14 Why Investigate? The primary purpose of an investigation is to: Identify the sequence of events leading to a failure. Identify the direct and indirect (root cause). Identify steps to prevent future occurrence. Investigations should be fact finding, not fault finding.

15 Incident vs. Accident Incident - an event that disrupts normal activities and may become a loss, claim or business interruption. Also referred as near miss Accident - an unplanned event, definite in time and place, that results in property damage or injury to a person or property.

16 What to Investigate? All accidents and incidents should be investigated regardless of loss or severity to include: Near misses First-aid injuries Minor property damage

17 How to Investigate Develop an investigation plan Have individuals trained in conducting investigations Know who should be notified and what resources are needed (policy requirements) Have resources available to aid in investigations (i.e. camera, accident reports)

18 When to Investigate Begin Immediately! Most of the crucial information needed to be gathered is available at the time of the accident. Ensure that the hazards related to the accident do not still pose a threat of further loss. Check for danger Help the injured Secure the scene

19 Five Why s The five whys is a simple way to apply a root cause analysis method. Benefits to this approach include: Quickly separates symptoms from causes Helps determine relationship between causes Flexible and inexpensive

20 Causal Factors Equipment Was a hazardous condition a contributing factor? Environment Was the location of the equipment/ materials/employee(s) a contributing factor? People Was an unsafe act(s) a contributing factor? Management- Was a management system a contributing factor?

21 Utilization of Key Elements in Investigations Management Commitment/Employee Involvement: Policies and procedures Prior enforcement issues Training: Initial training documentation Hazard Identification: Previously identified hazards JSA Accident/Injury Investigations: Review similar investigations for trends

22 Hazard Identification Most employers are committed to providing a workplace free of known hazards. Regular safety inspections help to ensure safe working and work site conditions.

23 Hazard Identification The primary purpose of an inspection is to spotlight unsafe working conditions and equipment, unsafe acts, and reveal any need for new safeguards and procedures. Inspections also foster safety awareness and involves employees in the risk management program.

24 Hazards Hazard - any existing or potential condition in the workplace that can result in losses. Hazards are classified as either unsafe acts or unsafe conditions.

25 Unsafe Acts Unsafe Act - a behavioral departure from an accepted, normal, or correct procedure or practice which, in the past, has produced injury or property damage and has the potential for doing so in the future or conduct that reduces the degree of safety normally present in an activity.

26 Unsafe Conditions Unsafe Conditions - are any physical state that results in a reduction in the degree of safety normally present.

27 Hazard Identification Employees shall promptly report the need for repairs of any owned or leased property issued to, used by, or possessed by the employees to the supervisor and to the department officially charged with maintenance of such property.

28 Job Safety Analysis The JSA is documented by using the following analysis process: Sequence of basic job steps Potential hazards Recommended action or procedure

32 Training There should be four types of training: Orientation Initial training Regular safety training Special needs

33 Training Supervisors are responsible for training those staff they supervise. The intent of this training is to provide an understanding of the job requirements, create awareness of hazards associated with such requirements, and steps to reduce hazards. Training should be documented.

34 Utilization of Key Elements in Training Management Commitment/Employee Involvement: Policies and procedures new and current Prior enforcement issues Training: Previous training Hazard Identification: Common hazards New equipment/processes JSA Accident/Injury Investigations: Review injury trends

35 Management Commitment Management demonstrates its commitment to employee safety through comprehensive policies and procedures. Supervisors should be held to a higher expectation of commitment through consistent demonstration of expectations and accountability.

36 Management Commitment Management commitment is one of the most crucial elements of any program. Tips to gain management commitment and support include relating the risk management initiatives to: Organizational objectives/mission Core Values Council/Board Priorities Accreditations/Awards

37 Employee Involvement Employees are responsible for exercising care and good judgment in preventing accidents and for observing safety rules when performing job duties. Employees are required to: Report all accidents to their supervisor and seek first aid for all injuries, however minor they may be Report any unsafe work conditions, equipment, or practices to their supervisor as soon as possible
