1 Minimum Project HSE Requirements 1.0 GENERAL 1.1 Health, Safety, and Environmental (HSE) compliance in the workplace is the Company s primary initiative. The Client and the Company require that the Subcontractor and its Lower Tier Subcontractors place the highest importance on HSE at all times during the performance of the Subcontract Work at the Site. 1.2 Subcontractor shall correct any deficiencies immediately upon notification by Contractor s personnel. Contractor s personnel shall have the right to require immediate cessation of work in the case of imminent danger to life or property. 1.3 The failure and/or refusal of the Subcontractor or any of its lower tier Subcontractors to fulfill its HSE responsibility or to correct or abate non-standard HSE conditions, practices, or acts within a reasonable period will result in corrective action being taken by Contractor and/or client. 1.4 The Subcontractor shall ensure that these requirements are included in any Lower Tier Subcontractors contracts. 1.5 All Subcontractors and Lower Tier Subcontractors shall comply with applicable Federal, State, and Local regulations. 1.6 If USACE EM (2014) is applicable to the project, all Subcontractors and Lower Tier Subcontractors shall comply with the requirements of EM Subcontractor shall provide HSE training for its employees, provide specific HSE instructions on special activities, and provide ongoing HSE supervision. 2.0 HSE POLICY 2.1 It is the Policy of the Company and a key component of the Company s culture, to execute all activities in such a manner that will ensure Health, Safety, and Environmental protection of all employees, Clients, Subcontractors, Suppliers, and the communities in which we live and work. 2.2 All Managers and Supervisors of the Company and Subcontractor have the responsibility to continuously improve HSE awareness among all employees, to manage proper usage of the environment, tools and equipment, and to create a culture in which everyone shares responsibility for the well-being of their fellow workers and community. 2.3 All employees of the Company and Subcontractor have the responsibility to execute their personal and work activities in such a manner as to prevent all circumstances which could lead to incidents that may cause personal injury or illnesses, security incidents, or environmental damage. 2.4 The Company and Subcontractor will implement this Policy in combination with the HSE laws, regulations, standards, and codes of practice of our Clients and all applicable governmental agencies. 2 3.0 APPLICABILITY 3.1 The Project Policy and Accident Prevention Plan (APP)/Site Safety and Health Plan (SSHP) apply to all Company, Subcontractor, and Lower Tier Subcontractor persons at the Site, regardless of position. The safety rules apply to anyone who is on the Site at the invitation or under the control of Company, Subcontractor or Lower Tier Subcontractor. 4.0 RESPONSIBILITIES The requirements of this document represent the minimum HSE requirements from the APP. The APP also contains specific site requirements such as site maps to medical facilities, emergency response, project organization, reporting contacts, etc. The APP will be provided to the subcontractor at or prior to mobilization. Each person on the Project is responsible for the following: 4.1 Learning, understanding and complying with all HSE procedures, Legal Duties, Regulation and Codes, that are applicable to employer or workers. 4.2 Complying with applicable legal duties, regulation and codes which may be imposed on: Employers, Workers, Licensees (construction or engineering licenses if applicable) Contractors / Subcontractors, Prime Contractors, and Suppliers 4.3 Being responsible for his or her own personal safety and the safety of co/workers, through both their acts and their omissions, 4.4 Being constantly aware of his or her work situation and for reporting hazardous situations to supervisors, immediately stopping work, and for informing immediate supervision if there is believed to be the potential for any harm, 4.5 Use and maintaining appropriate Subcontractor supplied Personal Protective Equipment (PPE). 4.6 Understanding that any employee who jeopardizes his or her own safety and health and /or the safety and health of others will be subject to disciplinary action (up to and including immediate termination of employment), 4.7 Working in a safe manner at all times. This includes complying with all HSE instructions as contained in the Company procedures, orientation and any subsequent information that may be conveyed from other internal sources (i.e., HSE Meetings, training classes, or special HSE bulletins, alert notices, know how notices, etc.). Employees shall stop their immediate or impinging work as soon as safely possible if they consider the work being performed is at risk or unsafe. 5.0 IMPLEMENTATION 5.1 As it is Project Policy to endeavour to prevent incidents, the Company is committed to the safety and well-being of personnel on the Site as well as the prevention of wasteful and inefficient operations and damage to property, equipment, and the environment. The Subcontractor shall comply with this Policy and ensure that its employees and Lower Tier Subcontractors commit to the requirements. 3 5.2 This HSE Program has been designed for 100% Subcontractor involvement as follows: Management's commitment to HSE Effective job safety training for all categories of employees Safety Task Instruction, i.e. Safety instructions given with every work task. 6.0 HEALTH AND SAFETY CULTURE 7.0 GOALS 6.1 The Subcontractor shall promote awareness of health and safety among staff, personnel, Lower Tier Subcontractors and suppliers employed under this Subcontract. 7.1 Excellent HSE performance and the health and safety of everyone who works for the Company are critical to the success of the Company s business. Company HSE goals are simply stated: No harm to any person, No damage to property, No compromise to project security in any respect, No harm to the environment, and Improve on current best industry practice. 7.2 The Company recognizes that HSE management is a line management responsibility, the principles of which Company employees and subcontractors (including Subcontractor) will support and that management will lead by example and support its workforce in working safely. 7.3 The goals are a clear affirmation that injuries, accidents and harm to people or the environment are unacceptable to Company within the Project s operational culture. All incidents that do occur will be investigated to determine the root causes, to endeavor to prevent any repetition. Findings of such investigations will be communicated to the Project workforce including Subcontractors and Lower Tier Subcontractors. 8.0 PREMOBILIZATION 8.1 The Subcontractor shall participate in a kick-off or prep meeting as appropriate with designated members of the Company s Project Management staff prior to start of the Subcontract Work. This phase is to provide an opportunity for the Subcontractor to become familiar with the location, facility, procedures, personnel and other work information including: To ensure a clear understanding of the Company and Subcontractor s responsibilities in compliance with HSE requirements; To communicate Subcontractor s commitment to safe practices; To identify with the Subcontractor any other activities being conducted on site that could affect the subcontractor s work or safety; and To communicate the Company s commitment to safe practices. 8.2 Site Pre-Access Drug and Alcohol testing shall be completed before any employee or potential employee can be given Site orientation or commences work on Site. 4 9.0 PROJECT HSE CHECKLIST 9.1 The following Project HSE Check List details the minimum requirements to be observed on the Project Site. 9.2 General HSE Rules Drinking water coolers are for drinking water and ice only. Tampering with or placing items such as drinks, etc., in the water cooler will result in immediate termination. Common drinking cups" are not allowed. Disposable or personal use cups only. Drinking water coolers shall be sealed and dated with tape around the lid. Fresh drinking water (changed at least daily) shall be readily available to workers HORSEPLAY" on the jobsite is strictly prohibited Maximum jobsite speed limit is 10 MPH (20 Km/h) unless otherwise posted. No one is permitted to ride in or on a truck standing up. Riding as a passenger on equipment is prohibited. Riding in the bed of a pick-up truck is prohibited. Seat belts shall be worn at all times when in a moving/operating vehicle Cameras, weapons (firearms, knives, etc.), alcoholic beverages, or illegal drugs are not allowed on site. Medication prescribed by physician must be registered with Project HSE Hoses, cords, and leads shall be routed overhead, buried, or otherwise placed to avoid causing tripping hazards and to prevent damage Prior to starting work, Subcontractor s personnel shall complete an HSE Orientation to include, as a minimum: Contractor HSE Philosophy Project and Company HSE Rules Job site and Client specific HSE rules and procedures Location and availability of first aid and medical facilities Method of reporting incidents Emergency procedure and muster points All unsafe conditions and incidents shall be reported immediately to the Company s supervisor and the Company s HSE department immediately so corrective action can be taken. 9.3 Housekeeping and Sanitation General neatness Regular disposal of trash Passageway, driveways and walkways clear Adequate lighting Oil and grease removed Waste streams shall be segregated into separate containers as required by contract requirements Adequate supply of drinking water Sanitary facilities must be adequate for staff levels Adequate ventilation Impalement Hazard Protection Workers shall be protected from vertical and horizontal impalement hazards via: Hard barricades with appropriate signage Wood covers Guard rails and other suitable devices to prevent falls. 5 Capping the ends of protruding vertical rebar with caps containing steel plates when an impalement hazard exists Bending the ends of the protruding steel rebar Training of workers Smoking is strictly prohibited except in designated areas only Housekeeping shall be an integral part of every job. Subcontractor is responsible for conditions in its respective areas. Supervisors and employees are responsible for keeping their work areas clean and hazard free Refuse and scraps must not be allowed to accumulate, particularly when they interfere with work flow or create fire hazards. All refuse containers shall be marked as to contents such as recycle metal, wood, trash, etc. In addition, clean-up of the work area is required when you finish a job or at the end of the day Glass bottles or containers are not permitted on site. 9.4 Personal Protective Equipment All Hard hats shall meet the specifications of ANSI Z89.1 as required by OSHA or the equivalent recognized CE, AS/NZS standards. Hard-hats will be worn in accordance with manufacturers recommendations by all employees on the Project Site. Alterations or modifications of the hat or liner are prohibited All hard hats work reversed shall be marked by the manufacturer indicating reverse donning is acceptable Bump Caps and metallic hard hats shall not be used at APTIM sites All personnel shall have their Company logo and their name prominently displayed on their hard hat. The name shall be applied above the brim of the hard hat using block letters Hard hats that have been altered by drilling or cutting shall not be permitted All personnel shall have their Company logo and their name prominently displayed on their hard hat. The name shall be applied above the brim of the hat using block letters Welders are required to wear hard hats when in a hardhat area. Soft cap welding shall not be permitted When using a face shield or welding hood, protection that is compatible with a hard hat shall be used Operators of vehicles and equipment shall wear protective headgear during the operation of such equipment except in enclosed cabs Hard hats shall also be required in areas that typically would not require the use of a hard hat (examples include offices, break areas, and barracks) when the work activity creates an overhead hazard Safety glasses will be ANSI Z87.1 approved and worn as the minimum required eye protection. Additional eye and face protection such as mono goggles and face shields are required for such operations as grinding, jack hammering, utilizing compressed air, or handling of chemicals, acid and caustic. Burning goggles for cutting, burning or brazing and welding hoods for welding, etc Dark safety glasses will not be allowed inside of building or other low light areas Goggles shall be worn where flying particles exist Gloves where required and appropriate for the task being performed Form Number: AMSF FM Revision 0 Issued for Use: 9/08/2017 Page 5 of 14

6 9.4.4 If fall protection is needed by the Subcontractor for their scope of work, Subcontractor is responsible for providing fall protection and required training for their employees in accordance with the requirements of ANSI Z Subcontractor will required to have a certified 24-hour Fall Protection Competent trained individual on site when their employees are working at heights, exposed to fall hazards and/or using fall protection equipment Fall protection (an approved guardrail, fall restraint or personal fall arrest system) shall be used any time there is a fall hazard of more than 6 feet (1.8 meters) or more above the ground or next lower working surface % tie-off is required on all ladders where an employee is exposed to a fall of 6 foot (1.8 meters) or more. This distance is measured from the employee s feet The use of a Safety Monitor System (Controlled Access Zones/Safe Zones) is not permitted Failure to follow fall protection requirements will result in immediate removal of the offending Subcontractor personnel from the site. Multiple infractions may result in contract termination Rigging used as part of a Fall Protection system must be designated for Fall Protection Only and be painted florescent orange or have a florescent orange tag affixed indicating its purpose. No rigging which has been used for material handling or otherwise been loaded may be used for fall protection Lifelines shall be erected to provide fall protection where work is required in areas where permanent protection is not in place. Horizontal lifelines shall be a minimum of 1/2 inch (12mm) diameter wire rope % fall protection includes the use of double lanyards Scaffold builders will use snap hooks on their lanyards specifically designed for scaffold builders Work clothing must provide adequate protection for the body. Shirt sleeve length and shirt material shall be established by hazard assessment (JSA or client requirement) Shirts must be worn tucked in at all times unless the employee is flame cutting or welding. Shirts must be tucked back in before grinding or working around other rotating equipment Long trousers required, no short pants allowed No polyester or nylon clothing will be allowed In some instances and at certain facilities Flame Retardant Clothing (FRCs) must be worn. When FRCs are required they must be worn as the outermost garment. Personnel at work locations shall wear protective footwear with ASTM-F2413 approved safety toe design. The work boot shall have an upper that covers the ankle when the hazards warrant such protection. Safety toed tennis shoes are prohibited Open toe shoes, high heel shoes, sandals, hush puppies, tennis shoes, jogging or athletic looking shoes, and other such footwear is prohibited on APTIM sites. Some synthetic materials can have chemical absorption rate issue s and care should be given to ensure boots made of synthetic material meet or exceed the absorption qualities of leather boots. 7 9.5 Fire Protection/Prevention All employees must be presented general awareness level Fire safety training Fire extinguishers must be inspected, charged, and maintained Smoking is strictly prohibited except in designated areas only Subcontractor s Gasoline and other flammable materials must be kept in OSHA compliant or DOT approved containers, and storage requirements for quantities and types must comply with applicable governmental regulations. Bulk storage of fuels is discouraged. Special permission shall be obtained from the Company prior to storing fuels in bulk volumes No Smoking signs must be posted in areas where flammable materials are stored. Company will identify designated smoking areas on project site, and Subcontractor must enforce no-smoking areas on the Site Subcontractor must furnish the appropriate number, size, and type of portable fire extinguishers required for its work area and must train its personnel in proper use. Fire extinguishers must not be tampered with or removed from their assigned locations. 9.6 Hand and Power Tools All tools must be inspected for the proper operating condition All tools shall be stored properly and neatly All power tools shall be properly grounded All tools must be inspected for proper safety guards including functional Dead Man s Switch Crowfoot connections on air hoses shall be secured with the correct pin (clevis pin) to prevent accidental disconnection. Compressed air shall not be used to dust off hands, face, or clothing. Whip checks must be installed on all connections or the supply must be equipped with excess flow valves Where ever possible the Subcontractor will evaluate and implement the use of battery operated tools (i.e., drills, saws, etc.) in place of AC driven electrical power tools All equipment and tools, whether company or personal, must be in good working condition. Defective equipment and tools must not be used. Examples: chisels with mushroomed heads, hammers with loose or split handles, guards missing on saws or grinders, etc. All power tools and engine driven equipment will be inspected by the Subcontractor and tagged/identified in a readily visible manner as having been inspected and found safe to use in accordance with Contractor s requirements All electrical tools and equipment shall be inspected and color-coded by a designated competent person each month. (See Section 39.0) Low voltage, GFCI controlled or intrinsically safe lighting is required in metal tanks/vessels and in other confined spaces with flammable atmospheres. If GFCI s are used on lights for confined space entry, backup lighting shall be immediately available in case the GFCI trips Electrical cords and equipment must be properly grounded or double insulated and checked. 9.7 Electrical All portable tools and cords shall be properly grounded or of a double insulation design Daily visual inspection of caps, ends, and cords for deformed or missing pins, insulation damage, and internal damage shall be performed Tests of cords, tools and equipment for continuity and correct attachment of the equipment grounding connector to the proper terminal shall be made every month and: Prior to first use Prior to return to service after repairs Prior to return to service after any incident which may have caused damage to cord or equipment Form Number: AMSF FM Revision 0 Issued for Use: 9/08/2017 Page 7 of 14

8 9.7.4 Cords and equipment that do not meet requirements shall be removed from service until repairs have been made A written log of all tests on cords, tools, and equipment shall be maintained. 9.8 Portable Ladders Only Type 1A or 1AA ladders may be used. (Extra Heavy Duty or Extra Extra Heavy Duty) Competent Person shall inspect and color code portable ladders monthly. (See Section 39.0) User must complete a pre-use visual inspection prior to using ladder. Ensure: There are No broken or missing rungs or steps. There is No broken or split side rail. The ladder extends at least 3 feet (1 meter) above the walking surface/landing and is secured at the top and bottom Wooden ladders shall not be painted Damaged ladders shall be removed from service Metal ladders around electrical work are prohibited. 9.9 Motor Vehicles Vehicles shall not be overloaded (maximum load to be clearly posted on vehicle) Trash trucks shall have covers Riding in bed or on edge of pickup truck bed at any speed is prohibited Riding on concrete trucks, loaders, backhoes, etc. is prohibited Back-up alarms shall exist and be operational on all trucks and heavy equipment, e.g., loaders, tractors, backhoes, etc. (Alarms shall emit a minimum noise level of 90 dba at 1m) Employees operating vehicles and passengers in motor vehicles shall wear seat belts at all times. The seat belt requirement applies to front and back seat passengers. The vehicle driver is responsible for ensuring compliance with these requirements Vehicle and equipment operators are not allowed to talk on or answer mobile phones while driving/operating vehicles or equipment on site Material Storage and Handling A material storage area shall be designated Material shall be kept at least 3 feet (1 meter) from edge of excavations Automatic sprinkler controls and electrical panel boxes shall be kept free and unobstructed Material storage and handling areas shall be inspected frequently Environment All material containers shall be properly labeled Secondary containment shall be provided for storage of hazardous material. See Construction/Facility Environmental Management Plan Incomplete waste and chemicals shall be properly segregated Spills and/or releases shall be contained, cleaned up by trained personnel and reported immediately Waste shall be transported and disposed of in accordance with applicable local laws and regulations A Spill Prevention Control and Countermeasures (SPCC) Plan shall be in place A Waste oil accumulation plan shall be in place, where applicable A Storm Water Pollution Prevention Plan shall be in place, where applicable Storage tanks must have secondary containment with sufficient capacity to meet applicable regulations Form Number: AMSF FM Revision 0 Issued for Use: 9/08/2017 Page 8 of 14

9 Subcontractor shall transfer all hazardous or contaminated waste to a point to be designated by Contractor. The Subcontractor shall be responsible for proper disposal of hazardous waste unless otherwise instructed by Contractor Subcontractor will supply a copy of Safety Data Sheets (SDS) to Contractor for all Subcontractor supplied hazardous and toxic material prior to such material being introduced to the Project Site. The Subcontractor will provide its employees and Lower Tier Subcontractors employees adequate training regarding the hazards and safe use of these materials. A central file of all SDSs will be maintained by Contractor. SDSs are to be provided in both English and the commonly used language of their employees DRUG AND ALCOHOL POLICY 10.1 Subcontractor agrees to advise its employees and the employees of its Lower-Tier Subcontractors and agents that it is the policy of Client/Contractor that (1) the manufacture, dispensation or sale, offer to sell, purchase, use, transfer of possession of illegal drugs on Client s premises, is prohibited; (2) employees, while on Client s/contractor s premises, are prohibited from being under the influence of alcohol ( Under the Influence means that the employee is affected by alcohol in any detectable manner); (3) entry onto Client s premises constitutes consent to an inspection of the employee and his or her vehicle or personal effects of any kind while entering, on, or leaving said premises; (4) any employee who is found in violation of this Policy or who refuses to permit an inspection may be removed and barred from Client s/contractor s premises, at the discretion of Client/Contractor. As used herein, Client s/contractor s premises means Client s property including Client/Contractor owned project site locations in which Client s/contractor s business is being conducted and Client s/contractor s owned or rented vehicles and/or equipment Use of alcoholic beverages or controlled substances is prohibited Subcontractor shall indemnify, defend, and hold Client and Contractor harmless from any damages, suits, or claims by its employees relating to enforcement of this provision FITNESS FOR DUTY 11.1 Subcontractor is responsible for providing workers of sufficient skill and knowledge to accomplish the Work in conformance with quality and safety requirements. Subcontractor shall undertake all training required for its workers to execute the Subcontract Work. All Training Documentation shall be provided as requested prior to accessing the site. Subcontractor is responsible for taking all reasonable measures toward Client s and Contractor s goal of a drug-free workplace except where prohibited by state or local laws or regulations. The cost and time required to complete such testing is to be borne by Subcontractor Company will not be liable for any loss of production associated with fitness for duty or postaccident screening FIRST AID FACILITY 12.1 All project-related injuries or illness must go through the on-site first aid station (if any) or designated off-site medical providers. If the Subcontractor has a preferred occupational medical provider different than the designated provider, the name of the occupational medical provider must be provided to HSE prior to first site access All Subcontractor and Lower Tier Subcontractor motor vehicles used for project work are required to have a First Aid kit in the Vehicle meeting the requirements of AMS PR-01600, First-Aid & Medical Services. 10 13.0 INCIDENT MANAGEMENT REQUIREMENTS 13.1 The Subcontractor shall verbally report immediately all injury / illness incidents to the Company for the following: Fatalities, Lost time injury / illness, Restricted Work injury / illness, Medical treatment injury / illness, First Aid, Loss of Consciousness, All significant diagnosed injury / illness, Environmental incidents (spills); and Great Catches (Near Misses) The Subcontractor shall provide injured parties with adequate first aid care using the facility described in Section 12.0 First Aid Facility above and shall perform the appropriate level of case management The Subcontractor will utilize a structured Restricted Duty Program and attempt to make injured workers productive as soon as possible without compromising the healing process The Company and Subcontractor shall investigate all incidents, including near miss incidents, and shall identify the root and contributing causes of the incident using the Company s incident investigation program. The Company and Subcontractor shall base the magnitude of its incident investigation effort upon the potential seriousness of the incident, rather than the actual outcome of the incident. The Subcontractor shall provide qualified senior management, line management and safety representatives to oversee and participate in incident investigations The Subcontractor will participate in an Incident Review Board (IRB) with the Company, and share lessons learned with the Company and Subcontractors COMMUNICATION OF SAFETY EXPECTATION 14.1 The Subcontractor shall communicate and periodically reinforce the Company s safety management expectations to all levels of Site personnel at the Site Subcontractor may be required to: Participate in Company and Client sponsored meetings and alignment workshops SAFETY MANAGEMENT PERSONNEL 15.1 Subcontractor s site supervisor or lead person for the work is considered the site safety and health officer on projects and must understand their responsibilities under this document ENFORCEMENT OF HSE REQUIREMENTS 16.1 Contractor shall inform the Subcontractor of any HSE violations. The Subcontractor shall immediately take appropriate corrective actions Violation of a HSE rule may result in suspension or immediate removal from the project site. 11 Any Subcontractor (or lower tier Subcontractor) employee receiving three (3) violations within a six (6) month period shall be removed from the project site Any HSE violation by a Subcontractor employee found to subject any employee to an Immediate Danger to Life or Health may result in immediate removal of the Subcontractor employee from the project site If the Subcontractor discovers or creates a hazard or situation that can cause death or serious injury, work shall be immediately stopped by the Subcontractor until the hazard is corrected. The Contractor reserves the right to validate corrective measures prior to resumption of work. The Subcontractor shall reimburse expenses resulting from a continuing uncorrected hazard to Contractor. These expenses include impacts to Contractor, Clients, other Contractors, visitors, vendors, and suppliers. If the Subcontractor refuses to correct the hazard, Contractor shall take corrective action and will withhold Subcontractor payment. Contractor shall exercise the right to terminate the Subcontractor for recurring violations or refusal to correct hazards PROJECT SAFETY TRAINING 17.1 The Subcontractor shall provide personnel with the knowledge, skills and training to perform their jobs in a safe manner The Subcontractor shall provide proof of training in the form of certificates or other documentation to demonstrate training compliance prior to work activities beginning when requested Subcontractor personnel will attend HSE and skills training as deemed necessary by the Client or Contractor ACTIVITY HAZARD ANALYSIS 18.1 If EM is applicable, Subcontractor shall use the Activity Hazard Analysis (AHA) program that describes the process of identifying hazards before commencing each major phase or activity at the Site. All others shall comply with the client requirements. The analysis will also describe the plans and assign responsibility for hazard prevention or control of identified hazards JOB SAFETY ANALYSIS PROCESS 19.1 The Subcontractor shall develop daily JSAs to communicate risks/hazards and the mitigation requirements necessary to protect the individual work crews based on job tasks being performed JSA s are to be reviewed and revised if needed prior to the start of each shift and major break (lunch) or while in the field where there has been a change in the work plan or immediate surroundings When hazards are identified, Subcontractor shall attempt to eliminate the hazard. When elimination is not achievable, Subcontractor shall control the hazard through use of each engineering controls, administrative controls, additional protective equipment, or heightening awareness of the hazard SAFETY DATA SHEETS 20.1 The Subcontractor will provide to the Company copies of Safety Data Sheets (SDS) for all materials that it or its Lower Tier Subcontractors bring on the Site. Provision of such SDS s will occur 7 days prior to the arrival of the materials on the Site. Subcontractor will provide training to the Company s employees and employees of other parties who may be routinely exposed to such materials. This training will be at no cost to the Company. 12 21.0 SAFETY MEETINGS 21.1 The Subcontractor shall conduct a tailgate safety meeting at least weekly. All Subcontractor supervisory and craft employees shall attend these meetings, and attendance shall be documented. Topics shall be pertinent to the work the Subcontractor is performing LESSONS LEARNED AND BEST PRACTICES 22.1 Upon request by the Contractor, the Subcontractor shall attend Lessons Learned Workshops or meetings to capture Site lessons and best practices over the course of the work. The Company and Subcontractor shall disseminate high impact lessons learned as they are captured The Subcontractor shall cooperate with incident investigations, attend Incident Review Board(s) (IRB), and disseminate lessons learned as requested USE OF CELL PHONES AND RADIOS 23.1 If required for project work communication purposes and if allowed by Site safety and security rules, cell phones are only to be carried by Supervision No non-supervisory craft workers are to carry cell phones on the jobsite, but are to leave cell phones in lunchroom, vehicle, or off site Without regards to the previously stated limits, Subcontractor employees are discouraged from using their cell phones to place or answer calls while driving: If necessary, calls may be answered using hands free devices only Texting or any activity that requires manual manipulation of the phone or takes the operator s eyes off of the road while driving is prohibited The foregoing prohibition against cell phone use does not apply to the following: As allowed by site and as approved by PM subs can use communication devices like cell phones. Use of cell phones by passengers is permitted if the use is not a distraction to the driver of a passenger vehicle Use of cell phones by passengers is permitted if the use is not a distraction to the driver of a passenger vehicle ENVIRONMENTAL WILDLIFE MANAGEMENT 24.1 Subcontractor shall comply with following practices for the Site. These practices outline the strategies to minimize the harm to wildlife and site personnel by using best management practices. An Environmental Work Permit may have to be obtained prior to commencing activities that have the potential for wildlife interaction. Restrictions may be identified by the Client and or Environmental and Regulatory team for sensitive wildlife areas Company will attempt to implement measures to protect animal species and their habitats by limiting access to areas where work is not required Collection of dead or living animal materials shall not be allowed Hunting or any other method for catching or killing animals shall not be allowed No worker shall feed or harass wildlife, including insects, birds and reptiles. 13 24.6 Wildlife topics will form part of orientation to all employees that will be working in areas with the potential for wildlife interaction SAFETY DRILLS AND EMERGENCY RESPONSE 25.1 Emergency contact list (containing direct, cell and ) shall be developed by each Subcontractor, and placed in a conspicuous location near the telephone (such as the bulletin board in the office trailer). All personnel shall be made aware of emergency contact list location(s) Subcontractor and Company personnel will be required to participate in the following drills, during the period of work at the Site Fire Drill, Emergency Evacuation Drill, Man Down Drill (ERST), and Emergency Response Exercise The Subcontractor shall comply with Emergency Evacuation Drills as required by the Client. Subcontractor shall make allowances in supervision and schedule for such drills The Subcontractor shall review and brief to employees the Company s Emergency Response Plan SPILL PREVENTION AND RESPONSE 26.1 Subcontractor shall immediately notify the Company and shall report all spills occurring at the Site. A written spill report, which will be on the Company Incident Report (AMS FM ), must be provided to the Company within twenty four (24) hours of the occurrence or discovery of a spill. The report must include the date, time, location, volume, material(s) involved, specific gravity, clean-up actions taken, root cause analysis and corrective actions to prevent future occurrence Subcontractor's vehicles shall carry weather tight "Spill Kit for immediate use in the event of a fluid or Oil leak SECURITY 27.1 Subcontractor shall fully participate in and cooperate with all security programs implemented by the Client and/or Company to meet the security objectives of the Project, and shall provide all statistics, information, training and education in security required by such programs. Subcontractor shall provide sufficient staff on the Site to provide such information and programs in accordance with Subcontractors Site Security Plan (SSP) Subcontractor shall provide a Project specific SSP, in compliance with Client's and/or Company s security requirements, and which may address, but not be limited to, the following: Security Management; Perimeter Barriers; Access Controls; Building Security; Closed Circuit Television (CCTV); Protective Lighting; Guard Service Operations; Incident Reporting and Investigation; Security of Information; Derogations of Standards; and Compliance Assurance Program SEARCHES AND INSPECTIONS 28.1 Client and Company s appointed security service will carry out random searches of all personnel and any vehicles both on entry to the Site and on departure and Subcontractor must comply with any such request.
