Adult Control A. Adulticiding in Authorized Spray Zones: 1. Truck ULV treatments to be conducted in. 2. Route mileage must me a minimum of 85 route miles to schedule treatment. If such minimum is not met, Clarke will invoice the customer for the minimum amount of route miles. Treatments will be completed using Biomist or synthetic pyrethroid insecticide at $40.79 per route mile. Total cost for 85 route mile treatment area is $3, In the event that Duet is requested by the County for treatment, the same minimums will apply as stated above with a cost of $67.00 per route mile. Total cost for 85 route mile treatment area is $5, Mosquito Surveillance will continue to be performed by using either CDC light traps or BG Sentinel traps. B. Adulticiding Operational Procedures 1. Notification of community contact. 2. Weather limit monitoring and compliance. 3. ULV particle size evaluation. 4. 2 Part III SITE Guard Service for Container Breeder Mosquitoes OVERVIEW SITE Guard Services have been created to provide communities and mosquito control programs options for controlling container breeder mosquitoes that can vector Zika virus. It is designed to be used as a flexible, supplemental service to an existing program. Zika human case response support services are outlined and priced separately from all other available services. Protocols in line with International Best Practices. Chemicals used in service and/or additional services requested will be billed at the rates below. SITE GUARD ZIKA RESPONSE SERVICE Customer Responsibilities Swift, effective response to a local suspected or confirmed human case of Zika requires preparedness. The following are customer responsibilities. 1. Have established communication with your state health department and understand their protocol to notify you in the event human case of zika. 2. Understand the HIPPA laws and how to handle information provided to you for response. 3. Know what level of GIS case location detail will be provided to you by the state (or county) 4. Have a plan for how resident consent will be obtained, authorizing mosquito control inspection services to be performed on their property. 5. Have a plan on how to communicate the need to inspect / treat a specific area 6. Have a plan on how to address inaccessible properties, whether consent has not been obtained OR where team is unable to enter yard at the desired time. Clarke SITE Guard Response Service Service will consist of two-person teams placed in your community within 48 hours of notification. They will provide: A. Inspection of private properties, going door-to-door properties within a minimum of 1/8 mile radius of the reported human case. B. BG Sentinel Traps will be set IF there is no standing knowledge of vector presence: Aedes aegypti or Aedes albopictus C. Backyard ULV treatment with Duet dual-action Adulticide when Adult mosquitoes are present. D. Barrier treatment will be made to knock the population down and help minimize bites to inspection teams. Flit 13.3 EC (mixed at 1:24 dilution 0.5% emulsion) per gallon applied. E. Source reduction will be done, emptying containers holding water on properties within the defined human case zone (HCZ). F. Larvicide treatments as appropriate for containers or bodies of water that will remain, using with environmentally responsible residual products, such as, Vectobac WDG; and Natular DT, G30, T30, XRT formulations.(per price schedule) G. Pupaecide treatments will be made as appropriate to containers or bodies of water that will remain and which have pupae, using CocoBear Mosquito Larvicidal Oil. Page 2 of 7

Door-to-door distribution of Natular DT Consumer Packs to residents with educational and product use material. BG-Sentinel Trapping. Trapping adult mosquito with BG- Sentinel Trap(s) to confirm presence of adult Aedes aegypti and Aedes albopictus mosquitoes. The service includes surveillance reports of mosquitoes trapped. ABC Miniature Light Trapping. Trapping adult mosquito with ABC Light Traps to understand makeup of mix of mosquito species in area. Operational Mapping. Development of surveillance, larval and adult control maps for the implementation of community program services. 4 Larval Control Adult Control community program is determined and calculated by the map units (number of sites, acres, catch basins, spray miles, backpack barrier miles, etc.). Larval Site Inspection. Inspection of potential larval sites outside the designated treatment area. Larval samples will be taken in each site and include the following services: Identified to species Number of larvae per dip recorded Larval site $80.24 per site Prius computer system and hand-held PDAs will be utilized to record larval site inspection activities. A post project inspection report will be prepared for the community. Hand or Backpack Equipment Liquid Larviciding. Larviciding with hand or power backpack equipment with Vectobac WDG at the rate of 0.5 pounds per acre of habitats 0.25 Acre $24.17 found developing larvae during inspection for residual control. Wide Area Truck Turbine Spraying. Truck mounted Buffalo Turbine spraying of a community spray route with VectoBac WDG at the rate of 0.5 pounds per acre. GPS mile $ technology records operational coverage and provides postmission reports for documentation. Larviciding with Natular G30 applied per 1/10 of pound 0.10 lb $1.50 Hand Larviciding with Natular DT (each tablet applied) tablet $0.80 Hand Larviciding with Coco Bear (each oz. applied) ounce $0.50 Catch Basins with Natular T30 (per tablet applied) tablet $1.50 Catch Basins with Natular XRT (per tablet applied) tablet $4.24 Wide Area Truck ULV Spraying. Duet handheld/backpack ULV spraying per ¼ acre house/site applied at 1.28 oz./acre (high ¼ acre site $0.95 rate). Backpack Residual Barrier Spraying. Reporting utilizing Clarke proprietary Prius data capture system 1. Handheld technology for field data acquisition 2. Operational reporting on a daily, weekly, monthly and annual basis 3. Record keeping database 4. NPDES reporting I. Comprehensive insurance coverage of $25 million, naming your organization as additionally insured. J. Trained / Licensed staff K. Regulatory compliance for professional services performed in accordance with state statutes for mosquito control / public health, and local and federal levels. L. Product and process quality control monitoring for all services M. Risk-assessment operational map using key indicators (demographics, ports of entry, etc.) **NPDES Permit: A National Pollutant Discharge Elimination System (NPDES) permit is necessary for the execution of the work for mosquito control effective October 31, Any additional costs associated with activities and/or services that may be required by Clarke in order to comply with an NPDES permit are not included in this proposal. 6 Clarke Environmental Mosquito Management, Inc. Client Agreement Authorization for 2017 Environmental Mosquito Management (EMM) Program I. TERM AND TERMINATION: This Agreement has an automatic Renewal Clause. The term of the Agreement shall commence on the date when both parties have executed this Agreement and shall continue for a period ending on December 31, 2017 (the Initial Term ). Unless either party hereto provides the other party with written notice at least Ninety (90) days prior to the end of the Initial Term or any subsequent renewal term, this Agreement shall automatically continue to renew for additional term, each term having duration equal to the Initial Agreement. If a party hereto fails to comply with a provision of this Agreement, then the other party shall have the right to terminate this Agreement if it gives written notice of the default to the defaulting party and the defaulting party fails to cure the default within sixty days of receipt of said notice. II. Price Increases: The price for the services rendered hereunder may be increased by Clarke Environmental Mosquito Management, Inc. ( Clarke ) on the first day of any Calendar year commencing on January 1, 2018 (a Price Increase Date ) by a percentage which shall not exceed the greater of the percentage increase of the consumer price index during the calendar year which immediately precedes the Price Increase Date or five percent (5%) ( Price Increase Percentage ). Clarke Environmental Mosquito Management, Inc. may petition the Customer at any time for an additional rate adjustment on the basis of extraordinary and unusual changes in the cost of operations that could not be reasonably foreseen by a prudent operator. New areas to be covered will be pro-rated to the program cost at the rates in effect at the time. III. Program Payment Plan: For Parts I and II as specified in the 2017 contract. Each authorized application will be invoiced after performed. IV.
