1 Pro Prospect Skill Camp 2018 (IHDC) Contract: Terms and Conditions Camp Dates: Arriving to Kuopio: In the evening Leaving from Kuopio: Program Cost: Total Euro 1.999,00 Early Bird Euro 1.850,00 Includes: 1. Training Camp 2. Transportation Daily on-ice training (1-1.5 h per day) Daily off-ice training (1-2 h per day) Professional Coaching Facilities (On and Off-Ice) Transportation provided for player includes: In Kuopio area: Transportation, all reasonable costs (fuel, insurance), to all group activities or a vehicle to use for the same purpose during the camp. Driver of the vehicle must have a valid European drivers licence and it has to be carried at all times while driving. 3. Accommodations Accommodation in Kuopio at the Rauhalahti Holiday villas or Spa Hotel Rauhalahti. 4-6 players per villa or 2-3 players / room in Spa Hotel Rauhalahti. Residential arrangement made by Pro Prospect Ltd. Daily lunch and dinner provided Monday Friday. (Breakfast not included, if to stay at villas.) 4. Coaches and other staff in the camp 5. Medical and physiotherapy staff on site to provide assistance, if need (additional cost for the participant according to a price list)
2 Registration Policies Acceptance: IHDC 2018 will be implemented if at least 6 players have officially registered by Maximum of 10 players will be accepted to the camp in order of registration. Player s acceptance into the program is confirmed only after a payment has been received. Full payment of Euro 1.990,00 must be made prior to Early bird price of Euro 1.850,00 must be made prior to Payment Options: Payments are accepted in the form of a Money Transfer. Conditions of Environment: IHDC reserves the right to suspend and/or expel a player from the program for any of the following reasons: financial delinquency, falsification of registration information, failure to abide by program and facility rules and policies. Head instructors reserve the right to place a player in a group according to his/her skill level. Cancellation Policy: In the event that it is necessary for a registrant to cancel, a refund will be granted if cancelled before There are no cancellations refunds granted after (Extreme cases will be reviewed and dealt with on a case by case basis). Damage Deposit: Player will be held personally responsible for any damages incurred to their accommodation or such property. Deposit (200 ) will be collected from the player after arriving to the camp and if such damages occur the costs of repair will be charged of the deposit. Deposit (200 ) will be returned to player s bank account after the camp, in case there is no damage found in any goods.
3 Code of Conduct: All players registered in the Pro Prospect Skill Camp 2018 (IHDC) program are required to submit to a strict set of guidelines regarding their code of conduct while participating in the program. These are as follows: All players will respect the integrity of the IHDC, its facilities, property and its staff members. Player will attend all on- and off-ice training sessions, and video review sessions. Absence from any of these activities must be approved by IHDC staff prior to the activity, and with a justifiable reason, i.e. injury, emergency. Players must respect all facilities, to use them with care and do their part in keeping them clean. Players must understand and acknowledge that association with or use of drugs, doping substances (alcohol or tobacco during IHDC official activities) of any kind is not acceptable and will not be tolerated. Players will be responsible financially for any damages caused to the facilities as a result of their own behaviour or negligence. IHDC organisers are not responsible for participants actions in any way during the camp. Participants will take care of themselves during free time. IHDC organisers are responsible only for the coaching etc. that is listed in this contract. Players must understand and acknowledge that failure to comply with any of the above rules and guidelines will result in the suspension or expulsion of the offending player(s). Camp fee will not be refunded. I have read the Code of Conduct and understand that my failure to comply will result in my suspension or expulsion from the program. ATHLETIC WAIVER AND RELEASE OF LIABILITY Acknowledging that there is a great risk involved, including the potential for permanent paralysis and death, during the participation in any sport, I agree that Pro Prospect Skill Camp (IHDC) its agents, servants, employees, and consultants shall not be liable to me for any injury or damage resulting directly or indirectly from any participation during ice skating, ice hockey or other program activities, whether incurred on or off the ice or otherwise in or around the buildings and properties.
4 I further agree that I discharge Pro Prospect Skill Camp (IHDC), its agents, servants, employees and consultants from all actions, claims and demands I may have for any injury or damage. I understand that my said agreement, release and discharge, shall bind my heirs, legal representatives and legal successors and shall come into effect to the benefit to Pro Prospect Skill Camp (IHDC), its agents, servants, and consultants and their successors. Pro Prospect Skill Camp (IHDC) reserves the right to use any pictures, video or other media taken during the camp for advertising and/or instructional purposes. In the event that the player is injured during the operation of the program, permission is given for their transportation as needed to a medical practitioner/facility at player s own expense. We strongly recommend that all players have their own physical examination done prior to their participation in the program. PLAYER HAS READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTANDS ITS TERMS, UNDERSTANDS THAT HE HAS GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING/APPROVING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
5 Insurance Coverage: Player must provide, by himself, sufficient insurance coverage for the duration of his stay in the camp. I do not have any medical condition that could affect my full participation in the program s activities (on or off-ice). I, the undersigned, have read and understand the registration policies, terms and all this written material Pro Prospect Skill Camp 2018 including ATHLETIC WAIVER AND RELEASE OF LIABILITY what so ever put forth by Pro Prospect Skill Camp (IHDC). If not signed by the player the registration payment of the camp is considered as an accepted confirmation by the player for all what is said in this registration material and its annexes. If the Parent/Guardian s Signature of the Player under 18 years is not enclosed it is required that a separate from the parent s/guardian s own address is sent confirming authorization. FOR PARTICIPANTS OF MINORITY AGE (Under 18 years of age at the time of registration): This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his release as provided above of all the Releases, and, for myself, my heirs, legal successors, and next of kin, I release and agree to indemnify the Releases from any and all liabilities incident to my minor child s involvement and participation in their program as provided above.
