CIMB Auto Relief is a comprehensive Personal Accident Insurance policy. Besides Accidental Death/Permanent Disablement and Medical Expenses benefits, it also provides you and your passengers with a 24-Hour Car Assistance Program and a host of additional benefits such as Car Replacement, Compassionate Cover, Burglary/Breakin/Robbery and Flood Cover. 2. What are the covers/benefits provided? This policy covers: (a) Personal Accident Benefits Accidental Death/Permanent Disablement Medical Expenses (b) Car Assistance Program 24-Hour Emergency Towing And Minor Roadside Repairs Taxi Assistance Arrangement for Car Rental and Hotel Accommodation International Assistance (Singapore and Thailand) Arrangement for Emergency Evacuation Referral to Service Centre Emergency Message Transmission (c) Additional Benefits Car Replacement for Peninsular Compassionate Cover (Subject to the Named Vehicle being insured under a Private Car Comprehensive Motor Policy, Third Party Fire and Theft Motor Policy) Optional Car Replacement Burglary/Break-in/Robbery Flood Cover (d) Auto Loan Protection (this benefit is only available to individual policy owners and NOT company) Duration of cover is for one (1) year. You need to renew your insurance cover annually. Note: Please refer to the full feature and benefits and scale of benefits in the policy contract. 3. How much premium do I have to pay? The total premium that you have to pay may vary depending on the plan purchased. 2 5. What are some of the key terms and conditions that I should be aware of? (a) Duty of Disclosure Consumer Insurance Contract Pursuant to Paragraph 5 of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance wholly for purposes unrelated to your trade, business or profession, you have a duty to take reasonable care not to make a misrepresentation in answering the questions in the Proposal Form and/or all the questions required by Allianz fully and accurately and disclose any other matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied, otherwise it may result in avoidance of contract, claim denied or reduced, terms changed or varied, or contract terminated. Non-consumer Insurance Contract Pursuant to Paragraph 4(1) of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance for purposes related to your trade, business or profession, you have a duty to disclose any matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied and any matter a reasonable person in the circumstances could be expected to know to be relevant, otherwise it may result in avoidance of contract, claim denied or reduced, terms changed or varied, or contract terminated. This Duty of Disclosure for Consumer and Non-consumer Insurance Contract shall continue until the time the contract is entered into, varied or renewed. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us, any of the information given in inaccurate or has changed. (b) Cash before cover The premium due must be paid and received by Allianz prior to the commencement of coverage (c) Eligibility Insured Person aged between seventeen (17) years old to eighty (80) years old (d) This policy shall lapse/terminate when the Insured attains the age of eighty (80) years old (e) The Insured shall within fourteen (14) days after the happening of the accident give notice to Allianz with full particulars of the accident and injuries (f) The Insured shall give immediate notice to Allianz of any change in his/her name, residence, business or occupation 6. What are the major exclusions under this policy? (a) Loss occasioned by war, invasion, act of foreign enemy, hostilities or war like operations (whether war be declared or not), mutiny, civil war, revolution, insurrection, conspiracy, military or usurped power or martial law (b) Pre-existing illness (c) While the Named Vehicle is used for hire, racing, pacemaking, speed-testing or for any purpose in connection of with motor trade (d) Insured/driver does not hold a valid driving licence to drive the Named Vehicle or is not qualified for holding or obtaining a valid driver s licence under the regulations of the Malaysian Road Transport Department or Court of Law (e) Suicide or any attempt thereat (sane or insane) (f) No valid road tax displayed on the Named Vehicle (g) While the Named Vehicle is used for illegal business pursuit as an unlicensed common carrier (h) Childbirth or miscarriage or insanity unless caused solely and directly by accidental means to the Insured/rider and/or pillion rider while, riding, alighting from or boarding the Named Vehicle. Note: This list is non-exhaustive. Please refer to the policy for the full list of exclusions under this policy. 7. Can I cancel my policy? You may terminate your policy by giving written notice to Allianz. Termination shall become effective on the date the notice is received or on the date specified in such notice whichever is the later. Refund of premium, however, is subject to the terms and conditions stipulated in the Policy. 8. What do I need to do if there are changes to my contact/personal details? It is important that you inform us of any changes in your contact and personal details to ensure that all correspondence reach you timely manner. You may inform our authorised agent, branch office or our customer service. 9. Where can I get further information? CIMB Auto Relief adalah polisi insurans Kemalangan Diri yang komprehensif. Selain melindungi anda dan para penumpang dengan faedah-faedah Kemalangan Diri/Hilang Upaya Kekal dan Perbelanjaan Perubatan, ia juga menyediakan Program Bantuan Kereta 24-Jam. CIMB Auto Relief juga mempunyai faedah tambahan seperti Penggantian Kereta, Perlindungan Belas Ehsan, Kecurian/Kejadian Pecah Masuk/Rompakan dan Perlindungan Banjir. 2. Apakah perlindungan/faedah-faedah yang disediakan? Polisi ini melindungi: (a) Faedah-faedah Kemalangan Diri Kematian/Hilang Upaya Kekal Perbelanjaan Perubatan (b) Program Bantuan Kereta Khidmat Tunda Kecemasan 24-Jam dan Pembaikan Kerosakan Kecil Di Tepi Jalan Bantuan Teksi dan Kereta Sewa Mengaturkan Penginapan Hotel Bantuan Antarabangsa (Singapura dan Thailand) Mengaturkan Evakuasi Kecemasan Rujukan Pusat Servis Penghantaran Mesej Kecemasan (c) Faedah-Faedah Tambahan Penggantian Kereta bagi Semenanjung Malaysia Perlindungan Belas Ehsan (Tertakluk kepada Kenderaan Dinamakan di bawah Polisi Motor Komprehensif Kereta Persendirian, Perlindungan Pihak Ketiga dan Perlindungan Pihak Ketiga, Kebakaran dan Kecurian) Pelan Pilihan Penggantian Kereta Perlindungan terhadap Kecurian/Kejadian Pecah Masuk/Rompakan Perlindungan Banjir (d) Perlindungan Pinjaman Kenderaan (faedah ini hanya boleh digunapakai untuk polisi individu dan BUKAN polisi syarikat) Tempoh perlindungan adalah untuk satu (1) tahun. Berapakah premium yang saya perlu bayar? 5 5. Apakah terma dan syarat utama yang saya harus ketahui? (a) Kepentingan pendedahan Kontrak Insurans Pengguna Menurut Perenggan 5 Jadual 9 di bawah Akta Perkhidmatan Kewangan 2013, sekiranya anda memohon Insurans ini sepenuhnya bagi tujuan yang tidak berkaitan dengan perdagangan, perniagaan atau profesion anda, anda mempunyai kewajipan mengambil penjagaan munasabah supaya tidak membuat salah nyataan semasa menjawab soalan-soalan di dalam Borang Cadangan dan/atau semua soalan-soalan yang dikehendaki oleh Allianz dengan penuh dan tepat dan mendedahkan apa-apa perkara lain yang anda ketahui berkaitan dengan keputusan kami dalam menerima risiko dan dalam menentukan kadar dan terma yang hendak dipakai, jika sebaliknya ini boleh menyebabkan kontrak terbatal, penolakan atau pengurangan tuntutan, penukaran terma atau penamatan kontrak. (a) Kontrak Insurans Bukan Pengguna Menurut Perenggan 4(1) Jadual 9 di bawah Akta Perkhidmatan Kewangan 2013, sekiranya anda memohon Insurans bagi tujuan berkaitan dengan perdagangan, perniagaan atau profesion anda, anda mempunyai kewajipan untuk mendedahkan apa-apa perkara yang anda ketahui sebagai berkaitan dengan keputusan kami dalam menerima risiko dan dalam menentukan kadar dan terma yang hendak dipakai dan apa-apa perkara yang seorang yang munasabah dalam hal keadaan itu boleh dijangka untuk tahu sebagai berkaitan, jika sebaliknya ini boleh menyebabkan kontrak terbatal, penolakan atau pengurangan tuntutan, penukaran terma atau penamatan kontrak. Kewajipan pendedahan ini bagi Kontrak Insurans Pengguna dan Bukan Pengguna akan berterusan sehingga masa kontrak tersebut dibuat, diubah atau diperbaharui. Anda juga mempunyai kewajipan memaklumkan kepada kami dengan segera sekiranya pada bila-bila masa, selepas kontrak insurans di buat, diubah atau diperbaharui dengan pihak kami, sebarang maklumat yang diberikan adalah tidak tepat atau telah berubah. (b) Tunai Sebelum Perlindungan Premium mesti dibayar dan diterima oleh Allianz sebelum perlindungan bermula. (c) Kelayakan Orang Yang Diinsuranskan yang berumur antara tujuh belas (17) sehingga lapan puluh (80) tahun (d) Polisi akan luput/terbatal apabila Orang Yang Diinsuranskan menjangkau usia lapan puluh (80) tahun (e) Orang Yang Diinsuranskan hendaklah mengemukakan notis kepada Allianz beserta butiran lengkap kemalangan dan kecederaan dalam masa empat belas (14) hari selepas berlakunya kemalangan (f) Orang Yang Diinsuranskan harus memberikan notis serta merta kepada Allianz berkenaan apa sahaja pertukaran pada nama, tempat tinggal, perniagaan atau pekerjaan 6. Apakah pengecualian utama di bawah polisi ini? (a) Kerugian disebabkan oleh peperangan, pencerobohan, tindakan musuh asing, permusuhan atau kegiatankegiatan seperti perang (sama ada perang diishtiharkan atau tidak), dahagi, perang saudara, revolusi, kebangkitan, konspirasi, undang-undang ketenteraan (b) Penyakit Sedia Ada (c) Ketika Kenderaan yang Dinamakan itu digunakan untuk disewa, perlumbaan, pacemaking, ujian kelajuan atau untuk sebarang tujuan yang berkaitan dengan perdagangan motor (d) Pihak diinsuranskan/ penumpang tidak memiliki lesen memandu yang sah untuk memandu Kenderaan Dinamakan atau tidak memenuhi syarat untuk memegang atau mendapatkan lesen memandu di bawah Peraturan-peraturan Jabatan Pengangkutan Jalan Raya Malaysia atau Mahkamah Undang-undang (e) Membunuh diri atau percubaan untuk melakukannya (siuman atau tidak siuman) (f) Cukai jalan yang sah tidak dipaparkan diatas Kenderaan Dinamakan (g) Ketika Kenderaan Dinamakan digunakan sebagai pengangkutan awam tanpa lesen dalam sesuatu perniagaan haram (h) Kelahirkan atau keguguran atau tidak siuman kecuali disebabkan semata-mata dan secara langsung oleh punca-punca kemalangan ke atas Pemandu dan/atau penumpang ketika sedang memandu, turun daripada atau menaiki Kenderaan Dinamakan Nota: Senarai ini bukan senarai yang lengkap. Sila rujuk kepada polisi untuk senarai penuh bagi pengecualian di bawah polisi ini. 7. Bolehkah saya batalkan polisi ini? Anda berhak membatalkan polisi ini dengan memberi notis bertulis kepada Allianz. Pembatalan tersebut akan berkuat kuasa pada tarikh notis itu diterima atau pada tarikh yang dinyatakan di dalam notis, yang mana-mana lebih terdahulu. Setelah pembatalan, sebarang pulangan bayaran premium adalah berdasarkan kepada syarat-syarat yang dinyatakan di dalam kontrak polisi. 8. Apakah yang harus saya lakukan jika terdapat perubahan dalam butiran peribadi saya? Adalah penting bagi anda memaklumkan kami terhadap sebarang perubahan di dalam perincian maklumat untuk menghubungi anda untuk memastikan yang semua urusan surat-menyurat sampai ke tangan anda tepat pada masanya.
