Disclosure of Modification: All cars in the Tifosi challenge must run in stock form except for the limited modifications. The limited modifications permitted can be found in the 2015 Rule Book. Cars that are not in compliance with the modifications permitted may still be allowed to run under the Prior Modification Acceptance. However, ANY and ALL modifications from stock MUST be disclosed on this form regardless of whether it is determined that the modifications are permitted under the rules. Modification is defined as and any change whatsoever to the vehicle or replacement of any part that is not OME or an different brand. Modifications include but are not limited to the removal of any part or material. For any modification an exact description of the modification must be listed including the manufacturer, item number and part of each modified part, and or the modifications performed to existing parts or complements. For ease of use and disclosure of modification, this form lists categories that follow. This list is not exclusive of categories. If a modification falls outside of the categories it must me listed in "other." 2

Body: Including but not limited to the following: replacement of any hood, doors, body panel, or glass with any non OME material, and any aerodynamic changes such as additions to any front or rear bumper, addition do any front splitter, rear diffuser, or addition of a wing or winglets. B. Engine: Including internal parts and including bit not limited to modification replacement or removal of catalytic converters, headers and/or exhaust. C. Electronics: Including but not limited to ECU replacement or reprogramming. D. Interior: Including the replacement of any none OME part, and removal of any material. 3

Have you determined that the vehicle and the above modifications fall within the rules and regulations of CCR? Yes/No. 12. If No, are you applying for the discretionary Prior Modification Exception? Yes/No. If yes, please attach documents attesting to proof of the car's dyno, results. Such dyno must be performed within the 30 days of this form and submission. CCR reserves the right to subject the vehicle to a dyno prior to an scheduled event or at a scheduled event. If such car shows results that exceed the dyno results given in this form, any such material variation will be considered at direct breach of the obligation of disclosure in this form. At the sole and complete discretion of CCR, CCR may accept a non- complaint car with a prescribed weight penalty above the minimum prescribed weight. Such weight penalty may depend upon the modifications and dyno results. The amount of weight will be totally an completely at the discretion of CCR and may be increased or decreased at anytime for any reason before any event. Any car accepted under the Prior Modification Exception and running for points in any class against compliant car is vulnerable to an anonymous appropriateness challenge by any competitor from any class running in any compliant car. If such a challenge is logged, CCR may perform am re inspection and/or reconsideration and adjust the weight penalty. The submission of a request for entry of a vehicle under Prior Modification Exception may also be rejected at the discretion of CCR. 4

5 This Challenge Car Registration form must be affirmed by the Support Team Manager and Driver/Owner and a copy filed with the CCR before the start of the season or before the first event entered by the owner/driver in any season. The Challenge Car Registration form is good for a season but does not take the place of the individual tech forms necessary prior to each event. The signing of the Challenge Car Registration carries with it the ongoing duty to disclose and of any new information later comes to light or the conditions to the automobile change. Any discovery of new information or change in condition must be timely disclosed between the time of discovery and prior to the entry of the vehicle in any series event. Challenge Car Registration forms will be kept in a series database and Teams and Drivers acknowledge and assent to the sharing of the information contained in this form with any other team or driver in the series under the supervision of CCR and with the permission and discretion of CCR. I affirm and swear it is true and correct to the best of my knowledge. I have not knowingly omitted any known and required information requested by the Disclosure of Modification. I acknowledge that the duty to disclose is ongoing and of any new information later comes to light or the conditions to the automobile change, I will timely disclose any such change between the time of discovery and prior to the entry of the vehicle in any series event. I acknowledge I am subject to the penalties for knowingly making a false affidavit as prescribed in the Tifosi Challenge CCR 2015 Tifosi Challenge Rule Book.
