommodation. At an additional cost, you can count on us to make arrangements for your transportation and hotel reservations with selected hotels for the duration of your course. We can also arrange for transportation from any point within Singapore or Malaysia as well as pick-ups from major airports, i.e : KLIA, Malaysia, Miri International Airport (Sarawak) and Changi Airport, Singapore. Please note: Rates will vary depending on the services you require. For further information, please bookings@msts-my.org I wish to drive to MSTS s training centre in Johor Bahru, do I need to make any special arrangements to enter the Johor Port area? Our training centre in Johor Bahru is located within the restricted area of the Johor Port. All private vehicles are required to register with the port authorities before entering the port area. A Johor Port Clearance Form is provided along with your course confirmation form (CCF). You will need to provide a copy of the completed form to the port authorities when you enter Johor Port to get to our training centre. Other Frequently Asked Questions Are courses conducted in English? All of our courses are conducted in English. Participants should be proficient in the English language. If required and at a client s request, MSTS may assist in getting an interpreter if the participant is not proficient in the English language. What time should I arrive at the training centre? Participants are required to be at the training centre at 0815 hrs on the first day. Training will begin at 0845 hrs on subsequent days. Are meals provided at the training centre? Lunch and tea breaks at the centre are included in the course fee. Dinner is not provided. Do I need to bring my own personal protective equipment (PPE) for the training? All Personal Protective Equipment (PPE) will be provided during training. For practical pool exercises, participants are required to bring their own swimwear.

5 What type of attire should I have during training? Participants are requested to wear appropriate attire for training. Shorts, singlet, sandals and thongs (flip-flops) are not allowed. Personal Protective Equipment (PPE) will be provided during training. What should I bring along to the training centre? 1. a copy of course confirmation form for the course(s) you are attending 2. a copy of passport or NRIC 3. a copy of valid medical certificate 4. existing Personal Safety Logbook (if any) 5. appropriate footwear 6. swimming attire (if applicable) What does OPITO stand for? OPITO stands for Offshore Petroleum Industry Training Organization (UK). OPITO is the custodian of industry standards in emergency response and industry training and competence. The OPITO standards are a set of training and/or competence standards, produced and agreed by the offshore oil and gas industry covering a variety of offshore emergency response duties and occupational roles. If you would like to learn more about OPITO, please visit What does NOG stand for? When is BOSIET with NOG approval required? NOG stands for Norwegian Oil and Gas Association. The Norwegian Oil and Gas Association (Norwegian Oil and Gas) is a professional body and employer s association for oil and supplier companies engaged in the field of exploration and production of oil and gas on the Norwegian Continental Shelf. The Norwegian Oil and Gas Association works to solve common challenges for the members and to strengthen the competitiveness of the Shelf. To learn more about NOG, please visit Should I apply for the Seaman Card before or after the Basic Training Course (IMO STCW 78)? Can MSTS assist me to apply for the Seaman Card at the Malaysia Marine Department? A Malaysia Marine Seaman Card is the pre-requisite for you to undergo the IMO STCW 78 Basic Training as this course is certified by the Malaysia Marine Department. If you do not have a Malaysia Marine Seaman Card, we can assist you with the application prior to the training at an additional fee. We will arrange for you to undergo a Malaysia Marine Department Medical Check Up (the pre-requisite for the Malaysia Marine Seaman Card application) before proceeding with the seaman card application. What is BOSIET (Basic Offshore Safety Induction & Emergency Training)? The BOSIET Course (OPITO approved) is designed to assist in meeting the initial onshore safety and emergency response training and assessment requirements for personnel new to the offshore oil and gas industry as identified in the OGUK Guidelines for the Management of Competence and Training in Emergency Response. The BOSIET course is a component part of the UK Step Change Common Induction Process which

6 supports performance standards and knowledge requirements for the following National Occupational Standards: Unit 1: Travel safely by helicopter to an offshore installation Unit 2: Live and work safely on an offshore installation Unit 3: Respond to offshore installation emergencies The BOSIET (OPITO Approved) certificate is recognized in both tropical and non-tropical regions. Please note: BOSIET includes the HUET with EBS module which is a compulsory requirement by Shell and Petronas. Travel Safely By Boat is a requirement by Sarawak and Brunei Shell and Petronas. Please request for BOSIET with Travel Safely by Boat (OPITO) if you will make your way to Shell's installations in Sarawak and Brunei and Petronas. Are there any pre-requisites to attend a course? Participants must ensure that they are in possession of the correct pre-requisite certificates or logbook endorsement required for the course. As certain elements of the course are of a physical nature, a certain level of fitness, sobriety and possession of a valid medical certificate are required prior to registration of the participant. Drugs and alcohol are prohibited on the premises. What is the difference between T-BOSIET and BOSIET? T-BOSIET certifications are only valid for use in tropical region while BOSIET certification is valid for both cold water and tropical water regions (except the Norwegian sector of the North Sea). MSTS's BOSIET course includes additional Norwegian Oil and Gas Association modules, which is accepted to be used in Norway by some operators. Is the Helicopter Underwater Emergency Training (HUET) included in the BOSIET course? Yes, the BOSIET is inclusive of: 1. Basic Sea Survival 2. Basic Fire Fighting 3. Basic Emergency First Aid 4. Safety Induction 5. Helicopter Underwater Escape Training with Emergency Breathing System (HUET EBS) We also include additional Norwegian oil and gas modules of Norwegian Heli-suit and Skyscape training as free add-ons into our BOSIET to expose our participants to Norwegian Oil and Gas Association requirements.
