Please acknowledge receipt of this Addendum No.1, dated January 8, 2013 on the Bid Form when returning your Bid package. Failure to acknowledge this addendum may result in your bid being declared nonresponsive. All other general terms, conditions and specifications shall remain the same. Project Manual: 1. Section 00905, PART 6, 6.01, : REPLACE the first sentence with the following: The CONTRACTOR shall assume and agree to hold harmless, indemnify, save, protect and defend Henrico County, Capital Region Airport Commission, and URS Corporation against any and all liability for injuries and damages to CONTRACTOR himself and to CONTRACTOR's employees, agents, subcontractors and guests, third parties or otherwise, incident to or resulting from any and all operations performed by the CONTRACTOR or his agents, employees and subcontractors under the terms of this Contract. 2. Section 00905, PART 6, 6.01, : REPLACE the entire paragraph with the follow: The CONTRACTOR shall carry Public Liability Insurance in the amount specified below, including the Contractual Liability assumed by the CONTRACTOR, and shall deliver Certificate of Insurance from carriers acceptable to the OWNER specifying such limits with County of Henrico, Capital Region Airport Commission and URS Corporation named as an additional insured. 3. Section 00905, PART 6, 6.01, : REPLACE the entire paragraph with the following: Limits of Liability:... $1,000,000 per occurrence Aggregate:... $2,000,000 per occurrence Products/Completed Operations:... $2,000,000 Personal and Advertising Injury:... $1,000,000 Fire Damage Legal Liability:... $100,000 Coverage must be broad from property damage and include Explosion, Collapse and Underground Coverage (XCU)

2 Umbrella Liability:... $2,000,000 per occurrence The CONTRACTOR shall bind itself and its successors in interest to indemnify and save harmless the Owner, its officers, agents and employees; Capital Region Airport Commission, and URS Corporation from all suits brought against it, or them, and all costs or damages to which it, or they, may be put on account of, or by reason or, any injury to the person or property of another resulting from acts or omissions during the performance of the work by the Contractor or its agents. The Contractor further agrees to indemnify and save harmless the Owner against any claims, suits or demands against it by reason of injury sustained by an employee of the Contractor in and about the work, under any provision of the Workmen s Compensation Law, and the Contractor shall produce certificates or other satisfactory evidence of ample protection against such liability. 4. Section 01000, 1.05: ADD the following: F. All WORK and phases of construction within the Capital Region Airport Commission shall be scheduled and coordinated with the Owner a minimum of 20 days prior to any activity and access onto the property. Bidder Questions: Given that the type/size/coating of the pipe to be installed per this contract is relatively difficult to obtain in a timely manner without a full order being placed at once, will Henrico County allow the contractor too invoice for stored materials? 8. Clarification: The County will pay for store materials provided that they are stored in accordance with the manufactures recommendations and they are stored at a location where they can be inspected and verified by the County s Representatives. The Contractor shall include with each Partial Payment Application copies of invoices for stored materials. See Section , Sec. 14.2, Application for Progress Payment. There seems to be conflicting details on the project plans. Sheet C-7 indicates a backfill standard for material adjacent to the proposed pipeline per Public Utilities standards; however, Sheet C-11 indicates a different bedding/backfill standard according to Public Works. Can you please confirm which standard will dictate during construction of this project? 9. Clarification: WORK associated with this project is located within Public Utilities easements, and road right-of-ways. The standard used for this project is based on DPU Trench Bedding detail shown

3 on Sheet C-7. The exception to this standard is per Sheet G-3/ County of Henrico Department of Public Works (DPU) Notes/ #21, excavation in the roadways and shoulders must be backfilled per the County s and/or VDOT s Standards and Requirements, whichever is more stringent. 6 December 27, 2013 Mr. David Tostenson Henrico County Department of Public Utilities Woodman Road Henrico, VA Subject: Project , Geotechnical Data Report, Old Williamsburg Road Pavement, Old Williamsburg Road at Scotch Pine Drive, Henrico County, Virginia Dear Mr. Tostenson: SCHNABEL ENGINEERING CONSULTANTS, INC. (Schnabel) is pleased to submit our geotechnical data report for this project. This document includes attached figures with relevant data collected for this study. Our services were provided in accordance with our contract with the County for Geotechnical Engineering Services and our proposal dated November 26, SCOPE Our proposal dated November 26, 2013, defines the scope of this study. Our services include coring of the existing pavement. The objective of this study is to evaluate the existing pavement at the site for this project. Services not described in our agreement are not included in this study. We would be happy to provide additional support services to the design team as the project demands. SITE DESCRIPTION The site is located just west of the intersection of Old Williamsburg Road and Scotch Pine Drive in Henrico County, Virginia. East Williamsburg Road runs in an east-west direction south of the site. Old Williamsburg Road consists of asphalt pavement over concrete pavement, both with unknown thicknesses. The roadway also terminates about 200 ft west of the above intersection. An existing church and some residential dwellings are located near the site. Some lightly wooded areas are also present on both sides of Old Williamsburg Road. We obtained the site information through discussions with your office on November 26, 2013 and from the Henrico County White Oak Trunk Sewer Replacement Plan and Profile dated November 2013, prepared by URS and provided to us by your office. We also obtained site information through review of site aerial photographs and through our site visit. A Site Vicinity Map is included as Figure 1.

7 County of Henrico Old Williamsburg Road Pavement PROPOSED CONSTRUCTION We understand that a new 36-inch diameter trunk sewer line is planned for construction at the site. The entire length of the sewer line will be about 6,500 lineal feet. The alignment is expected to cross Old Williamsburg Road at the site described above. The invert of the new sewer line is expected to be about 18 ft. below the existing ground surface. We also understand that the sewer pipeline installation methodology will utilize an open cut technique and that a portion of the existing pavement will be removed at the roadway crossing during construction. We obtained the project information through communication with you. Data Collection Techniques Pro-Cuts Professional Concrete Cutting, a division of Canada Contracting Co., Inc. of Sandston, Virginia, drilled one 6-inch diameter asphalt and concrete core under our observation. The approximate core location is shown on Figure 2. We will retain the core sample up to 45 days beyond the issuance of this report, unless you request other disposition. PAVEMENT We have measured the existing pavement based on the core obtained and presented in Figure 3. The core we obtained from the pavement consisted of two pieces. The upper piece was asphalt with a thickness of 5.0 inches. The lower piece was a combination of an upper asphalt layer with a thickness of 0.5 inches and a lower concrete layer with a thickness of 6.5 inches. The total thickness of the pavement was 12.0 inches. We did not conduct laboratory tests to evaluate the characteristics of the pavement materials. Immediately below the concrete we observed soil. LIMITATIONS We have endeavored to complete the services identified herein in a manner consistent with that level of care and skill ordinarily exercised by members of the profession currently practicing in the same locality and under similar conditions as this project.
