3 WELCOME -3- Mount Ida College offers a competitive benefits package to all eligible faculty and staff. The following is a summary of the benefit plans offered. Additional information about each of these benefits may be obtained from the Office of Human Resources. 4 -4- Medical Insurance Your Medical Plan Options The College offers two types of medical plans through Blue Cross Blue Shield: an HMO and PPO plan. All full time benefit eligible staff and faculty members are eligible to enroll in the College s health insurance plan upon date of hire. Coverage is available to employees and their dependents, including domestic partners. HMO Blue New England Enhanced Value In the HMO plan, members will be required to select a Primary Care Physician (PCP) and receive all care from participating providers (in-network). The Blue Cross HMO network covers the entire New England region. Most specialist services will require a referral from a PCP. There is no coverage for services received from a non-participating provider (out-of-network). You will be covered for Emergency treatment regardless of whether care is provided by a participating provider or non-participating provider. Preferred Blue PPO Blue Cross Medical HMO 2017/2018 Medical Rates Blue Cross Medical PPO Bi-weekly Monthly Bi-weekly Monthly Individual $75.27 $ $ $ Family $ $ $ $ The PPO medical plan consists of a national network of participating providers. You do not need to elect a Primary Care Physician and you do not need a referral to see specialists. You are also allowed to go to any non-participating providers (out of network providers). For this option, you will pay more out of pocket and higher premiums. Check here to see if your doctor currently participates in the Blue Cross Blue Shield network:

6 -6- The College offers a dental plan through Blue Cross Blue Shield. With the Blue Cross Dental Blue Plan, eligible employees will have access to Blue Cross expansive national network of dentists. All full time benefit eligible staff and faculty members are eligible to enroll in the College s dental insurance plan upon date of hire. Coverage is available to employees and their dependents, including domestic partners. Blue Cross Dental Blue Benefit Highlights The following outlines the benefits offered. 7 -7- The College offers a vision plan through EyeMed Vision Care. Under the EyeMed Select Plan, you will have access to EyeMed s national network of doctors and vision care providers. All full time benefit eligible staff and faculty members are eligible to enroll in the College s vision insurance plan upon date of hire. Coverage is available to employees and their dependents, including domestic partners. Coverage is fully paid by the employee. Frames Standard Plastic Lenses: In-Network Member Cost $0 Copay, $150 Allowance; 20% off balance over $150 Out-of-Network Reimbursement Up to $90 Single Vision / Bifocal / Trifocal $25 Copay Up to $42/$78/$130 Contact Lenses (Contact Lens allowance includes materials only; in lieu of Standard Plastic Lenses): Conventional $0 Copay, $150 Allowance; 15% off balance over $150 Up to $120 Disposables $0 Copay, $150 Allowance; balance over $150 Up to $120 Medically Necessary $0 Copay, Paid in Full Up to $200 LASIK and PRK Vision Correction Procedures: 15% off retail price OR 5% off promotional pricing N/A Frequency: Frames Once every 24 months Standard Plastic Lenses or Contact Lenses Once every 12 months The College offers a Flexible Spending Account program through WageWorks that allows eligible employees to use pre-tax dollars to pay for qualified out-of-pocket medical expenses and dependent care expenses. All full time benefit eligible staff and faculty members are eligible to enroll in the College s Flexible Spending Account program upon date of hire. The maximum contributions to the Mount Ida FSA plans are: Vision Plan 2017/2018 Vision Rates Bi-weekly EyeMed Vision Plan Benefit Highlights The following outlines the benefits offered. 8 -8- Basic Life and Accidental Death & Dismemberment (AD&D) Insurance The College provides all full time benefit eligible staff and faculty members with a life insurance and AD&D benefit through Lincoln Financial of 1.5 times base salary up to a maximum of $250,000. Coverage is fully paid by the College and is effective as of the first of the month following date of hire. Supplemental Life and Accidental Death & Dismemberment Insurance All full time benefit eligible staff and faculty members are eligible to elect supplemental life and accidental death and dismemberment insurance. Eligible employees may elect up to five times their base salary in $10,000 increments to a maximum of $500,000. Coverage is fully paid by the employee. Eligible employees also have the option of enrolling their spouse, domestic partner, and/or dependent children in the College s supplemental life and AD&D plan. Employees must be enrolled in the supplemental life and AD&D plan to enroll their spouse, domestic partner, and/or dependent children. Spouse coverage is available in $5,000 increments up to two and a half (2.5) times the employee s annual salary, not to exceed 100% of the employee s benefit amount. Proof of insurability may be required. Child coverage is available in $2,000 increments up to $20,000. Please contact Human Resources for rates. Short Term Disability Insurance (STD) Full time benefit eligible staff and faculty members who have completed one year of service are eligible for the College s short term disability (STD) benefit. This benefit provides salary continuation for nonwork related, short-term disabilities lasting more than two weeks in length and up to six months, with proof of disability. Salary continuation payments begin after an employee has utilized all accumulated sick and vacation time (if applicable). For more information contact Human Resources. Long Term Disability Insurance (LTD) The College provides Long Term Disability insurance through Lincoln Financial to all full time benefit eligible staff and faculty members. This benefit provides for two-thirds of salary up to a maximum of $8,500 per month after 180 days of complete disability. This coverage is fully paid by the College and is effective as of the first of the month following date of hire.
9 -9-403b Retirement Plan The College offers a 403b retirement plan to all staff and faculty who have worked a minimum of 1,000 hours in a calendar year and have completed at least one year of service. The College will contribute five percent (5%) of an employee s eligible earnings after eligibility has been met. The College s 403b Retirement Plan is currently serviced through TIAA. 403(B) Supplemental Retirement Plan The College offers a 403(B) Supplemental Retirement Plan to all staff and faculty upon date of hire. This plan provides all employees the opportunity to save for retirement on a voluntary pre-tax basis. Employees may contribute up to the annual IRS allowable maximum. The College s 403(B) Supplemental Retirement Plan is currently serviced through TIAA. Tuition Benefits Mount Ida College encourages and supports the continuing education of our staff, faculty and their eligible dependents. To that end, the College offers several tuition programs including tuition remission, tuition reimbursement and the Tuition Exchange Scholarship program. Employees may take advantage of the tuition remission benefit for courses taken at Mount Ida in the undergraduate and graduate program. They may also utilize the tuition reimbursement program for courses taken at other accredited institutions. Dependents of College employees are eligible to participate in the tuition remission program for courses taken at Mount Ida in the undergraduate program only. Dependents are also eligible to participate in the Tuition Exchange Scholarship program. More specific information about these programs and eligibility guidelines can be obtained from the Office of Human Resources.
10 -10- Employee Assistance Program (EAP) The College provides an Employee Assistance Program to all employees at no cost, through AllOne Health. The EAP provides an array of services to employees and their dependents and/or household members for personal or work-related issues and includes the following services: Up to three (3) short-term counseling sessions (includes assessments, referrals, and follow-up sessions) Legal Consultation (includes an initial 30-minute consultation per issue, 25% discount off of an attorney s normal hourly rate if needed after consultation) Budget and Debt Counseling (available by phone thru American Consumer Credit Counseling. Service includes access to certified and professionally trained debt counselors) New Parents Partner Program (includes up to three (3) 60-minute coaching sessions designed to provide assistance and support to expecting parents as they progress through pregnancy, maternity leave, and their transition back to work) Online Work/Life Resources (access to over 12,000 articles, videos, tip sheets, calculators, and searchable provider directories. 11 -11- Group Auto & Homeowners Insurance The College sponsors a group auto and homeowners/renters insurance program through Liberty Mutual. As an employee of Mount Ida College, you can take advantage of special savings on car and home insurance with Liberty Mutual. To learn more, visit the Mount Ida College custom site Paid Time Off (Staff) The College provides eligible employees with paid time off including vacation time, sick time, and personal time. Vacation Time Employees in full time positions or in part time regular positions (.43 FTE or greater) are eligible for paid vacation time after completion of a 90 day introductory period. Vacation time is pro-rated for regular part time and academic year employees. Vacation accrues as follows: Years of Service Time Accrued Maximum Carryover 0-3 years 15 days 5 days 3 years or more 20 days 10 days Sick/Personal Time Full time staff members receive ten (10) sick days each calendar year; up to three of those days may be used as personal days. Regular part time employees (.43 FTE or greater) receive pro-rated sick time based on the number of hours they are scheduled to work per year. Sick time is available beginning the first of the month following the date of hire.
