1 Lindon City TEL North State Street FX Lindon, UT Invitation to Pre-Qualify Lindon City Public Safety Building Proposals ue: Thursday, January 21, 2016 at 5:00pm 1.1 Construction Services for Lindon City Lindon City is seeking contractors that would like to pre-qualify to submit bids for the construction of the Lindon City Public Safety Building. Only contractors who are pre-qualified by Lindon City through this process will be allowed to submit bids for the project and potentially be awarded the single contract. If you would like to submit information that would qualify your company, please review the attached pages and supply the requested information by Thursday, January 21, 2016 at 5:00pm. 1.2 Project escription Lindon City plans to construct a new Public Safety Building that will contain both the Police and Fire epartments. The two-story building will consist of the Police epartment and associated offices with an attached 3-bay pull through garage on the main floor and firefighter living accommodations upstairs. The facility will be constructed on land immediately south of the Lindon City Center at 60 North State Street, Lindon, UT Full bid documents and plans are anticipated to be available for bidding by pre-qualified contractors starting January 28 th. Bid packages will be posted on BidSync. Bids will be due on February 11 th at 5:00pm. Scope of work is anticipated to include the following general items: Construct two-story 17,538 sq/ft commercial facility consisting of: o Type 5 construction o Fully sprinkled building o Combination of wood frame and masonry block construction o ll brick and stone exterior Site work: project is being built over an existing parking lot and will include demo of existing asphalt/concrete/surface improvements removal, grading, utility relocations and new utility connections, new asphalt & concrete, landscaping, etc. Total work site will consist of approximately two acres. djacent city facilities will be in use during construction. Some parking lot traffic control may be required and pedestrian access to other city facilities and parking will be required. RFT floor plans are attached at the end of this packet. 2 P a g e Estimated Timelines for Bidding & Construction draft timeline for bidding selection and construction is below: Pre-qualify Contractors January 11 th 21 st, 2016 Bids due by Pre-qualified bidders February 11 th, 2016 at 5:00pm (complete bid package for pre-qualified bidders will be available January 28 th ) Lindon City Council wards Bid February 16, 2016 Construction March ecember 2016 (10 Months) 1.4 Pre-qualification Submittal Content Submitted materials shall represent the respondent s ability to provide the services described. Submittals should be concise and contain only information specific to the objectives of this project and the respondent s experience with the construction of facilities with similar type construction and size. Proposals should not exceed twenty (20) printed pages, not including covers or dividers. ll proposals submitted for consideration should be organized under the following headers in the same order as presented below.. Relevant Firm Experience, Qualifications, and Capacity Submissions should include: Firm names, addresses, contact information, contact , and current licensing; o Please provide a copy of the current contractor license information (the minimum license required for this project is a B-100 license); o Provide license holder name, type of license, and any supplemental classifications held; and o Please state and explain any disciplinary actions taken by the State of Utah to revoke or suspend the above license or attempts to investigate the license holder for business or construction related reasons (include on a separate and attached sheet of paper). brief history of the firm, including: o years of operation o number of full-time employees (separate office employees from field employees) o staff qualifications / certifications o intended project managers and/or project superintendent and their previous work experience on similar buildings, etc.; (NOTE: project references should include projects performed by these individuals); Verification of firm capacity to complete project. o Include any details that show contractor can complete project by end of ecember 2016 and willingness (or not) to commit to a specific contract time with possible penalties and/or liquidated damages if project is not completed on time; o On the four projects under your references as required below, please list contractual completion time frame for each project versus the actual completion time on each project (how long it was supposed to take before starting vs how long it actually took to complete) ny other qualifications applicable to demonstrate expertise and ability to successfully complete this specific type of project. B. Experience and Project References Contractor shall show a minimum of 5-years of experience constructing similar types of commercial projects. To verify this, please include a minimum of four (4) references to document the proposer s demonstrated performance and experience completing similar size and complexity of commercial projects. t minimum, please indicate the following: project locations and building owner; the contactor s role in the project; date of construction;

3 P a g e 3 building size and construction type; total cost of construction; whether the construction was completed on-time and within budgeted constraints; and telephone number and contact for the reference. C. Insurance Coverage Provide insurance certificate of coverage (or verification that insurance can be obtained) to work in the State of Utah with the following minimum coverages: Workers Compensation Insurance: $1,000,000 General Liability Insurance: $3,000,000 utomobile Liability Insurance: $3,000,000 mount of contractors Excess Liability Insurance (if any). Bonding capability Provide a letter from your bonding agent or surety addressing the following: Name of Surety (surety must be licensed in the State of Utah, have an.m Best rating of X or better, and listed in the U.S. Treasury Circular 570 of acceptable sureties); Name and phone number of surety underwriter (not agent); List if any surety has ever paid a claim or finished a project for this entity; List largest bid bond issued for this entity in the last three years; List largest performance and payment bond issued for this entity in the last three years; List date of last performance and payment bond issued for this entity; and, list single project and aggregate bonding capacity of this entity. E. Fees / Rates for Services O NOT submit pricing, fees, rates, or bids as part of this pre-qualification stage. Bid documents are anticipated to be available for bidding by pre-qualified contractors between January 28th and February 11th. 1.5 Evaluation and Selection The City will evaluate and select firms to be pre-qualified to bid on the project on the basis of items such as demonstrated competence, appropriate licensing and insurance/bonding coverage, similar project experience, quality of references, demonstrated ability to perform, and professional qualifications. selection committee will be made up of City staff, the architect, and may include elected or appointed officials. Qualifications, Expertise and Responsiveness The City will review the assigned personnel s experience and past performance on similar projects. The City will also evaluate the firm s responsiveness to the client s needs during past projects. References The selection committee will contact references and evaluate their experiences with the firm s work product and responsiveness to their needs during and after the project.

4 P a g e 4 The City may make a final selection of pre-qualified firms directly from the submitted proposals or short list several firms for additional interviews. Interviews may be required at the discretion of the City. If interviews are required, they will be scheduled after review of the submissions and will be made at the respondent s expense. Selected firms will be notified by if they have been pre-qualified to submit bids for the project. Lindon City shall not be bound by any statements, representations, conclusions, or assumptions made by any party, whether oral or written, except for written statements that are issued in an ddendum to all prospective bidders. While it is the intent of the Lindon City to proceed with this pre-qualification selection, this solicitation does not obligate the City to enter into a contract with any respondent. Lindon City reserves the right to: 1. isqualify incomplete proposals; 2. Waive minor defects in the proposals; 3. Request additional information from respondents; 4. Change the nature or scope of the project, without penalty; 5. Reject any or all proposals, without penalty; and 6.
