It is mtivated by yuth-adult partnerships that create essential and pwerful pprtunities that enhance and imprve lcal cmmunities. Cmmunity service, scial actin activities, participatin in advcacy fr safe and healthy envirnments, and prmtin f healthy plicies are rganized by yuth t appeal t yuth. FNL s main fcus is prmting healthy lifestyles free f alchl, tbacc, r ther substance abuse amng yuth. This prgram als fcuses n traffic safety and underage gambling preventin. The visin fr FNL prgrams is t frm yuth/adult partnerships with yung peple, t prvide prgrams rich in pprtunities and supprt, s yung peple will be less likely t engage in prblem behavirs, mre likely t achieve in schl, and mre likely t attend higher educatin r secure a full-time jb. Standards f Practice and Operating Principles: The FNL system has identified a list f prgram utcmes that are imprtant, achievable, and measurable. Prgrammatic Requirements: Implement at least 2 FNL Chapters, ne f which must be a Radmap Chapter (See Attachment A). Chapters prvide FNL prgrams with interactive pprtunities t prmte yuthled ideas and yuth decisin-making. Within a chapter, grups can take part in real cmmunity change that extends beynd the chapter walls, beynd the schl r site, and int the cmmunity at large. In a chapter, yuth experience a sense f belnging t smething larger than themselves and establish meaningful cnnectins with their peers and adults. Lcal chapter sites can be hused at a variety f places, such as: Schls, Cmmunity Centers, Churches, Juvenile Facilities, Recreatin Centers, Yuth Cuncils, Mentring Prgrams, Yuth Actin Grups, and Fster/Grup Hmes. Develp an annual Actin Plan t implement an alchl and r ther drug preventin prject each year fr the Radmap Chapter that fcuses n yuth develpment and changing the envirnment arund alchl and r ther drugs, including tbacc. This plan is due t the Friday Night Live (FNL) administrative staff each year n Sept. 30 th. The actin plan needs t be based n the steps and activities listed in the FNL Radmap Chapter Prject Guide. Enter all prgram activities int the State Department f Health Care Services webbased data cllectin system called Cal OMS Preventin (Pv). Activities can include meetings, trainings fr yuth and staff, prject develpment activities, yuth led presentatins, Radmap chapter activities, etc. Technical Assistance (TA) frm the Tulare Cunty Office f Educatin n data entry will be prvided if needed. Identify Gals and Objectives frm the BHCS Alchl and Other Drug (AOD) Preventin Strategic Plan that the FNL prgram will wrk twards. The FNL prgram activities will need t be linked t at least ne gal and bjective in the current BHCS AOD Preventin Strategic Plan. Once bjectives are identified, BHCS staff will link yur prgram with the identified gals and bjectives in Cal OMS Pv.

P a g e 4 Cmplete BHCS annual reprt n prgress/challenges and utcmes. The reprt template will be prvided by BHCS and is due n July 30 th f each year. Cmplete FNL Member in Gd Standing (MIGS) Reprt each year. The reprt template is prvided by the FNL administrative staff and is due June 30 th f each year. Administer annual FNL Yuth Develpment survey t Chapter members. Attend annual Leadership Develpment cnference (yuth leaders can als attend). Participate in mnthly cnference calls with ther Bay Area FNL prgrams and crdinatrs. Participate in quarterly in-persn TA trainings hsted by FNL administrative staff. Cntracting and Fiscal Requirements: Cmplete annual cntract and with BHCS. Cmplete annual Cst Recnciliatin/Settlement reprt. This annual reprt reflects an rganizatin s actual csts, revenues, and utilizatin fr a specific cntract perid. The submissin f this data is required by bth the Cunty and State Department f Health Care Services and represents the basis by which the Cunty, and ultimately the State, determines final net reimbursement amunts. This reprt is typically due Mid-September fr the previus fiscal year. BHCS Fiscal Cntract Manager will prvide TA n this requirement. Develp and submit an annual budget fr the FNL prgram. Invice BHCS Netwrk Office n a mnthly basis fr FNL csts. This can be included in the regular SUD Primary Preventin (PP) invice. Each invice must als include the service duratin reprt frm Cal OMS Pv t serve as evidence that hurs n the invice match the hurs listed in the Cal OMS database. Cmply with all BHCS audit prvisins. Yur BHCS Fiscal Cntract Manager will prvide additinal TA n this item. Applicatin Eligibility and Criteria Requirements: In rder t apply fr the FNL funding the applicant must: Be a current BHCS SUD Primary Preventin (PP) Prvider;

P a g e 5 Be currently using the Center fr Substance Abuse Preventin (CSAP) strategy f Envirnmental Preventin r prvide a descriptin f the rganizatin s capacity t implement envirnmental preventin strategies; Have current r relatively recent (within past tw years) yuth develpment experience; and Prvide a stand-alne budget and budget narrative fr the $30,000 r prvide a FNL budget and budget narrative that s incrprated int the agency s existing SUD preventin cntract.

P a g e 6 Attachment A FNL Radmap A Radmap chapter engages at least 8 yuth partners, utilizing yuth develpment principles and practices as a platfrm, t implement a Radmap prject prcess. The Radmap is a guide that takes yuth and adult partners thrugh a strategic preventin framewrk implementatin prcess, ver the curse f 1-2 years. An example f each main cmpnent f the Radmap prcess is utlined belw. Capacity Building During this phase, chapter members wrk tgether t learn abut ways t imprve their cmmunities and the lives f their peers. Chapter members learn new skills, ideas, and strategies t help them with their wrk. Assessment Planning Implementatin Evaluatin During this phase, chapter members will use their new skills t determine the rt causes f the prblems that they see in their cmmunity, s that they can chse slutins that will truly impact the prblem. Smetimes it is easy t get caught up with the idea f a prject that sunds fun r impressive, but if we dn t d research t find ut why the prblem exists, we may nt chse a prject that will impact the prblem we want t eliminate. During the planning phase, chapter members will use the infrmatin and data that they came up with during the assessment phase t chse a slutin that will nt nly address the prblem, but als fit with interests and abilities f the chapter members. It is imprtant that chapter members use a timeline t help with their planning prcess During this phase, chapter members will implement the slutin prject that they identified during their planning phase, utilizing their timeline as a guide. During this phase, chapter members will reflect upn the previus 5 phases t determine whether they achieved what they set ut t d. As a team they will determine their successes, areas f imprvement, and pprtunities t expand their ideas.
