5 . STEP 1 KNOW YOUR HEALTH Finding out how healthy you are is the first step to getting healthier. Vitality offers you a range of health assessments and checks in our Wellness Network and online. Doing these assessments can earn you thousand of Vitality points and a big percentage of your points can get accumulated at this stage. Vitality helps you to get healthier by giving you the knowledge, tool and motivation to Know your health and Improve your health, then access fantastic rewards. Not only does Vitality encourage the whole family to get healthier, research shows that being on Vitality lowers your healthcare costs more than non Vitality members.

6 REGISTRATION PROCESS The first step is to get you registered on the Discovery website with your own unique username and password. Go to Click on the registration icon. Follow the registration process. If the system replies with your details is not up to date with the Discovery system please dial The call center is very helpful and will update your details which will reflect within 24 hours. Note: All adult dependants over the age of 18 must register online to get their own username and password.

7 . THE ONLINE ASSESSMENTS Vitality Health Review 2000 points See your key health risks and get personal goals to improve them. Kids Vitality Health Review 2000 points per kid Know your children s health risks. This assessment will reflect on the parents profile if you have kids between 2-18 years of age. Mental Wellbeing Assessments 2500 points Get facts about your mental and emotional wellbeing. This assessment can be done twice a year 5 months apart. Online Vitality Fitness Assessment 1000 points Find out about your overall fitness and get some tips on how to improve it. Non smokers declaration 5000 points You can sign this declaration if you don t smoke or if you have stopped smoking in the last 6 months. After successful completion of the online assessments you and all the adult dependants on Vitality will have points each. Well Done!

8 DO A VITALITY CHECK TO FIND OUT HOW HEALTHY YOU ARE Earn thousands of Vitality points for learning about your health! The Vitality Health Check identifies your risk for chronic diseases of lifestyle by testing your blood pressure, glucose, cholesterol and weight. You and your family can complete the Vitality Health Check at any pharmacy ( Dis-chem or Clicks) in the Vitality Wellness Network, or at a Discovery Health Wellness Day or Discovery Wellness Centre. Vitality check cost is R135 (this is 2013 prices) If you are a Discovery health member your screening benefit will cover the total cost. Remember to take your Discovery health card with you. Weight assessment Earn 1000 Vitality points for just doing it and 4000 bonus points for being in acceptable range. Body mass index between 18.5 and 24.9 and a waist circumference of less than 102cm for men and less than 88cm for women Vitality Bonus points. Blood pressure Earn 500 Vitality points for just doing it and Vitality bonus points for being in a acceptable range. Reading of 140mmHg/90mmHg or less Glucose Earn 500 Vitality points for just doing it and Vitality bonus points for being in a acceptable range. Reading of less than 7.8 mmol/ Cholesterol Earn 500 Vitality points for just doing it and Vitality bonus points for being in a acceptable range. Reading of less than 5 mmol/l With these assessments you and your adult dependants can each earn up to points.

9 DO FURTHER HEALTH CHECKS AND EARN MORE POINTS. Prevention is always better than cure. Earn extra Vitality points for doing these gender, age, and lifestyle-specific health tests. Pap smear 2500 points (over 16 years of age) Paying for the Pap smear Discovery Health pays for the Pap smear on all Discovery Health Plans from your Screening Benefit. The consultation fee is paid from your day-to-day benefits. Mammogram 2500 points (over 40 years of age) Discovery Health pays for the Pap smear on all Discovery Health Plans from your Screening Benefit. The consultation fee is paid from your day-to-day benefits. Prostate screening 2500 points (over 50 years of age) Discovery Health pays for the Pap smear on all Discovery Health Plans from your Screening Benefit. The consultation fee is paid from your day-today benefits. Glaucoma screening 2500 points (over 40 years of age) Discovery Health pays for the test on all Discovery Health Plans from your day-to-day benefits, if you have funds available in your MSA. HIV test 5000 points (this can also be done as part of your Vitality check at the pharmacy) Discovery Health pays for the test on all Discovery Health Plans from the Screening Benefit,

10 Child Health Assessment Points Earn Vitality points if you take your child to a doctor in the Discovery GP Networker to a paediatrician for a Vitality Child Health Assessment. This in-depth assessment will tell you how healthy your child really is. If you have children older than 2 and younger than 18 years, we ll give you all the tools you need to give them a healthy head-start in life. The assessment consists of four components: 1. Your child s health history 2. A medical examination 3. Screening tests 4. Counselling Certain costs may apply Dental check-up Points Earn Vitality points if you take your child (between 2-18 years of age) for a dental check-up at any dentist. Certain costs may apply.

11 VACCINATIONS SPECIFIC Vitality looks after the health of the whole family and awards Vitality points for vaccinations specific to every stage of your life. Earn up to 7500 points depending on your current level of fitness. You can do this assessment twice a year 6 months apart which means you can earn up to points per year. Cost for this assessment is R 185 Earn Vitality 5000 points for assessing your diet and learn how to keep your plate balanced and health The Vitality Nutrition Assessment can be done at any dietitian in the Vitality Wellness Network or at a Wellness day. Cost for this assessment R 290.

12 COURSES THAT CAN EARN YOU MORE POINTS CPR Course earn Vitality points for doing CPR course with a CPR provider accredited to the Resuscitation Council of Southern Africa, SharpMinds, ER24 or Netcare 911. Antenatal classes Vitality points for going for antenatal classes to help prepare you for childbirth and motherhood.cost of these courses will differ depending on the service provider.
