1 CHESAPEAKE MATH AND IT ACADEMY BUS CONTRACT (AUGUST 2013 JUNE 2014) - Please read the entire contract with your child and sign / initial the appropriate areas as instructed. Incomplete contracts will not be processed and seats will not be reserved until the contract is complete. - A separate contract is needed for each student. - Contracts of students/parents with - outstanding enrichment or bus service balances or - 3 or more late bus payments in will not be processed. Read this Bus Contract thoroughly. 2. Submit this completed and signed Bus Contract to CMIT Academy. 3. Include with this Bus Contract an initial, non-refundable payment of $100 per CMIT student payable to Chesapeake Math and IT Academy. 4. Payment and Bus Contract are due no later than 3 pm, June 10, Please make all checks or money orders (no cash or credit card) payable to Chesapeake Math and IT Academy. 2 DROP OFF / PICK UP Students have to be dropped off at their designated bus stop at least 5 (five) minutes prior to the departure time. CMIT Academy has no supervision agreement with any library or shopping centers where the bus stops exist and cannot be held liable for any incident may happen to the students until they get on the bus. Similarly, CMIT Academy cannot be held liable for anything that might happen after the students leave the bus. It is the Parents/Guardians responsibility to be at the bus stops on time or make the necessary arrangements for drop off and pick up. EXITING AT DESIGNATED BUS STOPS Students may only be dropped off at their specific, pre-designated bus stop. In the event that your child s final destination is different than the bus stop that is not his/her regularly designated one, s/he must have, in hand, written permission signed by his/her parent/ guardian. The written authorization must be given to the bus driver EACH time s/he requests to leave the bus at a stop that is not his/her regularly designated stop. Students can only use their designated BUS and BUS ROUTE; usage of different bus routes is NOT permitted. CONDUCT Students are expected to treat each other and the driver with respect and consideration at all times. Rude, obnoxious, or mean behavior will not be tolerated. The students who are not following rules and procedures will face consequences which include suspension from bus and bus contract cancellation. In the event that bus contract is canceled, parents are required to provide transportation. If parents are unable to provide transportation or find a safe way to have their child commute to / from school, the parent will need to register the student to their home school by withdrawing from CMIT. DISCIPLINE POLICY: PGCPS student handbook and discipline policies will also be in place in school buses. WITHDRAWAL FROM THE SCHOOL: Any parent/guardian who is willing to withdraw his/her student from CMIT, has to give at least 30 (thirty) day written notice. For any case other than withdrawal from the school, ALL PAYMENTS will remain in effect for THE ENTIRE SCHOOL YEAR (42 WEEKS) and NO REFUND will be issued CMIT Student Bus Contract Student Initials: Parent Initials: Page 2 of 5

Bus and bus stops are considered as school grounds and students are to show positive and acceptable behavior that is specified in PGCPS student code of conduct. I understand and accept that students are required to be seated at all times on the bus. Students are required to sit when the bus is at parking lot, at a bus stop, or on the road. I understand and accept that students are required to sit in their assigned seats all the time and not to change their seat or walk around whether the bus is at the parking lot, at bus stop, or on the road. I understand and accept that students are required to respect the bus driver and follow their instructions. I understand and accept that students are required to refrain from eating, drinking, and chewing gum on the bus. I understand and accept that students are required to keep the buses clean and neat. The students who damage anything on the bus will be billed for the required repair and maintenance. I understand and accept that students are required to be at the designated bus stop at least 5 minutes before the scheduled arrival time of the bus. I understand and accept that students are required to respect others on the bus, keep their feet inside the seats not to block the aisle, keep their voice down. I understand and accept that students are required to keep hands and heads inside the bus by staying away from the windows. I understand and accept that students are required to respect other drivers and cars on the road by showing appropriate behavior. I understand and accept that students are required to be picked up at designated drop off time from the designated bus stop. Once students are dropped off at scheduled time, it is the parentsʼ responsibility to provide safety supervision to the student. I understand and accept that students are required to take care of their belongings on the bus. Neither CMIT nor the bus service provider company will be responsible for lost and stolen items. I understand and accept that students are required to follow cell phone and electronic device use policy. Cell phones may ONLY be used to inform parents about pick up time. Other than that, texting, making phone calls, and connecting to internet are not allowed. I understand and accept that students are required to use the same bus and bus route that is selected in this bus contract, at all times. 4 Please note: Bus riding privileges may be denied for the remainder of the school year if your child demonstrates that s/ he is unable to follow the rules. I/We, the undersigned parent(s)/guardian(s), have read this Bus Contract in its entirety and request bus service for the student(s) listed above according to the payment schedule. I/We agree to pay the entire annual rate regardless of whether my/our child(ren) ride(s) the bus on a daily basis. Once the bus route is established, I/We will designate the regular bus stop for my/our child(ren). I/We understand that while riding the bus, my/our student(s) must follow all CMIT rules and all bus driver instructions. I/We agree to discuss the bus rules with my/our child(ren). I/We understand that any breach of bus rules or PGCPS Code of Conduct will result in disciplinary action. Cases that are more serious and / or repeating violations may result in denial of bus privileges. THE BUS RULES ARE IN EFFECT FOR THE SAFETY OF EVERYONE ON THE BUS AND WILL BE ENFORCED AT ALL TIMES. I/We agree to accept liability for any actions resulting from vandalism or misuse of school property. CMIT reserves the right to cancel this contract anytime if payments that are not made on time and / or outstanding balances exist. In the event that bus contract is canceled, parents are required to provide transportation. If parents are unable to provide transportation or find a safe way to have their child commute to / from school, the parent will need to register the student to their home school by withdrawing from CMIT. CMIT will send a written confirmation ( or mail or fax) to confirm that bus contract is received, processed, and a seat is reserved for the student. Without the confirmation, students will not be allowed to utilize bus services; submitting bus contract without receiving a written confirmation from CMIT will not guarantee any services. Please note: Contracts will not be accepted unless completed entirely and submitted as well as $100 initial, non-refundable registration fee.
