4 CPA Membership Application References Membership is confined to businesses of good standing. In order to fast track your application the Association requires that each application be supported by two CPA members in your area. Please provide details of two local CPA members whose Principal will support your Membership application. You do not necessarily need to have a trading relationship with either but will be known to them. Your application will also be sent to all current CPA Council Members for their approval. If you do not know two CPA members please complete the section below under Applicant s Trade References. CPA MEMBER REFERENCES APPLICANT S TRADE REFERENCES In the event that you are unable to supply references from two local CPA members then your application for membership must be supported by two satisfactory trade references. Your application will be sent to all current CPA council members. It is a good indication of an applicant s credit worthiness to provide trade references from major public or private contractors or suppliers. Please provide the names of two trade references:

6 CPA Membership Application CPA Business Information and Subscription Levels CPA s membership benefits and services are detailed in the information pack form. The subscription year commences on 1 July. New members joining during a subscription year will pay pro rata, starting from 1 October each year. Your subscription to CPA s business and membership service is based on your total annual turnover (not just your plant hire turnover). Band Company Turnover Subscription (excl VAT) SB Less than 150, A Greater than 150,000 but less than 350, B Greater than 350,000 but less than 1.5m C Greater than 1.5m but less than 2.5m D Greater than 2.5m but less than 5m 1, E Greater than 5 but less than 7.5m 1, F Greater than 7.5m but less than 15m 2, G Greater than 15m 4, H n Plant-hire Corporate Member To process your application return this duly completed together with a cheque for ( 100 entrance deposit and 50 plus VAT administration fee). On acceptance into membership an invoice for your subscription will be raised. Please make cheques payable to: The Construction Plant-hire Association. The CPA aims to provide you with help when you need it, in matters which affect your business. CPA staff will become an extension of your business. Membership entitles you to contact CPA staff as often as you wish and obtain information, advice and assistance on Health & Safety Training matters, Contractual, Legal and Employment matters.
