1 1 issue: 08/14 1 of 8 NOTIFICATIONS It s Premuda requirement that the Owner/Management Company of the time chartered vessel reports accidents and near misses for fixed crew based on 24 hours service on board, according to these requirements. Accidents or near misses occurred during working time and non-working time on board shall be reported using the enclosed Form Nr. 1. Temporary or any type of riding crew shall report according to the reporting requirements, but will not be included in the performance calculations (enclosed Form Nr. 2 to be used). The report shall include a brief description of the event and the date of occurrence. Immediate reporting from Management Company to Premuda All accidents level 3, 4 and 5 and near misses level 4 and 5 described in the enclosed Report Consequence Matrix shall be reported to Premuda (sms@premuda.net c.c.to DPA@premuda.net) without unnecessary delay. The report shall include a brief description of the event, direct causes, root causes and actions to avoid reoccurrence.

3 3 of 8 REPORTING FORMS FOR ACCIDENTS AND NEAR MISSES This Section contains the forms to be used for accidents and near misses reporting. The Form nr. 1 has to be used for accidents, near missess occurred during working time and non working time on board. The Form nr. 2 has to be used for temporary or any type of riding crew. All reports should be sent to: sqe@premuda.net Deadlines for regular reporting of HSE data The quarterly report shall be sent to Premuda (sqe@premuda.net) no later than the 10th day in the month following each quarter (10th of April 10th of July 10th of October 10th of January).

6 6 of 8 HSE Definitions Accident An event resulting in personal injury, environmental damage, material damage/loss or loss operation. Detention Where a vessel is prevented from sailing, loading or discharging following action by a regulatory body. Environmental Incidents Pollution Incident (PI): A spill of cargo or bunkers (>1 bbl) to the environment. Significant Pollution Incident (SPI): A spill of cargo or bunkers >5000 bbls to the environment, or which results in legal action. (>5000 bbls is in line with ITOPF definition for a large spill). Oil Spill (OS): A loss of containment of cargo or bunkers with either no impact or with a release of <1 bbl to the environment. Equipment Leak (EL): Any leak, excluding cargo or bunkers, form equipment, glands, hydraulic system, etc. Unauthorized Discharge (UD): The non compliant release of a regulated substance (i.e. garbage, waste oil sludge/debris, stack emissions, bilge, ballast) to land, water or air. Exposure hours 24 hours per day while serving on board. Fatality Loss of human life. This does not include suicide. First Aid Injury requiring treatment or examination from a first aid team or nurse. Fixed crew Permanent crew employed on the vessel. Lost Time Injuries (LTI) Are the sum of fatalities, permanent total disabilities, permanent partial disabilities and lost workday cases. Lost Time Injury Frequency (LTIF) This is the number of Lost Time Injuries per unit exposure hours. The most common unit in respect of LTIF is one million man hours. LTIF = LTIs x Exposure Hours
7 7 of 8 Lost Workday Cases (LWC) This is an injury which results in an individual being unable to carry out any of his duties or to return to work on a scheduled work shift on the day following the injury unless caused by delays in getting medical treatment ashore. Medical Treatment Case Treatment or medical examination requiring medical knowledge or performed under guidance from a nurse or a doctor but where the person can start on the next shift. Near Miss An event that under slightly different circumstances could have resulted in an accident. Hazardous conditions shall be considered as a near miss. Permanent Partial Disabilities (PPD) Is any work injury which results in the complete loss, or permanent loss of use, of any member or part of the body, or any impairment of functions of parts of the body, regardless of any pre-existing disability of the injured member or impaired body function, that partially restricts or limits an employees basis to work on a permanent basis at sea. Such an individual could be employed ashore but not at sea in line with industry guidelines. Personal Injury All bodily harm occurred during working time or non-working time onboard the vessel. Process loss/operational loss Event that causes delay in vessel departure/arrival, delay of cargo transfer, off-hire. Property Loss Damage to ship or ship equipment. Restricted Work Case (RWC) Injuries that result in an individual being unable to perform all normally assign work functions during a scheduled work shift or being assigned to another job on a temporary or permanent basis of the day following the injury. Security Loss Theft, forced entry or criminal damage are security losses (undesired events) that involve illegal personal profit (burglary, theft, filching) and other intentional incidents like e.g. vandalism, fire raising etc. Total Recordable Cases (TRC) The sum of all work-related fatalities, lost time injuries, restricted work injuries and medical treatment injuries.
8 8 of 8 TRIF (TRCF) This is the number of TRCs per unit exposure hours. The most common unit in respect of TRCF is one million man hours. TRCF= (LTIs + RWCs + MTCs) x Exposure Hours Vetting findings per inspection Vetting Average No Deficiencies: any noted deficiencies by attending inspectors that could have been avoided by ship's staff and/or onshore management. Design issues are not to be included in this number (please refer to OCIMF). (Calculation: Number of Inspections divided by number of ship avoidable deficiencies = Avg number of deficiencies per inspection).
