3 HOMEOWNER POLICY GENERAL RULES ELIGIBILITY (FORM HO 00 01, HO 00 02, HO 00 03) (subject to Risk Selection Assistance) Owner-occupants of a dwelling. Purchaser-occupant(s) of a dwelling with a long term written contract. Occupant of dwelling under a life estate arrangement. Dwelling in course of construction for the intended owner-occupant(s). Used for private residential purposes only (except for certain permitted business occupancies). Not occupied by more than one additional family. Not more than two boarders or roomers per family. Seasonal dwelling. Secondary dwelling. Mobilehomes. Jointly owned 2 family owner-occupied dwelling, each owner occupying distinct living quarters with separate entrances. A homeowner policy providing building coverage may be issued to only one of the co-owner occupants of the dwelling. Name the other owner on the HO Additional Insured Endorsement to provide building and premises liability coverage for their interest. A separate HO may be issued to the co-owner occupying the other apartment in the dwelling to cover their contents and comprehensive personal liability. ADDITIONAL INSUREDS Extends the Homeowner s Policy, without an additional premium charge, to cover the interest of a non-occupant joint owner in the building and for premises liability. Full name and address as well as interest in the property must be provided. Use HO Additional Insured Residence Premises ELIGIBILITY (FORM HO 00 04) Tenant(s) (non-owner) of a dwelling or apartment. Used for residential purposes only. Not occupied by more than one additional family. Not more than two boarders or roomers. ELIGIBILITY (FORM HO 00 06) Owner(s) of a condominium or cooperative unit. 4 FINANCIAL HISTORY MEASUREMENT PROGRAM (INSURANCE SCORING): The Financial History Measurement Program is a Credit Based Insurance Score Program. This program will be used to underwrite and rate all new business homeowner submissions and existing homeowner renewals. The use of a credit or debit factor is another piece of the rating methodology that we use to determine a rate for a new or renewal homeowner policy. This CBIS factor is used in conjunction with other rating factors such as the distance to a fire hydrant or the existence of a fire protection system. An application for a new homeowner policy will not be declined, nor will an existing policy be cancelled or non-renewed, solely due to an adverse credit based score or absence of an insurance score. We will use a model that groups the insurance score into increments of 25 points. There is a category for submissions where the insurance credit report indicates No Score. This category will not be affected by any rating insurance score factor (i.e. 1.00). Patrons will use the actual factors for all the score ranges. If there are two named insureds, the highest insurance score will be used for rating purposes when the difference between the two scores is 50 or less. We will use the lower score if the difference between the scores is greater than 50. If one of the two scores is a "No Hit" or a "No Score", we will use the score factor from the score that produces the higher rate. All paper application submissions require this report be attached. New Business The Credit Based Insurance Score Program will require that an insurance score report be run at the point of sale. Patrons Online system will automatically order the insurance score for homeowner quotes and new business. If a quote is converted to a new business policy, Patrons On-line bridges the input information, including the insurance score. If a paper application is submitted for an on-line quote, the quote number should be indicated on the application. When using Silver Plume, you will have authorization to run the insurance score report directly through ChoicePoint with Patrons assuming the cost. Also, the score can be ordered directly through ChoicePoint. The insurance score report will be returned to you as a 3 byte encryption (example: PHA). A Credit Based Insurance Scoring notice must be given to the insured at the time of the application. This same notice will also be attached to all new business policies. The notice will advise the insured that their credit information will be accessed for underwriting purposes and explains the definition of an insurance score versus a credit score. It also advises the insured where they can request a copy of their Credit Report. To advise the insured of their rights, the notice Your Rights Under the Fair Credit Report Act and/or State Regulations will be attached to every new policy. A Notice of Adverse Action will be attached to any new business policy with an insurance score less than 625. Rev 06/15/17 Financial History Measurement Program 2

5 Renewals Effective 06/15/17 W e will order an insurance score for each named insured(s) on every existing renewal. This insurance score will apply to three policy renewal terms. Example: 6/15/17-18, 6/15/18-19, 6/15/ If there is more than one score for the policy, the highest score will be used when the difference between the two scores is 50 or less. We will use the lower score if the difference between the scores is greater than 50. If one of the two scores is a "No Hit" or a "No Score", we will use the score factor from the score that produces the higher rate. If we are unable to receive a credit insurance scoring report because of inaccurate information, a letter will be sent to the insured requesting 1) verification of the information currently on file and 2) their Social Security number. If the information is not received prior to the renewal processing, the policy will renew using the score factor that produces the higher rate. W hen the insurance score report used for rating is 625 or higher, the renewal policy will be processed based on the score with the appropriate credit rating factor. The Credit Insurance Scoring notice and the Your Rights Under the Fair Credit Report Act and/or State Regulations notice will be attached. If the insurance score report used for rating is 624 or lower, the renewal policy will be subject to a debit rating factor. On the first renewal, Your Rights Under the Fair Credit Reporting Act/Notice of Adverse Action will be attached to the policy. Renewal year two and three will have only the Your Rights Under Fair Credit Reporting Act attached. On year four, the system will order a new insurance credit score. The appropriate forms will be attached based on the score. The renewal ordering for new insurance credit scores will follow this three year cycle. Programming is in place to retrieve renewal scores. When we have an indication from our vendor that the account is frozen and no score is returned, the score from the prior year will be used. Credit Based Insurance Score Rating Factors The model s rating factors range from.75 to Insureds with a score of 800 and more will receive the best rate. The credit based insurance score factors shown below will be applied to the base premium. MINIMUM PREMIUM The minimum annual premium is $ TIER DEVIATIONS Superior (SUP).90 Preferred (PREF) 1.00 Standard (STD) 1.20 RESTRICTION OF INDIVIDUAL POLICIES If a policy would not be issued because of unusual circumstances or exposures, the named insured may request a restriction of the policy provided no reduction in the premium is allowed. Such requests shall be referred to the company. NON-SMOKER DISCOUNT 10% Credit applied to the base premium. All Homeowners forms and tiers. A signed PONS certificate or other (application) signed statement from insured is required. COASTAL ZONE A one to four family residential property located within two tenths of a mile (1056 feet) of the high tide water mark along the Maine Coast or on an inaccessessible coastal island is considered to be located in a defined Coastal Zone for underwriting, rating and the inclusion of the mandatory two percent Hurricane Deductible HO The location will be determined by the use of various programs that use longitude and latitude of the residential property location to determine the distance to the high tide water mark. An inaccessible coastal island is any coastal island that cannot be reached by a motor vehicle and requires the use of a boat or an aircraft to reach it. COASTAL SURCHARGE A. Effective 06/15/17, a Coastal Surcharge will be applied on renewal risks located in the Coastal Zone" with a distance from Maine high tide water mark as follows: Territory Distance Surcharge 30 and 31 (coastal) % Miles The surcharge will be applied to base premiums. LOSS FREE CREDIT (no paid losses in excess of $750) Applies to all pricing tiers. $750. threshold applies to each individual claim number. The agency has entered into an agreement with a managing officer of an affiliate group. An affiliate group is defined as an organization of 10 or more dues paying members controlled by a Board of Directors. A minimum enrollment is not required. The agent must indicate on the application the qualifying group for the 5% credit to apply. All lines of business are eligible for the 5% Group Credit. Applies to all pricing tiers. Applies to all coverages on the policy. Patrons optional bill plans are available. When choosing the EFT option, the insured will take advantage of the newly introduced 5% EFT Credit. This will result in a substantial savings to the insured. Existing Patrons insureds would be eligible for the 5% Group Credit if the agent of record has entered into an agreement with the insured s employer. However, if the existing Patrons insured is with another agent, the current policy would have to be cancelled and rewritten with the new agent of record to receive the 5% Group Credit. IDENTITY FRAUD EXPENSE COVERAGE Coverage Description When the optional Identity Fraud Expense Coverage endorsement is attached to the policy, $15,000 of coverage is available to pay for expenses incurred by an insured as a direct result of any one identity fraud first discovered or learned of during the policy period. Such expenses include the costs for notarizing fraud affidavits or similar documents; certified mail sent to law enforcement, financial institutions and credit agencies; lost income resulting from the time taken off work to meet with or talk to law enforcement or credit agencies; loan application fees for re-applying for a loan when the application is rejected solely because the lender received incorrect credit information; and reasonable attorney s fees incurred to defend lawsuits brought against the insured and to remove criminal or civil judgments. Limits of Liability Up to $15,000 coverage will be provided for the Identity fraud of an insured discovered or first learned of during the policy period. 15 HOMEOWNER POLICY cont. ISO ENDORSEMENTS WHEN MANUALLY RATING ISO ENDORSEMENTS, APPLY TIER DEVIATION. LIMITED FUNGI, WET OR DRY ROT, OR BACTERIA COVERAGE When the endorsement is attached to the policy, limited amounts of insurance are automatically provided as follows: Section I - $10,000 to pay for loss to covered real or personal property, owned by an insured, that is damaged by fungi or wet or dry rot, or bacteria on the residence premises as defined in the coverage endorsements. This Coverage applies only for the policy period in which the loss or costs occur. Section II - $50,000 to pay for damages because of bodily injury or property damage involving the inhalation of, ingestion of, contact with, exposure to, existence of, or presence of any fungi, wet or dry rot, or bacteria. Increased limits - Not available Application of Limits of Liability For Property Coverage, the $10,000 or the limit selected is the most coverage that will be provided during the policy period regardless of the number of locations insured for Limited Fungi, Wet Or Dry Rot, Or Bacteria Coverage or the number of claims made during the policy period. For Liability Coverage, $50,000 or the limit selected is an aggregate limit and is the most coverage that will be provided during the policy period regardless of the number of persons injured, the number of persons whose property is damaged, the number of insureds, the number of locations insured under this policy or the number of bodily injury or property damage claims made. Premium Basic Limits: There is no premium adjustment. Use Limited Fungi, Wet or Dry Rot, Or Bacteria Coverage Endorsement HO For all forms other than HO HO For Form HO HO For Form HO with HO and Form HO with HO or HO SPECIAL LOSS SETTLEMENT - ALL FORMS EXCEPT HO AND HO The policy provides loss settlement on a replacement cost basis if the Coverage A limit of liability represents at least 80% of replacement value. This percentage amount may be modified to 50%, 60% or 70% of replacement value, without affecting the loss settlement provisions. If this option is selected, the Coverage A limit of liability representing the percentage chosen of replacement value, is to be shown in the policy declarations. Functional Replacement Cost Loss Settlement - HO and HO Only The policy provides building loss settlement on a replacement cost basis, if, at the time of loss, the amount of insurance on the damaged building represents at least 80% of the full replacement cost of the building immediately before the loss. The policy may be endorsed to provide building loss settlement exclusively on a functional replacement cost basis if, at the time of loss, the amount of insurance on the damaged building is 80% or more of the functional replacement cost of the building immediately before the loss. Functional Replacement Cost means the amount which it would cost to repair or replace the damaged building with less costly common construction materials and methods which are functionally equivalent to obsolete, antique or custom construction materials and methods. Select the BASE PREMIUM from the rate pages for the amount of insurance selected for this option. Use Endorsement HO Functional Replacement Cost Loss Settlement. 2. Actual Cash Value Loss Settlement - Windstorm Or Hail Losses To Roof Surfacing - All Forms Except HO The policy provides settlement for building losses on a repair or replacement cost basis, subject to certain conditions. The policy may be endorsed to provide loss settlement exclusively on an Actual Cash Value basis for roof surfacing when damage is caused by the peril of Windstorm or Hail. To develop a premium for this option, multiply the BASE PREMIUM by a factor of.99. SPECIAL PERSONAL PROPERTY COVERAGE The policy may be endorsed to insure Coverage C against additional risks of physical loss subject to certain exclusions. -HO Preferred and Superior Tiers Rating: Apply factor 1.20 to the HO3 base premium. Use Endorsement HO Special Personal Property Coverage. When a condominium unit is owner occupied, the Unit Owners Coverage C Special Coverage Endorsement may be added. -HO Preferred and Superior Tiers Rating: Apply factor 1.40 to the HO6 base premium. Use Endorsement HO Unit Owners Coverage C Special Coverage PERSONAL PROPERTY (COVERAGE C) REPLACEMENT COST COVERAGE The premium to extend the limit of liability for Coverage C to include Replacement Cost Coverage, is computed by multiplying the BASE PREMIUM including any premium adjustment for Coverage C limits by the appropriate factor below. All Forms except HO and HO and PROTECTIVE DEVICES Use Endorsement HO Personal Property Replacement Cost. Approved and properly maintained installations of burglar alarms, fire alarms and automatic sprinklers in the dwelling may be recognized for a reduced premium. Multiply the BASE PREMIUM by the following factors: Central Station Reporting Burglar Alarm.95 Central Station Reporting Fire Alarm.95 Police Station Reporting Burglar Alarm.97 Fire Department Reporting Fire Alarm.97 Two Local Fire/Smoke Alarms.95 One Local Fire/Smoke Alarm and/or Local Burglar Alarm.98 Automatic Sprinklers in all areas including attics, bathrooms, closets, attached structures.90 Automatic Sprinklers in all areas except attic, bathroom, closet and attached structure areas that are protected by a fire detector.92 To obtain the Central Station Reporting credit, we require a copy of the Security System certificate. BUILDING ADDITIONS AND ALTERATIONS AT OTHER RESIDENCES The policy may be endorsed to provide this coverage to that part of a building which is rented to the insured as an other residence. Develop the premium per $1000 of insurance as noted below. Use Endorsement HO Building Additions and Alterations at Other Residence. BUILDING ADDITIONS AND ALTERATIONS - INCREASED LIMIT - HO The limit of liability of 10% of Coverage C provided under Additional Coverages in the HO may be increased. 20 BUSINESS PROPERTY - INCREASED LIMITS HOMEOWNER POLICY ISO ENDORSEMENTS cont. The $2,500 limit of liability for business property on the residence premises may be increased to $10,000 in increments of $2,500. The charge is $27 per $2,500. When the on-premises limit is increased, the off-premises limit of $250 is automatically increased, at no additional charge, to an amount that is 10% of the increased total limit of liability. Use Endorsement HO Increased Limits of Business Property. Note: The limit of liability in excess of $2,500 does not apply to: A. Business property in storage or held as a sample or for sale or delivery after sale. B. Business property pertaining to a business actually conducted on the residence premises. This exposure is addressed by HO Permitted Incidental Occupancies or refer to Patrons Oxford Home Business Endorsement PO H7. CREDIT CARD, FUND TRANSFER CARD, FORGERY AND COUNTERFEIT MONEY The limit of $500 may be increased. This coverage shall apply to all Section I Coverages for the same limits provided in the policy. For Form HO with HO and Form HO with HO 17 31, this peril is automatically included for Coverage C. DEDUCTIBLE - The base deductible is 5% of the limit of liability for each coverage and is subject to a $250 minimum. This deductible may be increased for a premium credit. This deductible applies separately to loss under the various Section I Property Coverages. If the limit of liability on certain property is increased by endorsement, the total limit of liability is used to determine the deductible. Include increased limits for Cov. C or D and the building coverage options (e.g. HO Other Structure increased limits and HO Cov A increased limits, Ordinance or Law Coverage or Building Additions and Alterations). If increased limits are not desired, enter $5,000 under Coverage A-Dwelling on the application. INCREASED LIMITS - The premium is developed based on the additional limit of insurance. The premium for each additional $1,000 of insurance is developed as follows: Multiply the HO Base Premium from the KEY PREMIUM CHART based on territory, protection and construction by.026. When HO (Ordinance or Law Coverage) is endorsed onto the policy add an additional $1 per $1,000 of coverage for this endorsement. SPECIAL COVERAGE - The Section I Perils Insured Against may be broadened to cover additional risks of loss with certain exclusions. Rating: $2 charge per policy for the $5,000 basic Coverage A for increased Coverage A add an additional $1 per $1,000 for this endorsement. Use Endorsement HO Unit Owners Coverage A Special Coverage. PERMITTED INCIDENTAL OCCUPANCIES - RESIDENCE PREMISES The policy may be endorsed to provide expanded Section I coverage and Section II coverage on a permitted incidental occupancy in the dwelling or in an other structure on the residence premises. Examples of such occupancies are Offices, Schools or Studios meaning offices for business or professional purposes and private schools or studios for music, dance, photography and other instructional purposes. If the permitted incidental occupancy is located in the dwelling, no additional charge is made for the structure. If the permitted incidental occupancy is located in an other structure, Coverage B does not apply to that structure. For specific insurance on that structure, charge $5 per $1,000. This endorsement may be attached to all Forms. PERSONAL PROPERTY Rating: $5.00 per $1,000 Use Endorsement HO Specific Structures Away From Residence Premises. INCREASED LIMIT - the limit of liability for Coverage C may be increased. Rating: charge $2 per $1,000 of increased limit or $3 per $1,000 if HO is endorsed on the policy. Patrons only charges for the increase over 70%. We give 70% of Coverage A at no charge. INCREASED LIMIT - OTHER RESIDENCES - Coverage for personal property usually located at other residences is limited in the policy form to 10% of Coverage C or $1,000, whichever is greater. This limit may be increased. Rating: charge $8.00 per $1,000 of increased limit Use Endorsement HO Increased Limits on Personal Property in Other Residences. REDUCTION IN LIMIT - The limit of liability for Coverage C may be reduced to an amount not less than 40% of the limit of Coverage A. The credit per $1,000 decrease is $1. Because we are increasing Coverage C from 50% to 70% at no extra charge, we will only return the credit on the decrease in increments of $1,000 from 40% to 50% of coverage A. Securities The $1,000 limit on securities may be increased to a maximum of $2,000. Rate: $4.00 per $100. D. Silverware, Goldware and Pewterware The special limit of liability of $2,500 for loss by theft of silverware, etc., may be increased to a maximum of $10,000 in increments of $500. Rate: $1.00 per $500. E. Firearms The special limit of liability of $2,000 for loss by theft of firearms may be increased to a maximum of $6,000 in increments of $100. Rate: $3.00 per $100. F. Electronic Apparatus The policy provides coverage, as described below, for loss of electronic apparatus equipped to be operated from the electrical system of a motor vehicle or motorized land conveyance while retaining its capability of being operated by other sources of power: (1) Up to $1,000 for apparatus in or upon a motor vehicle or motorized land conveyance, and (2) Up to $1,000 for apparatus not in or upon a motor vehicle that is away from the residence premises and used for business. Each of these limits may be increased to a maximum of $5,000 in increments of $500. Rate: $10.00 per $500. Use Endorsement HO Coverage C Increased Special Limits of Liability. Use Endorsement HO Coverage C Increased Special Limits of Liability for HO with HO or HO with HO REFRIGERATED PERSONAL PROPERTY - We add this endorsement to all Homeowner policies free of charge. This endorsement provides $500 coverage for covered property stored in freezers or refrigerators on the residence premises for loss caused by power service interruption or mechanical failure.
