1 SmartHome Essential Product Disclosure Sheet Important Note 1. Read this Product Disclosure Sheet before you decide to take out the SmartHome Essential Insurance Policy. Be sure to also read through the general terms and conditions. 2. You should satisfy yourself that this policy will best serve your needs. You should read and understand the insurance policy and discuss with the agent or contact the insurance company directly for more information. 3. Pursuant to Paragraph 5 of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance wholly for purposes unrelated to your trade, business or profession, you have a duty to take reasonable care not to make a misrepresentation in answering the questions in the Proposal Form (or when you apply for this insurance). You must answer the questions fully and accurately. Failure to take reasonable care in answering the questions may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. The above duty of disclosure shall continue until the time your contract of insurance is entered into, varied or renewed with us. In addition to answering the questions in the Proposal Form (or when you apply for this insurance), you are required to disclose any other matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us any of the information given in the Proposal Form (or when you applied for this insurance) is inaccurate or has changed. 1. What is this product about? This policy provides you with coverage for your building and contents and covers loss or damage by fire, lightning, or domestic gas explosion. 2. What are the covers / benefits provided? You may extend coverage to the following risks by paying additional premium: Aircraft and other aerial devices and/ or articles dropped therefrom Impact by a vehicle or animal excluding vehicles or animals belonging to or under the control of you or a member of your family Impact by a vehicle or animal including vehicles or animals belonging to or under the control of you or a member of your family Bursting or overflowing of domestic water tanks, apparatus or pipes Explosion Riot, Strike & Malicious Damage Earthquake & Volcanic Eruption Storm and Tempest Flood Subsidence/Landslip Falling Trees or Branches and Objects therefrom 3. How much premium do I have to pay? The total premium that you have to pay depends on the Construction Classification of your house and the additional perils that you opt: Main Cover Rates: Description Class 1A Class 1B Class 2 Detached Buildings 0.052% 0.064% 0.139% Non-detached and Semi-detached buildings 0.052% 0.064% 0.139% Flats and Apartments 0.052% 0.064% 0.139% Flats and Apartments (if shop and/or business of non hazardous trades do not exceed 20% on the total floor area) 0.057% 0.071% 0.114% 1 of 3

What are the fees and charges I have to pay? What you have to pay in addition to the premium Amount (a) Service tax (for policy transaction period before 1 st April 2015 and for organizational policies only) 6% (b) Goods & Services Tax (for policy transaction period after 1 st April 2015) 6% Stamp duty RM10.00 What is included in the premium Commission paid to the insurance intermediaries (if any) Amount 15% of premium 5. What are some of the key terms and conditions that I should be aware of? You must ensure that your property is insured at the appropriate amount taking into account the renovations made to your property You may select to insure your property on Market Value or Reinstatement Value basis o Market Value basis we will pay the cost of repairing the damage property less the amount for wear, tear and o depreciation Reinstatement value basis we will pay the full cost of repairing the damaged property without any deductions being made for wear, tear or depreciation, provided that the sum covered is adequate to cover the total cost of reinstating the property If the amount insured in your policy is less than the actual value at the time of loss (e.g. under-insurance), you are deemed to be self-insuring the difference. This average condition will apply in the event of a claim Excesses, being the amount you have to bear before we indemnify you, are applicable for certain perils, such as overflowing of domestic water tanks, windstorm, earthquake and flood claims 6. What are the major exclusions under this policy? This policy does not cover certain losses such as War, Civil War and any act of terrorism Radioactive and nuclear energy risks Date recognition Property damage to data or software The Liability to the Public section does not cover any asbestos related injury or damage involving the use, presence, existence, detection, removal, elimination or avoidance of asbestos or exposure or potential exposure to asbestos Please refer to the policy contract on the full list of exclusions under this policy. 7. Can I cancel my policy? You may cancel your policy by giving us a written notice. Upon cancellation, you are entitled to a refund of the premium based on short period rates* for the policy period which has been in force. Any minimum premium paid under this policy is not refundable. 4 SmartHome Essential Lembaran Pendedahan Produk Nota Penting 1. Sila baca Lembaran Pendedahan produk ini sebelum anda membuat keputusan untuk membeli Insurans SmartHome Essential. Pastikan juga anda telah membaca terma-terma dan syarat-syarat am. 2. Anda hendaklah membaca dan memahami polisi insurans dan berbincang dengan pengantara insurans anda atau hubungi terus kami untuk keterangan lanjut. 3. Menurut Perenggan 5, Jadual 9, Akta Perkhidmatan Kewangan 2013, jika anda memohon insurans ini untuk tujuan yang sama sekali tidak berkaitan dengan perdagangan, perniagaan atau profesyen anda, anda mempunyai tugas untuk berhati-hati secara munasabah untuk tidak melakukan salahnyata ketika menjawab soalan-soalan di dalam Borang Cadangan (atau apabila anda memohon untuk insurans ini). Anda mestilah menjawab soalan-soalan secara lengkap dan tepat. Kegagalan untuk berbuat demikian, boleh mengakibatkan pengetepian kontrak insurans anda, penolakan atau pengurangan tuntutan anda, perubahan terma-terma atau penamatan kontrak insurans anda. Tugas pendedahan di atas hendaklah berterusan sehingga kontrak insurans anda dimasuki, diubah atau diperbaharui dengan kami. Sebagai tambahan bagi menjawab soalan-soalan di dalam Borang Cadangan (atau ketika anda memohon insurans ini), anda dikehendaki untuk mendedahkan hal-hal lain yang anda tahu relevan kepada keputusan kami untuk menerima risiko-risiko dan menentukan kadar dan terma-terma yang terpakai. Anda juga mempunyai tugas untuk memaklumkan kepada kami dengan segera jika pada bila-bila masa selepas kontrak insurans anda dimasuki, diubahsuai atau diperbaharui dengan kami, apa-apa maklumat yang diberikan di dalam Borang Cadangan (atau apabila anda telah memohon untuk insurans ini) tidak tepat atau telah berubah. 1. Apakah produk ini? Polisi ini memberi perlindungan kepada rumah dan isi rumah anda sekiranya berlaku kebakaran, kilat atau letupan. 2. Apakah perlindungan / manfaat yang disediakan? Anda boleh melanjutkan perlindungan ke atas risiko berikut dengan membayar premium tambahan : Kerosakan oleh pesawat atau peranti udara yang lain dan/atau benda yang gugur daripadanya; Kerosakan disebabkan oleh hentaman kenderaan atau binatang tidak termasuk kenderaan atau binatang kepunyaan atau di bawah kawalan anda atau ahli keluarga anda Kerosakan disebabkan oleh hentaman kenderaan atau binatang di bawah kawalan anda atau ahli keluarga anda Letupan atau limpahan radas tangki air atau paip; Letupan; Kerosakan oleh rusuhan mogok dan niat jahat; Gempabumi dan letusan gunung berapi Kerosakan disebabkan oleh ribut dan taufan Kerosakan disebabkan oleh banjir Penenggelaman dan gelinciran tanah Kerosakan oleh pokok atau dahan tumbang dan objek darinya 3. Berapakah premium yang saya perlu bayar? Jumlah premium berbayar adalah bergantung kepada Pembangunan kelas bagi rumah yang diinsuranskan dan jenis peril tambahan yang diperlukan. 1 of 3

Apakah yuran dan bayaran yang saya perlu bayar? Apakah yang anda perlu bayar sebagai tambahan kepada premium (a) Cukai Perkhidmatan (untuk transaksi polisi bertarikh sebelum 1 April 2015); atau (b)cukai Barang atau Perkhidmatan (untuk transaksi polisi bertarikh selepas 1 April 2015) Duti Setem Apa yang dimasukkan ke dalam premium Komisen dibayar kepada pengantara insurans (jika ada) Jumlah 6% daripada premium 6% daripada premium RM10.00 Jumlah 15% daripada premium 5. Apakah terma dan syarat utama yang saya harus ketahui? Anda harus memastikan harta anda diinsuranskan pada amaun yang berpatutan dengan mengambil kira pengubahsuaian yang dilakukan pada harta tersebut. Anda boleh menginsuranskan harta anda berasaskan dasar Nilai Pasaran atau Nilai Pengembalian Semula o Dasar Nilai Pasaran pihak kami akan membayar kos pembaikan ke atas harta yang rugi atau rosak setelah o ditolak amaun susut nilai. Dasar Nilai Pengembalian Semula pihak kami akan membayar kos penuh untuk pembaikan harta yang rosak tanpa menolak susut nilai, dengan syarat jumlah perlindungan adalah mencukupi untuk menampung jumlah kos pengembalian semula harta tersebut Sekiranya jumlah diinsuranskan dalam polisi anda adalah kurang daripada nilai sebenar pada waktu kerugian berlaku (contohnya insurans terkurang), anda dianggap harus menanggung sendiri terhadap perbezaannya. Syarat purata akan dikenakan jika berlakunya tuntutan. Lebihan, sebagai amaun yang harus anda tanggung sendiri sebelum kami membayar balik kepada anda, adalah dikenakan untuk peril tertentu, seperti tuntutan terhadap tangki air domestik melimpah, ribut angin, gempa bumi dan banjir. 6. Apakah pengecualian utama di bawah polisi ini? Polisi ini tidak melindungi kerugian tertentu seperti: Perang, perang saudara dan mana-mana tindakan Keganasan Risiko radioaktif dan tenaga nuklear Pengenalpastian tarikh Kerosakan harta kepada data atau perisian Seksyen Liabiliti Terhadap Awam tidak melindungi apa-apa kecederaan yang dianggap berkaitan dengan asbestos atau kerosakan yang melibatkan penggunaan, kehadiran, kewujudan, pengesanan, pengalihan, pembuangan atau penghindaran asbestos atau terdedah atau potensi terdedah kepada asbestos. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk sampel kontrak polisi untuk senarai penuh bagi pengecualian di bawah polisi ini. 7. Bolehkah saya batalkan polisi ini? Anda boleh membatalkan polisi ini dengan memberi notis bertulis kepada pihak kami. Selepas pembatalan, anda layak mendapat pemulangan premium ditolak premium pada kadar tempoh singkat* bagi tempoh polisi yang telah berkuatkuasa, tertakluk kepada premium minimum yang ditahan oleh pihak kami. Tiada pemulangan premium dibenarkan jika timbulnya tuntutan di bawah polisi ini. 2 of 3
