ELIGIBILITY Employees who regularly work at least 30 hours per week Your spouse and dependent children Your domestic partner and his/her dependent children All benefits except life insurance Note: If you regularly work fewer than 30 hours per week, you may be still eligible for the Transamerica medical plans and some Aflac options. 3

WHAT S NEW No change in plan premiums Your costs stay the same: No rate/premium changes this year! Plus, with the new $4,500 deductible plan option, you ll even save money on premiums! 4

WHAT S NEW Annual Enrollment: 10/13 10/27 Plan designs are similar to last year, with some new offerings This year is an active benefits enrollment for the 2017 plan year for: Medical Dental Vision Spending Accounts There is a new $4,500 deductible medical plan. In order to enroll or re-enroll, check premiums, or make changes, you ll need to take the steps as noted in this presentation and in the enrollment materials. Help needed? Licensed Benefits Counselors are a few clicks or a phone call away 5

WHAT S NEW Telemedicine is now available for BCBS and UHC medical plans. Telemedicine is available for all BCBS and UHC medical plans. This is new for Blue Cross Blue Shield Plans for 2017. Low on time and money for a doctor s visit? Talk to a live doctor or nurse by phone with the new Telemedicine feature offered in our 2017 BCBS and UHC medical plans. Get medical advice, prescriptions, and more while saving an average of $60 per doctor s visit*. * Depending on plan type 6

WHAT S NEW Single Sign-On: One less password to remember Employees will now log into Mercer Marketplace with their SYKES NT/network username and password at: SYKESBenefits.sykes.com (Just like Employee Self-Service and other SYKES websites!) 7

WHEN TO ENROLL You must enroll either during the Annual Enrollment period (Oct. 13 Oct. 27), or if you are a new hire, during your benefits eligibility waiting period. Coverage takes effect: January 1 st for Annual Enrollment New hires become effective the first of the month after 60 days from the end of your 30-day Orientation Period, but in no case longer than 120 days from your hire date. Password Help: See next slide 8

HOW TO ENROLL ONLINE SINGLE SIGN-ON INSTRUCTIONS With single sign-on, you are now able to log on to the Mercer Marketplace using your SYKES NT/network username and password at: sykesbenefits.sykes.com What if I lock out my SYKES NT Network password? If you attempt to log in too many times with the wrong password and get locked out, you can: Option 1: Wait 30 minutes for the password to unlock. Option 2: Contact the SYKES SDC helpdesk at 1-888-556-0450. What should I do if I still can t get logged in? Call Mercer Marketplace at 1-855-543-4322. A licensed Benefits Counselor will log your issue and take your enrollment via phone. You can make changes to your benefit elections until the enrollment period ends. 9

MEDICAL PLAN OPTIONS More Flexibility with Several Medical Choices for Group 2 Several medical plan options for Group 2 employees: 1. Low Limited Benefit Medical Plan 2. Medium Limited Benefit Medical Plan 3. High Deductible Health Plans (HDHPs) $2,500 Deductible Plan with Health Savings Account (HSA) New: $4,500 Deductible Plan with Health Savings Account (HSA) All plan options satisfy your obligation to have medical insurance as mandated by the Affordable Care Act. 11 All plan documents and Summary Plan Descriptions govern.

All plans satisfy your obligation under Healthcare Reform to have coverage All plans cover essential benefits (preventive services) at 100% in-network Both plans can be paired with optional Health Savings Accounts (HSAs) What s different between the plans? SUPPLEMENTAL MEDICAL INSURANCE Supplemental medical insurance (such as Accident, Critical Illness, or Hospital Indemnity) protects you from expenses not covered by your medical plan. In fact, you may be able to save money by adding a supplemental plan to a lower cost medical plan. Supplemental plans are available in most, but not all states. Coverage is available for you and your dependents. On their own, supplemental plans do not provide the minimum level of coverage needed to meet ACA requirements. TERM LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT AFLAC: PERMANENT LIFE Virtual employees can elect permanent life via the Mercer Marketplace website at sykesbenefits.sykes.com Brick-and-mortar agents should contact their local Aflac representative Evidence of Insurability (EOI) Evidence of Insurability may be required for enrollment into these plans; if so, you will be prompted for it during the enrollment process.

SHORT-TERM AND LONG-TERM DISABILITY Short-Term Disability (STD) Voluntary coverage that pays you a fixed dollar benefit while away from work for a short-term disability Virtual employees can elect STD insurance through the Mercer Marketplace at sykesbenefits.sykes.com Brick-and-mortar agents should contact their local Aflac representative Evidence of Insurability (EOI) Evidence of Insurability may be required for enrollment into these plans; if so, you will be prompted for it during the enrollment process. If you live in a state that requires your employer to offer short-term disability benefits, your disability will be coordinated between your employer and the state. This applies to employees in California, New York, New Jersey, Rhode Island, Hawaii and Puerto Rico.

SMART WAYS TO SAVE MONEY Here are two easy ways you can save money on healthcare expenses: In-network vs. out-of-network: Before you visit a doctor, dentist, provider, or facility, make sure that the person or facility is in-network, meaning he or she (or it) is part of your health plan. If you see an out-of-network provider, your costs will be higher. You can check on this by calling the provider s office, or by looking online. Usually, it s safest to call. Generic medications: Make sure that when you visit the pharmacy or call mail-order for your (or your family s prescriptions), you ask for the generic equivalent of the medication. Sometimes, however, generic will not be available. 23
