3 4 Sign up for the right healthcare plan option. Make sure you enroll in the best option for you for Healthcare needs change over time, and what worked for you this past year may not be the best option for you next year. Use the ALEX tool to walk through different scenarios and cost outcomes of the plan options. ALEX can also help you decide if you want to enroll in an HSA. See pg. 10. ways to get the most out of your benefits When you re sick or injured, choose the right place to get help. If it s a life-threatening situation (or you think it might be), ALWAYS call 911 or go straight to the ER. But, for other illnesses or injuries, know your options: It s always best to call your doctor s office first to see if they can see you, since they know your medical history. If that s not an option, visit an urgent care center, which offers walk-in appointments and extended clinic hours. UHC participants can also do a virtual visit: See and talk with a doctor through your computer or mobile device for conditions like the flu, a cold, pink eye, rashes, etc. Benefit coverage amounts are the same as a regular office visit and apply toward your deductible, and you ll pay at the time of the visit using a credit or debit card. To find a virtual visit provider, log in to myuhc.com or Castlight (mycastlight.com/caterpillar). 4 Confirm your healthcare coverage before using it. Health insurance can be confusing, and it s never fun to get an unexpected bill in the mail. Before you get treatment, have a procedure, etc., call UHC ( ) or BCBS ( ) to find out what s covered. Sometimes a procedure will be covered, but only after you meet certain criteria or take other steps beforehand (prior authorization). Your doctor may not always know what your insurance plan requires, so it s best to check for yourself to avoid unhappy surprises. Compare prices of healthcare services (mycastlight.com/caterpillar). When you buy groceries, you can compare prices and know how much you re spending before getting to the cash register. Did you know you can also shop around for healthcare using the Castlight tool? Before seeing a doctor or specialist or getting a procedure or labs done, use Castlight to see how much you ll pay at different providers (yes, doctors and healthcare facilities often charge different prices for the same services!). 4 Healthcare Words to Know Premium: The amount you pay for healthcare coverage. Annual deductible: The amount you must pay before the plan begins paying any co-insurance for covered healthcare services (except for certain preventive services). Co-insurance: After you pay your deductible, this is the amount (usually a percentage) you pay for covered healthcare services until you reach your plan s maximum out-of-pocket limit. Co-pay: On the BCBS National EPO, this is the fixed amount you pay for office visits. You pay the co-pay whether or not you ve met your deductible. The co-pay does not count toward your deductible, but it DOES count toward your maximum out-of-pocket limit. Maximum out-of-pocket limit: The most you will pay for covered healthcare services in a plan year. Once you reach the maximum out-of-pocket limit, the plan pays 100% of your covered healthcare services for the rest of the year. For the CDHP options, this amount includes prescription drug costs. For the traditional plan options, prescription drug costs do not count toward the plan s maximum out-of-pocket limit. Specialty medications: Certain high-cost prescriptions that treat complex conditions and require special handling. They may be injected, inhaled, applied topically or taken orally. They may be self-administered or given in a doctor s office. The network pharmacy for specialty meds is Magellan Rx. The specialty products list is on benefits.cat.com in the Rx section. Prior authorization: It means a set of criteria must be met for your procedure or medication to be covered by the plan. Contact UnitedHealthcare ( ), Blue Cross Blue Shield ( ) or OptumRx ( ) for details on prior authorizations. Qualified status change: A life event, such as marriage, divorce, birth/adoption of a child, etc. that allows you a 31-day window (outside of annual enrollment) to make changes to certain healthcare benefits. On all plan options, you pay the full negotiated rate for ambulance and ER services until you meet the annual deductible, then you pay co-insurance. For an ER visit, you also pay an extra $125 fee (BCBS EPO) or $100 fee (Choice Plus PPO), which is waived if you re admitted. (No extra fees on the CDHP options.) 6 Plan Option Differences Coverage Differences Between UnitedHealthcare and Blue Cross Blue Shield Options The BCBS National EPO option and the UHC-administered options provide similar coverage for most healthcare services. But, there are a few areas where they re different:»»»» Chiropractic care Autism therapies Gender reassignment Fertility treatments If any of these services are important to you, you may want to contact BCBS ( ) and UHC ( ) for details before choosing your plan option during annual enrollment. How maximum out-of-pocket limits work BCBS National (EPO) Traditional plan option example: CDHP option example: Individual MOOP limit = $3,000 Michael and his 3 children are enrolled in the UHC Choice Plus PPO option. Michael has surgery, and his expenses meet the individual MOOP limit of $3,000. After that, the plan pays 100% of his covered healthcare costs, except for prescriptions. Family MOOP limit = $6,000 Two of his kids also end up having some major health expenses, so Michael and his children reach the plan s family MOOP limit of $6,000. At that time, the plan begins paying 100% of the covered healthcare costs of all 3 kids. But, Michael must still pay the prescription co-pays/ co-insurance for himself and his children until they meet the federal MOOP limit ($14,700 for family). Mia, her spouse and her son are enrolled in the UHC Consumer Max CDHP option: Mia s son develops a serious illness and has to be hospitalized. His expenses meet the individual MOOP limit of $5,000, but, because he s enrolled in family coverage in a CDHP, the family MOOP limit or the individual federal MOOP limit must be met before the plan begins paying 100% of his covered healthcare costs. Individual federal MOOP limit = Family MOOP limit = $10,000 $7,350 His expenses do end up reaching the individual federal MOOP limit of $7,350. At that time, the plan begins paying 100% of his covered healthcare costs, including his prescription costs, since he s enrolled in a CDHP. Mia is on some expensive medications, and her spouse also has some health expenses. Together they eventually reach the plan s family MOOP limit ($10,000). After that, the plan also begins paying 100% of both of their covered healthcare costs (including prescriptions) Benefits Annual Enrollment Guide

7 Prescription Drug Benefits OptumRx (formerly Catamaran) mycatamaranrx.com Our prescription drug benefit follows the Caterpillar Drug Formulary (list of covered medications). If a medication isn t listed on the formulary, it s not covered (ask your doctor for another option). CPRxN = Community Pharmacy Prescription Drug Network Network pharmacy for specialty medications is Magellan Rx. If you re enrolled in one of the CDHP options:» You must meet your deductible before the Caterpillar Drug formulary tier co-pay/co-insurance amounts apply. That means you ll pay the full negotiated retail prices (at network pharmacies) until then.» HOWEVER, certain preventive medications are covered before you meet your deductible. If your prescription is listed on the CDHP Preventive Drug List, you ll pay the Caterpillar Drug Formulary tier co-pay/co-insurance amounts, whether or not you ve met your deductible. The benefit is based on a tier system: the lower the tier your medication is in, the less you pay. Co-pay vs. co-insurance: Co-pay = flat dollar amount paid for a covered prescription medication Co-insurance = percentage paid for a covered prescription medication QUICK TIP To save money, ask your doctor if a generic version would work for you. Most Tier 0 and Tier 1 medications are generic versions. Tiers 2, 3 and 4 are mostly brand medications. 8 Health Savings Account (Hello tax savings!) Health Savings Account (HSA)* Which plan options can be paired with an HSA? What can I use this account for? What s the maximum amount that can be contributed to this account per year? How much money does the company contribute? When are the funds available? What happens if I don t use all the money during the year? Who is the account vendor? UHC Consumer Choice CDHP UHC Consumer Max CDHP Eligible healthcare expenses, including medical, dental, vision, hearing and prescription drugs, for you and your tax dependents $3,450 (including company seed) for retiree-only or spouse-only coverage $6,900 (including company seed) for retiree + spouse, child(ren) or family coverage During the year you turn age 55 and every year after, you can contribute an additional $1,000 (If your spouse has his/her own HSA, your combined contribution totals are subject to the family contribution limit.) You can deduct any contributions you make to your HSA on your annual tax return. Retiree-only or spouse-only coverage: Retiree + spouse, child(ren) or family coverage: $250 Consumer Choice $500 Consumer Choice $500 Consumer Max $1,000 Consumer Max The entire company seed amount is available in January. Your money stays in the account and can earn tax-free interest. Once you reach $1,000, you can invest it in mutual funds, if desired. There are specific legal requirements to be eligible to open and contribute to an HSA. The Caterpillar healthcare plan uses Epic Hearing Healthcare for hearing aid services. How to best use your benefits: Create an account at VSP.com (if you haven t already) to review your coverage details and claims. You can also sign up for notifications about special offers, coupons, etc. Use a VSP network doctor. To find one, visit VSP.com or call VSP. You won t get an ID card. At your appointment, just tell them you have VSP. (If you d like an ID card, you can print one at VSP.com.) For the lowest prices on frames, use a VSP network doctor or purchase directly through VSP.com. QUICK TIP You don t have to use a Cigna network dentist, but you ll usually pay less if you do. 11 Annual Enrollment Checklist Read through this booklet and review the info available on benefits.cat.com. Think about how much you spent on healthcare this year and what your needs may be next year. Use the ALEX tool to explore your plan options and cost scenarios. (See pg. 10.) During the annual enrollment window (Nov. 6-24, 2017), enroll in your healthcare benefits for 2018 through the Caterpillar Benefits Center at Alight. If you re planning to continue participating in a CDHP option for 2018 and want to receive the company seed in your health savings account, you ll need to reconfirm your HSA eligibility. If you enroll a new dependent, be prepared to provide supporting documentation to verify their eligibility (such as a birth certificate or marriage certificate). If you add a spouse/same-sex domestic partner, you ll also need to complete the spousal surcharge affidavit.
